The two most effective options for breaking a fever are ibuprofen (Advil, Motrin) and acetaminophen (Tylenol), both available over the counter. Ibuprofen tends to work slightly better: in a large meta-analysis of over 240,000 patients, children given ibuprofen had lower temperatures within four hours and were nearly twice as likely to be fever-free compared to those given acetaminophen. Both are safe for most adults and children when taken at the right dose.
Ibuprofen vs. Acetaminophen
Ibuprofen and acetaminophen both lower your temperature, but they work through different pathways and are processed by different organs. Ibuprofen reduces inflammation and is filtered through the kidneys. Acetaminophen acts on the brain’s temperature-regulation center and is processed by the liver. For pure fever reduction, ibuprofen has a modest edge, lowering temperature about 0.4°C (0.7°F) more than acetaminophen in head-to-head trials. That difference is small but can matter when you’re miserable at 103°F.
Acetaminophen is still a solid choice, especially if you have stomach sensitivity or kidney concerns. It’s also the only option for babies under six months, since ibuprofen isn’t approved for that age group. For most healthy adults, either one will bring a fever down within 30 to 60 minutes.
Alternating the Two Medications
If one medication alone isn’t keeping your fever down, you can alternate between ibuprofen and acetaminophen. The key is timing: take one first, then wait four to six hours before taking the other. You can continue rotating every three to four hours throughout the day. This approach targets different pathways simultaneously and can provide more consistent relief than either drug alone.
Keep a written log of what you took and when. It’s easy to lose track, especially when you’re sick and groggy, and double-dosing is the real danger here. If you’re alternating for more than three days straight, that’s a signal to talk with a healthcare provider about what’s causing the persistent fever rather than just managing it.
Dosing Limits That Matter
Acetaminophen has a firm ceiling: no more than 4,000 milligrams in 24 hours for adults. Extra Strength Tylenol caps that limit at 3,000 milligrams per day, which is a safer target to aim for. Going over this threshold risks serious liver damage, and the danger increases significantly if you drink alcohol. Check the labels on cold medicines, sleep aids, and combination products, because many of them contain hidden acetaminophen that counts toward your daily total.
For ibuprofen, the over-the-counter maximum is 1,200 milligrams per day for adults. Take it with food or a full glass of water to protect your stomach lining. Avoid ibuprofen if you’re pregnant (especially past 20 weeks, when it can affect fetal kidney function and amniotic fluid levels) or if you have a history of stomach ulcers or heart disease.
What About Aspirin?
Aspirin reduces fever effectively in adults, but it should never be given to children or teenagers. Aspirin use during a viral illness like the flu or chickenpox is linked to Reye’s syndrome, a rare but potentially fatal condition that causes rapid swelling in the liver and brain. Without treatment, it can be deadly within days. Stick with acetaminophen or ibuprofen for anyone under 18.
Non-Drug Approaches That Help
Medication works faster, but a few simple steps support the process. Stay hydrated. Your body loses extra fluid when your temperature rises, roughly 10% more for every degree Celsius above normal. Water, broth, and electrolyte drinks all count. If you’re urinating less frequently or your urine is dark, you’re behind on fluids.
Dress in light layers and keep your room comfortably cool. Bundling up in heavy blankets traps heat and can push your temperature higher. A lukewarm washcloth on the forehead feels good but won’t significantly lower your core temperature on its own. Skip ice baths or cold showers, which can trigger shivering and actually raise your internal temperature as your body fights to warm back up.
Rest matters more than people give it credit for. A fever is your immune system’s deliberate response to infection, and your body is burning through energy to maintain that elevated temperature. Sleep and low activity let those resources go toward fighting whatever caused the fever in the first place.
Fever Thresholds by Age
Not every fever needs medication. A temperature of 100.4°F (38°C) taken orally, rectally, or with an ear thermometer meets the clinical definition of a fever. In otherwise healthy adults, a low-grade fever in the 100 to 102°F range often resolves on its own and may actually help your immune system work more efficiently. Adults with fevers of 103°F (39.4°C) or higher typically look and feel noticeably sick, and that’s a reasonable threshold for seeking medical attention if it doesn’t respond to medication.
The rules are stricter for babies. Any infant under three months old with a fever needs immediate medical evaluation, regardless of how mild the temperature seems. For children under two, acetaminophen should only be given under a doctor’s guidance. Children’s liquid acetaminophen comes in a standard concentration of 160 mg per 5 mL, and dosing is based on your child’s weight rather than age. Your pediatrician’s office can give you a precise dose, and many keep dosing charts on their websites.
For children under 12, acetaminophen can be given every four hours, up to five doses in 24 hours. Children over 12 can take extra-strength formulations every six hours, with a maximum of six tablets per day. Extended-release 650 mg products are off-limits for anyone under 18.
