For a hornet sting, the immediate steps are simple: get away from the area, wash the sting site with soap and water, apply ice, and take an over-the-counter pain reliever if needed. Most hornet stings cause intense but short-lived pain that resolves within hours. The critical exception is a severe allergic reaction, which requires emergency help right away.
Immediate First Aid Steps
First, move away from where you were stung. Hornets can sting multiple times (unlike honeybees, they keep their stinger), and disturbed hornets may recruit others from the nest. Once you’re in a safe spot, gently wash the sting with soap and water to reduce the chance of infection.
Apply a cold pack or ice wrapped in a cloth to the area for 10 to 15 minutes at a time. This constricts blood vessels near the surface, which slows the spread of venom into surrounding tissue and reduces swelling. Elevating the limb, if the sting is on an arm or leg, also helps limit swelling.
Unlike honeybees, hornets almost never leave a stinger behind. If you do spot one embedded in the skin, scrape it out sideways with a flat edge like a credit card rather than squeezing it with tweezers, which can push more venom into the wound.
Managing Pain, Swelling, and Itch
Over-the-counter pain relievers like ibuprofen or acetaminophen can take the edge off. For itching, an oral antihistamine works well. Options that cause drowsiness include diphenhydramine (Benadryl); non-drowsy alternatives include cetirizine (Zyrtec), loratadine (Claritin), or fexofenadine (Allegra). A hydrocortisone cream applied directly to the sting can also calm localized itch and redness.
Avoid scratching. Broken skin over the sting site opens the door to bacteria, and an itchy sting that you repeatedly scratch is the most common path to infection.
Why Hornet Stings Hurt So Much
Hornet venom is a cocktail designed to cause pain and damage tissue. Its key components include enzymes that break down cell membranes, which is why the area around the sting swells and turns red so quickly. The venom also contains proteins that trigger your immune system to release histamine, the same chemical behind allergy symptoms. That histamine response is what causes the itching, warmth, and puffiness around the wound.
Because hornets are larger than most wasps and bees, they inject a relatively large dose of venom per sting. This is why a single hornet sting tends to hurt more and swell more than a typical bee sting, even in people with no allergy.
What About Home Remedies?
You may have heard that baking soda paste helps stings. That remedy is better suited to bee stings, where the venom is acidic and baking soda (a base) can help neutralize it. Hornet and wasp venom is alkaline, so baking soda won’t do much. Vinegar, which is acidic, is a better match for neutralizing the alkaline compounds in hornet venom and may ease pain somewhat when dabbed on the sting.
That said, ice and a pain reliever are more reliably effective than any kitchen remedy. If you want to try vinegar, soak a cotton ball and hold it on the sting for a few minutes. It won’t hurt, and some people find it helpful.
Normal Healing Timeline
A typical localized reaction, meaning redness, swelling, and pain right around the sting, usually fades within a few hours. It can sometimes linger for a couple of days, particularly if the sting is on a sensitive area like the face or a finger.
Some people develop what’s called a large local reaction: the swelling expands well beyond the sting site, sometimes covering an entire forearm or calf. These reactions typically start 6 to 12 hours after the sting, peak over the next day or two, and take 5 to 10 days to fully resolve. A large local reaction looks alarming but isn’t the same thing as an allergic emergency. It’s an exaggerated inflammatory response, not anaphylaxis.
In rare cases, a delayed reaction called serum sickness can appear 6 days to 2 weeks after the sting, causing joint pain, fever, or a rash. These symptoms generally resolve within days to a few weeks.
Signs of a Dangerous Allergic Reaction
Anaphylaxis is the reason hornet stings can be life-threatening, and it develops fast, usually within minutes. The warning signs include:
- Breathing trouble: throat tightness, swollen tongue, wheezing
- Skin changes beyond the sting site: widespread hives, flushing, or sudden paleness
- Cardiovascular signs: a weak or rapid pulse, dizziness, fainting
- Gastrointestinal symptoms: nausea, vomiting, or diarrhea shortly after the sting
If any of these appear, call emergency services immediately. If the person carries an epinephrine auto-injector, use it right away. Even if symptoms improve after the injection, an emergency room visit is still necessary because symptoms can return in a second wave, sometimes hours later, without any additional exposure to venom.
Multiple Stings and Toxic Reactions
Even without an allergy, a large number of stings can cause a toxic reaction from the sheer volume of venom. The general threshold is about 10 stings per pound of body weight. For an average adult, that means the body can typically handle over 1,000 stings before a toxic dose becomes dangerous. For a small child, the threshold drops to around 500. In practice, getting stung dozens of times at once (which can happen if you disturb a nest) warrants medical attention regardless, especially in children or older adults.
Watching for Infection
A sting that’s healing normally will gradually become less red, less swollen, and less painful each day. An infected sting does the opposite. Watch for increasing warmth, expanding redness, pus, or the skin becoming more painful rather than less over the following days. Fever or chills alongside a worsening sting site suggest a bacterial skin infection that needs medical treatment.
If you notice a growing red rash around the sting but have no fever, it’s still worth getting it checked within 24 hours. If a fever develops alongside the swelling, that moves the timeline up to right away.
