Ozempic pens come with 32-gauge, 4 mm needles included in the box. This is the thinnest and shortest needle available for any once-weekly injectable in its class, and it works for all Ozempic dose strengths (0.25 mg, 0.5 mg, 1 mg, and 2 mg). You don’t need to buy needles separately unless you run out or prefer a slightly different length.
The Included Needle: 32G, 4 mm
Every Ozempic pen ships with NovoFine Plus 32G 4 mm disposable needles. The “32G” refers to the gauge, which is the thickness of the needle. Higher gauge numbers mean thinner needles, and 32G is among the thinnest available for pen injectors. The 4 mm length is just enough to reach the layer of fat beneath your skin, where semaglutide needs to be delivered.
These tiny needles are designed for comfort. Research comparing needle gauges from 23G up to 32G found that thinner, shorter needles caused significantly fewer painful injections and less bleeding. Most people describe the 32G 4 mm needle as barely noticeable, especially when injecting into the abdomen or thigh.
Compatible Needle Sizes
If you need replacement needles or want a slightly different option, Ozempic pens are designed to work with NovoFine Plus and NovoFine disposable needles up to 8 mm in length. The two most common choices are:
- NovoFine Plus 32G, 4 mm: The same needle included with the pen. The thinnest option and the most comfortable for most people.
- NovoFine 32G, 6 mm: A slightly longer alternative. Some people with more body tissue at their injection site prefer the extra 2 mm of length for confidence that the medication is reaching the right depth.
For most people using a pen, 4 mm or 6 mm is all you need. Both are 32-gauge, so the thickness and comfort level are essentially the same. The only difference is how deep the needle reaches.
Ozempic pens also meet an international compatibility standard (ISO 11608-2), which means third-party pen needles that follow the same standard will physically fit. That said, Novo Nordisk specifically recommends sticking with NovoFine or NovoFine Plus needles for their pens.
Needles for Compounded Semaglutide Vials
If you’re drawing semaglutide from a vial with a syringe rather than using a prefilled Ozempic pen, the needle size is different. Syringes typically require longer needles in the range of 8 mm to 12 mm to properly reach subcutaneous tissue. Your prescriber or pharmacist will specify the right gauge and length based on the syringe type and injection site.
Buying Replacement Needles
Each Ozempic injection requires a fresh needle, and since the pen is used once weekly, the included needles usually last the life of the pen. If you do need extras, NovoFine Plus 32G 4 mm and NovoFine 32G 6 mm needles are sold in 100-count boxes at many pharmacies. Whether you need a prescription for pen needles depends on your state’s laws, so check with your pharmacy. In many places they’re available over the counter.
Why You Should Use a New Needle Every Time
Pen needles are single-use for several practical reasons. Each needle comes coated with a thin layer of silicone that helps it glide through skin smoothly. That coating wears off after one use, making a second injection noticeably more painful. The tip also dulls and can bend after a single puncture. In rare cases, a damaged needle tip can actually break off under the skin.
Beyond comfort, reused needles pick up bacteria even when only one person uses them, raising the risk of infection at the injection site. A reused needle can also develop tiny blockages that interfere with how much medication is actually delivered, potentially affecting your dose accuracy.
Disposing of Used Needles Safely
After each injection, remove the needle from the pen and drop it into a sharps disposal container right away. You can buy FDA-cleared sharps containers at most pharmacies, or use a sturdy, puncture-resistant household container like a heavy plastic laundry detergent bottle. Fill the container no more than three-quarters full, then follow your local community guidelines for disposal. Many pharmacies, hospitals, and waste management programs accept full sharps containers. Keeping the container out of reach of children and pets is essential.
