A total testosterone level below 300 ng/dL is the widely accepted cutoff for low testosterone in adult men. The American Urological Association uses this number as its diagnostic threshold, and most labs flag results below it as abnormal. But a single number on a lab report doesn’t tell the whole story: a diagnosis requires both low levels and symptoms.
The Clinical Cutoff: 300 ng/dL
The normal range for total testosterone in healthy, nonobese young men (ages 19 to 39) runs from about 264 to 916 ng/dL. That lower bound of 264 ng/dL comes from the Endocrine Society’s harmonized reference range, representing the bottom 2.5% of the population. In practice, though, most clinicians round up slightly and use 300 ng/dL as the working cutoff for low testosterone.
Readings between roughly 200 and 400 ng/dL are considered a borderline zone. If your result lands here, the picture gets murkier. A man at 280 ng/dL with fatigue, low sex drive, and erectile problems looks very different from a man at 280 ng/dL who feels fine. That’s why a low number alone isn’t a diagnosis. The clinical term, male hypogonadism, applies only when low lab values come paired with symptoms.
Why One Test Isn’t Enough
Testosterone fluctuates throughout the day, peaking in the early morning and dipping by afternoon. A blood draw at 2 p.m. can read significantly lower than one taken at 8 a.m. A bad night of sleep, a heavy drinking session, or acute stress can all temporarily suppress your levels.
For this reason, guidelines require at least two total testosterone measurements taken on separate days, both drawn in the early morning, before a diagnosis is made. If your first result comes back low, a second test on a different morning confirms whether the reading reflects your actual baseline or just a bad day.
How Testosterone Levels Change With Age
Testosterone naturally declines as men get older, typically dropping about 1 to 2% per year starting around age 30. The ranges shift accordingly:
- Age 40 to 49: 252 to 916 ng/dL
- Age 50 to 59: 215 to 878 ng/dL
- Age 60 to 69: 196 to 859 ng/dL
- Age 70 to 79: 156 to 819 ng/dL
Notice that the lower end of normal keeps dropping with each decade. A 70-year-old man at 200 ng/dL is technically within the age-adjusted range, even though that same number would be flagged as low in a 30-year-old. This is part of why the 300 ng/dL cutoff is a guideline rather than an absolute rule. Context matters: your age, your symptoms, and your overall health all factor into the interpretation.
Total Testosterone vs. Free Testosterone
Most of the testosterone in your blood is bound to proteins, primarily one called sex hormone-binding globulin (SHBG). Only about 2 to 3% circulates freely and is available for your body to use. This “free testosterone” is what actually enters cells and does the work.
Free testosterone becomes especially important when your total testosterone is borderline or when conditions are present that raise SHBG levels (aging, liver disease, and certain medications can all do this). You might have a total testosterone of 350 ng/dL, which looks adequate, but if most of it is bound up, your free testosterone could be low enough to cause symptoms. The normal range for calculated free testosterone is roughly 50 to 200 pg/mL. If your total testosterone is in that gray zone and you have symptoms, free testosterone testing can clarify the picture.
Symptoms That Point to Low Testosterone
Sexual symptoms are the most specific indicators. Low libido, loss of morning erections, and difficulty getting or maintaining erections are the hallmark signs. These symptoms correlate more reliably with low testosterone than other complaints that could have multiple causes.
Other symptoms that strongly suggest hypogonadism include loss of body hair (particularly armpit and pubic hair), shrinking testicles, hot flashes, and low or absent sperm counts. Beyond those, men with low testosterone commonly experience depressed mood, difficulty concentrating, memory problems, increased body fat, loss of muscle mass and strength, reduced endurance, and breast tissue enlargement.
The tricky part is that many of these symptoms overlap with other conditions. Depression, sleep apnea, thyroid problems, and anemia can all produce fatigue, low mood, weight gain, and reduced sex drive. This overlap is exactly why a blood test is essential rather than relying on symptoms alone, and why a thorough evaluation matters before jumping to a testosterone diagnosis.
What Causes Low Testosterone
Low testosterone falls into two categories based on where the problem originates. In primary hypogonadism, the testicles themselves aren’t producing enough testosterone. This can result from injury, infection, chemotherapy, radiation, or genetic conditions. The signaling from the brain is working fine, but the testicles can’t respond adequately.
In secondary hypogonadism, the problem is upstream. The hypothalamus and pituitary gland in the brain normally send hormonal signals that tell the testicles to produce testosterone. When something disrupts that signaling chain, production drops even though the testicles are physically capable. Pituitary tumors, obesity, opioid use, and certain chronic illnesses can all cause this type. Distinguishing between the two matters because it changes what treatment looks like and whether an underlying condition needs to be addressed first.
What Happens If Low Testosterone Goes Untreated
Beyond the day-to-day symptoms, chronically low testosterone carries longer-term health consequences. Testosterone plays a direct role in maintaining bone density, so prolonged deficiency increases the risk of osteoporosis and fractures. It also contributes to how your body distributes fat and maintains muscle, so untreated low testosterone tends to shift body composition toward more fat and less lean mass over time. The sexual symptoms generally don’t resolve on their own if the underlying cause persists.
The mood and cognitive effects can be particularly disruptive. Men with untreated hypogonadism report persistent low mood, difficulty with focus and memory, and reduced motivation. These changes often develop gradually enough that they’re easy to attribute to aging or stress rather than a treatable hormonal deficit.
How Testing and Diagnosis Work
If you suspect low testosterone, the process starts with a morning blood draw for total testosterone. If that first result comes back below 300 ng/dL, a second test on a different morning confirms it. Your provider will likely also check free testosterone, SHBG, and hormones from the pituitary gland to determine whether the issue is primary or secondary.
If treatment is recommended, the goal is to bring levels into the mid-normal range of about 450 to 600 ng/dL. That target uses the lowest effective dose rather than pushing levels to the top of the range. Treatment options vary, but the key thing to understand is that testosterone therapy can affect fertility by suppressing sperm production. Men who want to have children need to discuss this before starting any treatment.
