Most expired diabetic supplies should be disposed of properly, but some may still be usable in a pinch, and others can be donated or recycled depending on their condition. What you do depends on the type of supply, how far past expiration it is, and how it was stored. Here’s a breakdown for each category.
Expired Insulin: Use With Caution or Discard
Insulin is required to retain at least 95% of its potency through its printed expiration date. After that, potency gradually declines, and the rate depends heavily on how the insulin was stored. At room temperature (around 77°F), insulin loses less than 1% of its potency per month. Refrigerated insulin loses less than 0.1% per month. So a vial that’s a month or two past expiration and was stored in the fridge is likely still functional, though there’s no guarantee.
The real danger comes from insulin that was exposed to heat or freezing. There are documented cases of diabetic ketoacidosis (a life-threatening emergency) caused by insulin that had been left in the sun or accidentally frozen in a hotel refrigerator. Temperature damage can destroy insulin’s effectiveness entirely, regardless of what the expiration date says.
Before using any insulin, inspect it visually. Clear insulin (like rapid-acting formulations) should remain completely clear. If you see clumps, particles, or a frosty film inside the vial or pen, discard it. These are signs of protein breakdown, and that insulin will not work reliably. Cloudy insulin types (like NPH) should look uniformly milky after gentle mixing. Any chunks or unusual texture means it’s degraded. The general rule: if it looks different from when you opened it, throw it out.
How to Safely Dispose of Insulin and Sharps
Insulin pens and syringes contain needles, which means they’re classified as sharps. Never toss loose needles into household trash or recycling bins, and never flush them. Place used or expired pens, syringes, and lancets into an FDA-cleared sharps disposal container, available at pharmacies, medical supply stores, and online. If you don’t have one, a heavy-duty plastic household container like a laundry detergent jug works as a substitute. Just make sure it has a tight-fitting lid and puncture-resistant sides.
Many pharmacies and hospitals accept full sharps containers. Some communities also run mail-back programs or have drop-off sites. Check with your local waste management office for specific options in your area.
Expired Test Strips: Expect Inaccurate Readings
Glucose test strips work through enzymes coated on the strip’s tip that react with blood sugar and convert it into an electrical signal your meter can read. Those enzymes break down over time, especially with exposure to humidity or temperature swings. A strip that’s a few months past expiration may still give a reading close to accurate. One informal comparison found that strips seven months past expiration read 101 mg/dL versus 98 mg/dL on current strips from the same brand, using the same blood sample.
But the further past expiration you go, the less reliable the reading becomes. Strips that are two or more years expired are extremely likely to give inaccurate results. If you’re using expired strips in a pinch, treat the numbers as rough estimates rather than precise values. For day-to-day insulin dosing decisions, inaccurate readings can lead to dangerous over- or under-dosing, so expired strips shouldn’t be your primary tool.
Expired test strips can go in the regular household trash. They don’t contain hazardous materials.
Expired CGM Sensors: Surprisingly Durable
Continuous glucose monitor sensors appear to hold up better past expiration than you might expect. A study tested 20 sensors ranging from 3 to 18 months past their expiry date and found that they continued to function reliably, with acceptable accuracy maintained throughout. The sensors worked for at least 7 days after insertion, which is their normal usage window. This doesn’t mean manufacturers recommend using expired sensors, and your device may reject an expired sensor based on the lot code. But from a functionality standpoint, CGM sensors seem to have a reasonable margin built into their expiration dates.
Expired Glucagon Kits: Replace, Don’t Risk It
Glucagon emergency kits are used to treat severe low blood sugar episodes when someone can’t eat or drink. The FDA’s guidance is straightforward: do not use glucagon if the expiration date has passed. Because glucagon is used in emergencies where a person may be unconscious, this is one supply where reduced potency could have serious consequences. Replace expired kits promptly and keep the new one in a place where family members or caregivers know to find it.
Donating Unexpired Supplies
If your supplies haven’t expired yet but you no longer need them (maybe you switched medications or devices), donation is the best option. The American Diabetes Association recommends Insulin for Life (iflusa.org), which accepts insulin vials and pens, syringes, glucagon, insulin pump supplies, A1C test kits, and more. You just pay for shipping to their center.
Most donation programs require supplies to be unopened and unexpired. Local animal shelters, health departments, relief agencies, and religious institutions sometimes run their own collection programs with varying requirements. It’s worth calling around, because diabetes supplies are expensive and many people go without.
Once supplies have passed their expiration date, donation programs typically won’t accept them. At that point, disposal is your only option.
Recycling Insulin Pumps and CGM Hardware
Insulin pumps, CGM sensors, and their components contain batteries and electronic parts that shouldn’t go in regular trash if avoidable. A few manufacturer-specific programs exist. Abbott has launched a sensor kit take-back pilot program in the United States, where users can return used sensors to be sent to specialized waste-to-energy facilities. Insulet runs a pod take-back program, though as of now it’s only available to Massachusetts residents.
If your manufacturer doesn’t offer a take-back option, check for local e-waste recycling programs that accept small medical electronics. Many community recycling centers have bins for devices with lithium batteries. For older insulin pumps that are out of warranty and no longer in use, some diabetes organizations accept them for parts or refurbishment in developing countries, though availability varies.

- Insulin (expired): Inspect visually. If it looks normal and was stored properly, it may still work short-term but with reduced reliability. Dispose of pens and syringes in a sharps container.
