“Adrenal fatigue” is not a recognized medical diagnosis, and no supplement has been approved to treat it. That said, the symptoms people describe, including persistent tiredness, brain fog, salt and sugar cravings, and trouble sleeping, are very real. The challenge is figuring out what’s actually causing them and what, if anything, is worth taking. Here’s what the evidence says about the most commonly recommended supplements, along with what to watch out for.
Why “Adrenal Fatigue” Isn’t a Medical Diagnosis
The idea behind adrenal fatigue is that chronic stress wears out your adrenal glands until they can no longer produce enough cortisol. It’s an intuitive concept, but no scientific proof exists to support it as a true medical condition. The Endocrine Society, the largest organization of hormone specialists in the world, has stated this clearly. There is no validated test for it either. Blood and saliva cortisol tests marketed for adrenal fatigue are not based on good scientific evidence, and their results can be misleading.
This matters because the symptoms commonly attributed to adrenal fatigue, such as chronic tiredness, muscle weakness, low mood, salt cravings, and difficulty waking up, overlap with several conditions that do have effective treatments. These include thyroid disorders, depression, sleep apnea, iron deficiency, and true adrenal insufficiency (Addison’s disease). If you skip proper evaluation and go straight to supplements, you risk missing something treatable.
Adrenal Insufficiency Is a Different Condition
True adrenal insufficiency is a serious, diagnosable condition where the adrenal glands genuinely cannot produce enough hormones. Its symptoms include long-lasting fatigue, muscle weakness, weight loss, low blood pressure that drops when you stand up, abdominal pain, and craving salty foods. People with Addison’s disease may also notice darkening of their skin, especially on scars, skin folds, elbows, knees, and knuckles.
Because these symptoms come on slowly, they’re often overlooked or confused with other problems. A standard blood test measuring cortisol levels and an ACTH stimulation test can diagnose it. If your fatigue is severe and accompanied by weight loss, dizziness when standing, or skin changes, getting tested for adrenal insufficiency is a worthwhile step before reaching for any supplement.

Ashwagandha
Ashwagandha is the most popular adaptogen marketed for stress and cortisol support, and it has more clinical trial data behind it than most herbal supplements in this category. Multiple studies have found that doses of 225 to 600 mg per day of standardized root extract can reduce self-reported stress and lower cortisol levels compared to placebo. In one trial, even a relatively low dose of 225 mg per day was enough to lower salivary cortisol within 30 days. Another 90-day study using 300 mg daily showed improvements in both stress scores and sleep quality. Benefits appear to be more consistent at 500 to 600 mg per day.
Ashwagandha won’t fix an underlying medical condition, but if stress and poor sleep are driving your fatigue, it has a reasonable evidence base. Most studies lasted 30 to 90 days, so it’s not something that works overnight. Common extracts used in research include KSM-66 and Sensoril, which are standardized to contain specific levels of active compounds called withanolides.
Rhodiola Rosea
Rhodiola is another adaptogen with some evidence for reducing stress-related fatigue. Study doses have ranged from 100 to 600 mg per day for up to 12 weeks. It’s often described as more stimulating than ashwagandha, so people who feel sluggish and mentally foggy during the day may find it more noticeable in the short term. The evidence is thinner than for ashwagandha, but it remains one of the better-studied options in this space.
Phosphatidylserine
Phosphatidylserine is a fat-based compound found in cell membranes that may help blunt excessive cortisol spikes during stress. Cleveland Clinic notes that typical adult doses range from 100 to 200 mg daily. If stress-related sleep difficulty is one of your primary symptoms, some providers suggest taking it at bedtime. The evidence here is modest but points in a positive direction for people whose main complaint is feeling wired and unable to wind down.
B Vitamins, Vitamin C, and Magnesium
The adrenal glands use more vitamin C than almost any other organ in the body, and B vitamins (particularly B5 and B6) play roles in producing stress hormones. Magnesium is involved in hundreds of enzymatic reactions, including those related to energy production and nervous system regulation. Deficiencies in any of these nutrients can independently cause fatigue, poor sleep, and brain fog. Rather than taking a specialized “adrenal support” formula, checking whether you’re meeting basic nutritional needs through diet or a quality multivitamin is a practical first step. Magnesium in particular is worth noting because an estimated 50% of Americans don’t get enough from food alone.
Adrenal Glandular Supplements Are Risky
Some products marketed for adrenal fatigue contain bovine (cow-derived) adrenal glandular extracts. These are frequently labeled as “hormone-free” and “completely safe,” but a 2018 study that tested 12 such supplements found that many contained hidden active hormones. Pregnenolone appeared in 42% of the samples, followed by budesonide (a steroid) in 25%, and smaller percentages contained androstenedione, cortisol, and cortisone.
Taking unknown amounts of steroid hormones without medical supervision can lead to serious problems. Your body may begin depending on the external hormones, suppressing its own production. If you stop taking the supplement abruptly, you could develop actual adrenal insufficiency. Long-term unsupervised steroid exposure can also trigger Cushing syndrome, which causes weight gain, high blood sugar, and weakened bones. Of all the supplements in this space, glandular extracts carry the clearest risk of harm.
Dietary and Lifestyle Changes That Help
If you’re experiencing the cluster of symptoms associated with “adrenal fatigue,” a few basic changes can make a noticeable difference regardless of the underlying cause.
Blood sugar stability is a big one. Eating protein and healthy fat at breakfast instead of fruit, cereal, or toast helps prevent the blood sugar spike and crash that can worsen fatigue mid-morning. Eating at regular intervals throughout the day, roughly every three to four hours, keeps energy more steady than relying on caffeine and large meals.
Salt intake is another area where the typical advice may not apply to you. If you have low blood pressure and crave salty foods, your body may genuinely need more sodium. Adding a bit of extra salt to your food or water can help maintain blood pressure and energy, particularly if you feel dizzy when standing up. This is a hallmark of both low cortisol states and simple dehydration.
Sleep consistency, even more than sleep duration, affects cortisol rhythms. Going to bed and waking up at the same time daily helps reset a cortisol curve that may have flattened from chronic stress. Reducing caffeine after noon and limiting screen exposure in the evening are small changes with outsized effects on sleep quality.
What’s Worth Trying and What to Skip
The most reasonable approach if you’re drawn to supplements for persistent fatigue is to start with the basics: make sure you’re not deficient in iron, vitamin D, B12, or magnesium, all of which are common and easily tested. Address sleep, blood sugar stability, and caffeine dependence before adding anything else.
If you still want to try an adaptogen, ashwagandha at 300 to 600 mg daily has the strongest evidence for reducing stress and cortisol. Rhodiola at 200 to 400 mg daily is a reasonable option if daytime fatigue and mental fog are your main issues. Phosphatidylserine at 100 to 200 mg is worth considering if you feel over-stimulated at night.
Avoid adrenal glandular products entirely. The risk of hidden hormones, combined with the potential for suppressing your body’s own cortisol production, makes them a poor trade-off for any possible benefit. And keep in mind that supplements marketed for adrenal fatigue are not FDA-approved for that purpose, which means insurance won’t cover them and quality varies widely between brands.
The most important thing you can do is get the basic workup: thyroid function, cortisol levels, a complete blood count, and metabolic panel. These tests are inexpensive, widely available, and can identify treatable causes that no supplement will fix.
