Night sweats range from a minor nuisance to a sign of something that needs medical attention, so the first step is figuring out which category yours fall into. For most people, a combination of sleep environment changes, trigger avoidance, and sometimes medication can dramatically reduce how often they wake up drenched. Here’s what actually works and when to dig deeper.

Rule Out the Serious Stuff First

Night sweats have a long list of possible causes, and most of them are benign. But a handful of combinations should prompt a doctor’s visit sooner rather than later. Night sweats paired with unintentional weight loss, persistent fever, bone pain, a new cough, or swollen lymph nodes can signal infections like tuberculosis or HIV, or blood cancers like lymphoma and leukemia. Hyperthyroidism, certain autoimmune disorders, and heart valve infections (endocarditis) also cause drenching sweats at night.

If your night sweats appeared suddenly, happen most nights, and come with any of those other symptoms, get evaluated. If they’ve been happening on and off for a while with no other changes, the cause is more likely hormonal, medication-related, or environmental.

Check Your Medications

Roughly 20% of people taking antidepressants experience excessive sweating, often in episodic bursts concentrated around the scalp, face, neck, and chest. Beyond temperature, your bedding matters. Standard cotton sheets absorb moisture and hold it against your skin, which makes things worse. Moisture-wicking sheets made from performance fabrics (similar to athletic wear) pull sweat away from your body and dry faster. Look for bedding marketed specifically for night sweats with features like enhanced airflow and fast-drying construction. Avoid non-breathable synthetic materials, which trap heat. A cooling mattress pad or pillow with gel layers can also help regulate surface temperature throughout the night.

Some practical moves that cost nothing: sleep with a fan pointed at your upper body, keep a cold water bottle on the nightstand, and use layered lightweight blankets instead of one heavy comforter so you can adjust coverage without fully waking up.

Cut the Triggers

Alcohol is one of the most reliable night sweat triggers. It widens blood vessels in your skin, raises your heart rate, and increases perspiration. If you’re physically dependent on alcohol, withdrawal itself can cause severe night sweats. Even moderate drinking in the evening can be enough to cause a soaking episode a few hours into sleep. Some people also have a genetic mutation that prevents their body from fully breaking down alcohol’s byproducts, making the flushing and sweating response even more intense.

Spicy food close to bedtime raises core body temperature and can provoke sweating in susceptible people. Caffeine, particularly in the afternoon or evening, is another common aggravator. Keeping a simple log of what you ate and drank on nights when sweats are worse versus better can reveal your personal triggers within a couple of weeks.

Treatments That Work for Menopausal Sweats

Hormone therapy remains the single most effective treatment for menopausal hot flashes and night sweats, and current guidelines support its use in women within 10 years of their final menstrual period who don’t have contraindications like estrogen-sensitive cancers or cardiovascular disease.

For those who can’t or prefer not to take hormones, several non-hormonal options have solid evidence behind them. The North American Menopause Society recommends cognitive-behavioral therapy and clinical hypnosis (both supported by strong evidence), along with several prescription options. Among medications, fezolinetant is the first drug designed to target the specific brain pathway responsible for menopausal temperature dysregulation. In clinical trials, a daily dose reduced the frequency of moderate to severe hot flashes and night sweats at rates comparable to hormone therapy and significantly better than older non-hormonal options.

Gabapentin, often taken at bedtime, reduces hot flash frequency by about 54% compared to placebo. Certain antidepressants at low doses (lower than what’s prescribed for depression) reduce hot flashes by 24% to 69%. Oxybutynin, a medication originally developed for overactive bladder, has shown reductions of 70% to 86% in hot flash frequency in clinical trials.

What the evidence doesn’t support: herbal supplements, soy extracts, acupuncture, yoga, paced breathing, and most over-the-counter “menopause relief” products. The North American Menopause Society specifically recommends against these based on the available data.

Non-Menopausal Night Sweats

If menopause isn’t the cause, treatment depends on what is. Sleep apnea is an underrecognized trigger. People with obstructive sleep apnea experience surges in stress hormones during breathing pauses, which can cause sweating. Treating the apnea (typically with a CPAP device) often resolves the sweats. If you snore heavily, wake up gasping, or feel exhausted despite a full night’s sleep, a sleep study is worth pursuing.

Anxiety disorders activate your body’s fight-or-flight system, which includes sweating. For anxiety-driven night sweats, the sweats tend to improve as the anxiety is treated, whether through therapy, medication, or both.

Infections cause sweats because your immune system raises your body temperature to fight off pathogens, and sweating is how your body cools back down. These sweats resolve when the infection is treated. Chronic infections like tuberculosis or HIV can cause night sweats that persist for weeks or months before other symptoms become obvious.

What to Try Tonight

If you’re looking for immediate relief while sorting out the bigger picture, layer these strategies together:

  • Drop the thermostat to 65°F or lower and point a fan at your bed.
  • Switch your sheets to moisture-wicking fabric and ditch the heavy duvet for layered lightweight blankets.
  • Skip alcohol and spicy food for a week and note whether the sweats improve.
  • Wear lightweight, loose sleepwear made from breathable fabric, or sleep without it.
  • Keep cold water and a damp washcloth on the nightstand for quick relief when you wake up sweating.

These changes won’t cure an underlying medical condition, but they can meaningfully reduce how often you wake up soaked and how long it takes to fall back asleep. If the sweats persist despite environmental changes, or if they’re accompanied by weight loss, fever, or other new symptoms, that’s the point where targeted testing and treatment become important.
