A sore muscle after a long run, an aching knee joint, nighttime cramps, and nerve pain shooting down from your back all respond to different treatments. For most common leg pain, an over-the-counter anti-inflammatory like ibuprofen is a solid starting point, but matching your remedy to the type of pain makes a real difference in how fast you feel better.

Over-the-Counter Pain Relievers

For general muscle soreness, strains, or joint aches in the legs, anti-inflammatory medications like ibuprofen (Advil, Motrin) and naproxen (Aleve) are typically more effective than acetaminophen (Tylenol) because they reduce both pain and the inflammation driving it. Acetaminophen relieves pain but doesn’t touch inflammation, so it works best for mild, non-inflammatory leg pain or when you can’t tolerate anti-inflammatories.

If you need stronger relief without increasing your dose of either one, you can take acetaminophen alongside an anti-inflammatory. This combination can provide equivalent pain relief at lower doses of both, which reduces side effects. Keep acetaminophen under 3,000 milligrams per day to protect your liver. Anti-inflammatories carry their own risks: gastrointestinal bleeding, kidney stress, and cardiovascular side effects, particularly for older adults or people who take them regularly. They’re best used for short stretches of a few days rather than as an ongoing daily habit.

Topical Options That Bypass Your Stomach

If you’d rather not take pills, or you want to add something on top of them, topical treatments applied directly to the skin can help. Diclofenac gel is a topical anti-inflammatory available over the counter (Voltaren) and by prescription. It works well for arthritis pain in accessible joints like the knee, and patches are also effective for acute pain from minor sprains, strains, and bruises. Because the medication absorbs through the skin near the pain source, it causes fewer stomach-related side effects than oral anti-inflammatories.

Menthol-based creams (Biofreeze, Icy Hot) create a cooling or warming sensation that distracts nerve signals and can ease muscle tightness. Lidocaine patches and creams numb a specific area and work best for localized pain you can pinpoint with your finger. These are particularly useful for sore calves, shin splints, or post-exercise soreness.

Nerve Pain Requires Different Medication

If your leg pain feels like burning, tingling, electric shocks, or shooting pain that travels from your hip or back down through your leg, you’re likely dealing with nerve pain. Standard anti-inflammatories and acetaminophen do very little for this type of pain because the problem isn’t inflammation in the leg itself. It’s a nerve being compressed or damaged.

Nerve pain in the legs, including sciatica, is typically treated with prescription medications that calm overactive nerve signals. The two most common options work by binding to calcium channels in nerve cells, which reduces the release of chemical messengers that amplify pain signals. Your doctor may also consider certain antidepressants that dampen pain signaling through a different pathway. These medications take days to weeks to build up effectiveness, so they aren’t quick fixes, but they address the root cause of nerve-based leg pain in a way that ibuprofen simply cannot.

Magnesium for Nighttime Leg Cramps

Leg cramps that wake you up at night are one of the most common reasons people search for leg pain relief. Magnesium supplementation is a popular remedy, though the evidence is modest. A randomized trial of 184 patients found that 226 milligrams of magnesium oxide taken once daily improved nocturnal leg cramps, but only after about 60 days of consistent use. So if you try magnesium, give it at least two months before deciding it isn’t working.

Most studies have tested magnesium oxide, though magnesium citrate and glycinate are often recommended because they’re easier on the stomach and may absorb better. In the short term, stretching your calves before bed and staying well hydrated are the most reliable ways to reduce nighttime cramps. Potassium-rich foods like bananas, potatoes, and leafy greens also support proper muscle function, though a true potassium deficiency severe enough to cause cramps is uncommon in people eating a normal diet.

When Poor Circulation Is the Problem

Leg pain that shows up predictably when you walk and fades when you rest is a hallmark of peripheral artery disease, where narrowed arteries restrict blood flow to the legs. This type of pain, called claudication, feels like cramping or heaviness in the calves, thighs, or hips during activity. Over-the-counter pain relievers won’t address the underlying circulation problem.

Higher compression levels are used for more significant swelling or to prevent blood clots, but those should be fitted based on your specific condition.

Non-Drug Approaches Worth Trying

Pain That Needs Immediate Attention

A blood clot in a deep leg vein (DVT) causes swelling in one leg, pain or cramping that often starts in the calf, skin that turns red or purple, and a noticeable warmth in that leg.
