These are called anticholinergic medications, and when you stack two or more together, the side effects multiply. Dry mouth, constipation, confusion, overheating, difficulty urinating, and dangerously fast heart rate all become more likely.

The FDA labeling specifically names these drug classes as ones that increase anticholinergic side effects when combined with dicyclomine:

  • Tricyclic antidepressants (like amitriptyline and nortriptyline)

  • Nitrates and nitrites (chest pain medications like nitroglycerin)
  • Sympathomimetic agents (decongestants and stimulants)

This matters because several of these are available without a prescription. Allergy pills, sleep aids, and cold medications containing antihistamines are easy to grab off the shelf without realizing they interact with dicyclomine.

Antacids

Antacids can interfere with how well your body absorbs dicyclomine. Taking them at the same time reduces the amount of medication that actually reaches your system, making it less effective. If you need both, separate them by at least one to two hours so the antacid doesn’t block absorption.

Potassium Chloride Tablets

This is a less obvious but important interaction. Dicyclomine slows the movement of your digestive tract, and that means any solid potassium supplement you swallow sits in one spot longer than it normally would. Prolonged contact between a potassium tablet and the lining of your stomach or intestine can cause ulcers, erosion, or other mucosal injuries. If you take potassium supplements in tablet or capsule form, your prescriber may need to switch you to a liquid formulation or adjust your regimen.

Alcohol

Dicyclomine on its own can cause drowsiness and blurred vision. Alcohol amplifies both of those effects. Even a moderate amount of alcohol while taking dicyclomine can impair your ability to drive, think clearly, or react quickly. The combination also increases the risk of dizziness and lightheadedness.

Conditions That Make Dicyclomine Unsafe

Certain health conditions are outright contraindications, meaning you should not take dicyclomine at all if you have them. These aren’t just caution flags; they’re situations where the drug can cause serious harm.

Gastrointestinal obstruction: Because dicyclomine slows gut motility, it can worsen or mask a bowel blockage. Severe ulcerative colitis: In large doses, dicyclomine can suppress intestinal movement enough to cause paralytic ileus, where the bowel essentially stops working. It can also trigger toxic megacolon, a life-threatening complication where the colon rapidly dilates and risks perforation. Milder cases of ulcerative colitis call for caution; severe cases rule out the drug entirely.

Unstable cardiovascular conditions, myasthenia gravis, and narrow-angle glaucoma are also listed as contraindications. Dicyclomine can raise eye pressure, worsen muscle weakness, and affect heart rate in ways that make these conditions significantly more dangerous.

Infants Under 6 Months

Dicyclomine is strictly contraindicated in babies younger than 6 months. Published case reports have documented serious respiratory symptoms in infants given the drug, including difficulty breathing, respiratory collapse, seizures, a drop in muscle tone, and in some cases, coma and death. The FDA labeling makes this one of the clearest age-based restrictions for the medication.

Nursing Mothers

Dicyclomine is also contraindicated during breastfeeding. The drug can pass into breast milk and potentially affect the infant, which is especially concerning given the respiratory risks already documented in young babies. If you’re breastfeeding and need treatment for gut spasms, alternative options exist that carry less risk for the infant.

Practical Tips for Staying Safe

The biggest risk with dicyclomine isn’t any single interaction. It’s the way anticholinergic effects accumulate without you noticing. You might be taking an allergy pill, a sleep aid, and dicyclomine, each one reasonable on its own, but together they create a heavy anticholinergic load. Signs that the combined effect is too strong include severe dry mouth, inability to urinate, confusion (especially in older adults), rapid heartbeat, and overheating because your body can’t sweat properly.

Before starting dicyclomine, make sure your pharmacist has a complete list of everything you take, including over-the-counter medications and supplements. Potassium tablets and antacids are easy to overlook because they feel routine, but both have specific, well-documented interactions with this drug.
