What to Take for Inflammation: Supplements and OTC Meds

The most effective approach to inflammation depends on whether you’re dealing with a short-term flare or a chronic, low-grade problem that has been building for months or years. Over-the-counter pain relievers like ibuprofen and naproxen are the fastest-acting option, but they’re meant for short-term use. For ongoing inflammation, a combination of targeted supplements and lifestyle changes tends to be safer and, over time, surprisingly effective.

How Inflammation Shows Up

Inflammation isn’t always obvious. Acute inflammation, the kind you get from a sprained ankle or infection, announces itself with redness, swelling, heat, and pain. Chronic, low-grade inflammation is quieter. It can show up as persistent joint stiffness, fatigue, brain fog, or slow recovery from exercise. Over years, it contributes to heart disease, type 2 diabetes, and autoimmune conditions.

If you want to know where you stand, a blood test called high-sensitivity CRP (C-reactive protein) gives a useful snapshot. Levels below 1.0 mg/L indicate low inflammation. Between 1.0 and 3.0 mg/L signals moderate, ongoing inflammation. Above 3.0 mg/L puts you in a higher-risk category for cardiovascular problems, and anything above 10.0 mg/L typically points to acute inflammation from infection or injury.

OTC Anti-Inflammatories: Fast but Limited

Ibuprofen and naproxen are the go-to over-the-counter options for inflammation. They work by blocking enzymes called COX-1 and COX-2, which your body uses to produce the chemicals that trigger swelling, pain, and fever. Naproxen lasts longer per dose, making it a better choice if you need steady relief throughout the day. Ibuprofen kicks in faster but wears off sooner.

These drugs are effective for short bursts of inflammation, like a sore back or a swollen knee after exercise. But they come with a firm time limit. If you need an OTC anti-inflammatory for longer than ten days, that’s the point where you should involve a doctor. Long-term use raises the risk of stomach ulcers, kidney damage, and cardiovascular events. The American Heart Association recommends taking the lowest effective dose for the shortest time necessary. People with high blood pressure, existing kidney disease, or heart failure face higher risks and need closer monitoring if they use these medications at all.

Omega-3 Fatty Acids

Fish oil is one of the most studied supplements for inflammation, and the evidence behind it is strong. The two active components, EPA and DHA, help your body resolve inflammation rather than just blocking pain signals. They work on a fundamentally different pathway than NSAIDs, making them better suited for long-term use.

There’s no official recommended dose of EPA and DHA specifically for inflammation, but the FDA has noted that daily intakes up to 2 grams from supplements are generally recognized as safe. Higher doses (around 900 mg of EPA plus 600 mg of DHA daily for several weeks or more) can suppress inflammatory responses enough to potentially reduce immune function, so more isn’t always better. A combined EPA/DHA intake in the range of 1 to 2 grams per day is where most people land. You can also get meaningful amounts from fatty fish like salmon, mackerel, and sardines eaten two to three times per week.

Curcumin

Curcumin, the active compound in turmeric, has genuine anti-inflammatory properties, but the raw spice you shake onto food delivers very little of it into your bloodstream. Most of what you swallow gets broken down in your gut before it can do much. This is why curcumin supplements often include piperine, a compound from black pepper that slows curcumin’s breakdown and increases the amount your body actually absorbs.

Dosing is where things get tricky. According to researchers at Oregon State University’s Linus Pauling Institute, it’s unclear whether doses below 3.6 grams per day are biologically active in humans, and optimal doses haven’t been formally established. Many supplement brands sell capsules containing 500 mg to 1,000 mg of curcumin, often with piperine added. If you’re trying curcumin, look for formulations specifically designed for absorption (these are sometimes labeled as “bioavailable” or include piperine/black pepper extract) and give it several weeks before judging whether it’s helping.

Ginger Extract

Ginger has a long reputation as an anti-inflammatory, and recent clinical work backs it up at surprisingly low doses. In a randomized trial of people with mild to moderate joint and muscle pain, just 125 mg per day of a ginger extract (standardized to 10% gingerols, delivering about 12.5 mg of active gingerols daily) improved pain, physical function, and inflammatory markers over 58 days. That’s a fraction of what most ginger supplements contain, which suggests you don’t need to megadose. A standardized ginger extract in the 100 to 500 mg range is a reasonable starting point.

Sleep and Inflammation

This is the factor people underestimate the most. Poor sleep doesn’t just make you tired; it actively drives inflammation. Sleep deprivation triggers a surge in circulating immune cells called neutrophils and ramps up inflammatory signaling throughout the body. Animal research has shown that prolonged sleep loss can induce something resembling a full-blown inflammatory storm, with dramatic increases in inflammatory chemicals. In those studies, the inflammation was driven in part by a molecule called prostaglandin D2, and when researchers blocked that pathway, sleep-deprivation-related inflammation dropped significantly.

You don’t need to lose sleep for days to see the effect. Even a few nights of sleeping five or six hours instead of seven or eight is enough to measurably raise inflammatory markers. If you’re taking supplements to fight inflammation while consistently sleeping poorly, you’re working against yourself. Prioritizing seven to nine hours of sleep is one of the most powerful anti-inflammatory interventions available, and it costs nothing.

When OTC Options Aren’t Enough

If your inflammation is caused by an autoimmune condition like rheumatoid arthritis, psoriatic arthritis, or lupus, over-the-counter medications and supplements are unlikely to control the disease on their own. The standard first step for most inflammatory autoimmune diseases is a prescription medication called methotrexate, which suppresses the overactive immune response driving the inflammation. The American College of Rheumatology recommends it as the first choice over more advanced treatments.

If methotrexate or similar medications don’t bring the disease under control, doctors may move to biologic drugs. These are targeted therapies that block specific parts of the immune system responsible for the inflammation. Not everyone needs them. Many people do well on conventional prescription anti-inflammatories alone. But the key point is that persistent, worsening inflammation, especially with joint swelling, prolonged morning stiffness, or unexplained fevers, signals something that supplements and lifestyle changes can support but not replace proper treatment for.

Putting It Together

For occasional inflammation from exercise, minor injuries, or temporary flare-ups, a short course of ibuprofen or naproxen handles it well. For chronic, low-grade inflammation, the combination of omega-3s (1 to 2 grams of EPA/DHA daily), a bioavailable curcumin supplement, and consistent sleep of seven-plus hours gives you a solid foundation without the risks of long-term NSAID use. Adding a standardized ginger extract is a reasonable low-cost addition, particularly if you have joint or muscle pain.

No single supplement is a silver bullet. Inflammation is driven by multiple pathways, and each of these options works on slightly different mechanisms. Using them together, alongside the basics of adequate sleep, regular movement, and a diet that isn’t heavily processed, gives you the broadest coverage with the least downside.
