What to Look for in Prenatal Vitamins: A Label Checklist

The best prenatal vitamins deliver adequate folate, iron, iodine, choline, and vitamin D, and ideally carry a third-party testing seal. Beyond that short list, the form of each nutrient matters more than the brand name on the bottle. Here’s what to prioritize when comparing labels.

Start Before You’re Pregnant

ACOG recommends taking a prenatal vitamin with at least 400 micrograms of folic acid starting at least one month before conception and continuing through the first 12 weeks of pregnancy. The neural tube, which becomes your baby’s brain and spinal cord, forms in the earliest weeks, often before you know you’re pregnant. If you’ve previously had a pregnancy affected by a neural tube defect, the recommendation jumps to 4,000 micrograms daily, beginning three months before conception.

Folate: Check Which Form

Most prenatal vitamins contain folic acid, the synthetic version of folate. Your body has to convert folic acid through several steps before it becomes L-methylfolate, the form your cells actually use. That conversion works fine for most people, but a common genetic variation in the MTHFR enzyme significantly reduces how much active folate you end up with. Estimates suggest roughly 40% of the population carries at least one copy of this variant.

Prenatal vitamins that list L-methylfolate (sometimes labeled as “5-MTHF” or “methylfolate”) skip the conversion process entirely. If you know you have an MTHFR variant, or you’ve never been tested and want to cover your bases, choosing a prenatal with methylfolate is a reasonable move. Either way, look for at least 400 to 800 micrograms of total folate on the label.

Iron: The Nutrient That Causes the Most Problems

Iron supports the massive increase in blood volume during pregnancy and helps prevent anemia. Most prenatal vitamins contain 27 milligrams, which matches the pregnancy RDA. The catch is that iron is also the ingredient most likely to cause nausea, constipation, and stomach cramps, which leads many women to stop taking their prenatal altogether.

The type of iron in the formula makes a real difference. Ferrous sulfate is the most common and cheapest form, but it’s also the hardest on your stomach. A study published in the American Journal of Obstetrics & Gynecology compared iron formulations in pregnant women and found that ferrous bisglycinate (sometimes called “chelated iron” or “iron bisglycinate”) produced a 98% compliance rate versus 74% for other forms. It was also more effective at raising hemoglobin levels, with 81.3% of patients hitting the treatment target compared to 68.4% with other formulations. If you’ve struggled with iron supplements before, look for bisglycinate or glycinate on the ingredient list.

Iodine: Quietly Critical

Iodine is easy to overlook, but it’s essential for your thyroid to produce the hormones that drive fetal brain development. Pregnant women need about 50% more iodine than usual, with most guidelines setting the target at 220 micrograms per day. Severe deficiency during pregnancy can cause irreversible cognitive impairment in children. A meta-analysis of studies in iodine-deficient regions found that children born to unsupplemented mothers scored an average of 8.7 IQ points lower than those whose mothers received iodine during pregnancy.

Not all prenatal vitamins include iodine. Check the label for at least 150 micrograms, which, combined with iodized salt and dairy in your diet, should get you to the target.

Choline: The Nutrient Most Prenatals Skimp On

Choline plays a direct role in fetal brain development and neural tube closure. The recommended intake during pregnancy is 450 milligrams per day. The problem is that choline is bulky, so most prenatal vitamins either leave it out entirely or include a token amount, sometimes as little as 25 to 55 milligrams. If your prenatal doesn’t cover the full 450 milligrams (and it probably won’t), you’ll need to make up the difference through food. Eggs are the richest common source, with two large eggs providing roughly 300 milligrams. Beef liver, salmon, and soybeans are other good options, or you can add a standalone choline supplement.

Vitamin D: Most Formulas Cover This

The RDA for vitamin D during pregnancy is 600 IU (15 micrograms) per day. Most prenatal vitamins meet or exceed this amount, so it’s rarely a gap. The safe upper limit is set at 4,000 IU per day. If your blood levels are low (something your provider can test with a simple blood draw), you may need a separate supplement on top of your prenatal, but the standard 600 IU is a solid baseline to confirm on the label.

DHA for Brain and Eye Development

DHA, an omega-3 fatty acid, supports fetal brain and retinal development, particularly in the third trimester when the baby’s brain grows rapidly. Most health organizations recommend at least 200 to 300 milligrams of DHA per day during pregnancy. Some prenatal vitamins include DHA in a combined softgel, while others sell it as a separate capsule. If your prenatal doesn’t contain DHA, a standalone fish oil or algae-based omega-3 supplement can fill the gap. Algae-based options are a good choice if fish oil triggers nausea or if you follow a vegetarian diet.

Gummies vs. Capsules vs. Tablets

Gummy prenatals are popular because they’re easier to take, especially during first-trimester nausea. But they come with a significant trade-off: no gummy prenatal on the market contains iron. The mineral’s taste and stability make it incompatible with the gummy format. Most gummy formulas also contain less choline and calcium than their capsule or tablet equivalents. If you choose a gummy, plan on supplementing iron separately.

Tablets are the most compact and affordable option but can be hard to swallow and sometimes cause more stomach upset. Capsules (especially those designed to be taken as two or three smaller pills per day rather than one large one) tend to be gentler on the stomach and allow for better nutrient absorption because the dose is spread across the day.

Third-Party Testing Seals

Dietary supplements in the United States aren’t required to prove their contents match what’s on the label before they hit store shelves. Third-party certification fills that gap. The two most recognized seals are NSF International and USP (United States Pharmacopeia). NSF’s certification program tests for three things: that what’s on the label is actually in the bottle, that the formulation doesn’t contain harmful contaminants, and that no undeclared ingredients are present. They also conduct annual audits and periodic retesting of certified products.

A third-party seal doesn’t guarantee a product is the best choice for you nutritionally, but it does confirm you’re actually getting what you’re paying for. Given that some supplement audits have found products containing significantly more or less of a nutrient than listed, this matters.
