The most effective thing you can put on toenail fungus depends on how severe the infection is. Mild cases sometimes respond to over-the-counter antifungal creams or ointments, while moderate to severe infections typically need a prescription topical solution or oral medication. Regardless of what you use, toenail fungus takes 12 to 18 months to fully grow out, so patience is part of every treatment plan.
Over-the-Counter Antifungal Products
Drugstore antifungal products come as creams, ointments, sprays, and liquids. The three active ingredients worth looking for are terbinafine (sold as Lamisil), clotrimazole (Lotrimin), and tolnaftate (Tinactin). These are the same ingredients used to treat athlete’s foot, and they work best on early or mild toenail infections where the discoloration hasn’t spread across the entire nail.
OTC products have a real limitation: toenails are thick and dense, so creams and sprays often can’t penetrate deeply enough to reach the fungus living underneath. If you’ve been applying an OTC product for two to three months without seeing any improvement, the infection likely needs something stronger.
Prescription Topical Solutions
Prescription nail treatments are formulated specifically to penetrate the nail plate, which gives them a meaningful edge over drugstore creams. Three are commonly prescribed, and their cure rates vary quite a bit.
Efinaconazole 10% solution has the strongest track record among topicals, with complete cure rates of 15% to 18% in clinical trials. Tavaborole 5% solution falls in the range of 6.5% to 9.1% for complete cure, though about 15% to 18% of patients achieved nails that were completely or almost completely clear. Ciclopirox 8% nail lacquer, the oldest of the three, has a complete cure rate of about 7%.
Those numbers sound low, but “complete cure” in clinical trials means the nail looks entirely normal and lab tests confirm the fungus is gone. Many more patients see partial improvement, with less discoloration and thinner, healthier-looking nails. Prescription topicals are applied directly to the nail daily, and treatment courses typically run for 48 weeks.
Preparing Your Nail for Better Results
Whatever you put on the nail works better if the medication can actually reach the fungus. Thick, crumbly nails act as a barrier. Dermatologists often file down the surface of the nail before starting topical treatment so the product can penetrate more deeply.
For very thick nails, a prescription cream or ointment containing 40% urea can soften the nail over time, improving how well topical antifungals absorb. Studies show that softening thick nails before applying medication noticeably improves treatment outcomes. One specific type of infection, where the fungus grows on top of the nail as white patches, can sometimes be treated by scraping off the infected surface and applying antifungal medication directly. That scraping should be done by a dermatologist rather than at home.
Oral Antifungal Medication
For moderate to severe toenail fungus, oral medication is the most effective option. It works from the inside out, reaching the fungus through the bloodstream and new nail growth rather than trying to soak through a thick nail.
Terbinafine taken daily for three months is the standard treatment. In head-to-head trials, it produced a fungal cure rate of about 81%, compared to 63% for the alternative oral option, itraconazole. Clinical cure, meaning the nail also looks normal, occurred in roughly 40% to 45% of patients on continuous terbinafine.
Oral antifungals do require some monitoring. Your doctor will likely order a blood test before you start and possibly during treatment to check liver enzyme levels. In a study of nearly 1,000 patients, only about 2.4% developed any liver enzyme elevation during treatment, and nearly all of those were mild and resolved on their own or after stopping the medication. Serious liver problems are rare, but the blood work is a reasonable precaution.
Home Remedies That Have Some Evidence
Vicks VapoRub is the one home remedy with actual clinical data behind it. In a published case series, 18 patients applied it to infected toenails daily for 48 weeks. About 28% achieved a full cure with negative lab tests, while another 56% had partial clearing. The average area of nail affected shrank from 63% to 41%. It’s not as effective as prescription treatment, but for someone with a mild case who wants to try something inexpensive first, the results are better than doing nothing.
Tea tree oil is frequently recommended online, but the evidence is thin. The Mayo Clinic notes that research hasn’t shown tea tree oil to be effective for toenail fungus. One small study found pure (100%) tea tree oil helped a small number of people, but studies using diluted tea tree oil showed no benefit.
Laser Treatment
Several types of laser devices are cleared for treating toenail fungus, including Nd:YAG lasers and newer dual-diode systems. In one study of a non-thermal laser, 67% of treated nails achieved meaningful clear nail growth over six months, and 89% showed at least some increase in clear nail area. Laser treatment is typically done in a dermatologist’s office over multiple sessions.
The catch is cost. Laser treatment is rarely covered by insurance, can run several hundred dollars per session, and the evidence base is still smaller than what exists for oral or topical medications. It’s most often considered when someone can’t take oral antifungals or hasn’t responded to other treatments.
Why Treatment Takes So Long
Toenails grow slowly, roughly 1 to 2 millimeters per month. Even after the fungus is killed, you’re waiting for an entirely new, healthy nail to replace the damaged one. That process takes 12 to 18 months for a big toenail. During that time, the nail will look partially affected as the old growth gradually gets trimmed away and new clear nail emerges from the base.
This timeline is why consistency matters so much. Stopping a topical treatment early because the nail still looks bad is one of the most common reasons for treatment failure. The medication needs to stay on the nail long enough to keep the fungus suppressed while the new nail grows in. Reinfection is also common, especially if the conditions that caused the original infection (warm, damp shoes, gym showers, minor nail injuries) haven’t changed.
