For most fevers in adults, acetaminophen (Tylenol) or ibuprofen (Advil, Motrin) will bring your temperature down effectively. A fever starts at 100.4°F (38°C) measured orally, rectally, or by ear. But not every fever needs medication. Fever is your immune system’s response to infection, and if you’re mildly uncomfortable, rest and fluids may be all you need.
Acetaminophen vs. Ibuprofen
Both acetaminophen and ibuprofen reduce fever by blocking enzymes your body uses to produce prostaglandins, chemicals that raise your temperature and trigger pain. The key difference is where they work. Acetaminophen acts only in the brain, while ibuprofen works throughout the body. That means ibuprofen also reduces inflammation (swelling, redness, joint pain), making it a better choice if your fever comes with body aches or a sore throat.
Acetaminophen is gentler on the stomach and is generally the go-to if you have acid reflux, ulcers, or kidney concerns. Ibuprofen, on the other hand, should be taken with food to avoid stomach irritation. Both typically start lowering a fever within 30 to 60 minutes.
One important safety limit: do not exceed 4,000 milligrams of acetaminophen in 24 hours. Many cold and flu products already contain acetaminophen, so check labels carefully to avoid doubling up. If you drink alcohol regularly or have liver disease, the safe ceiling is lower. For ibuprofen, spacing doses 6 to 8 hours apart and staying within the label’s daily maximum keeps your risk of stomach and kidney problems low.
Can You Alternate the Two?
If one medication alone isn’t bringing your fever down enough to keep you comfortable, you can alternate acetaminophen and ibuprofen. They’re safe to take at the same time or close together because they work through different pathways. When alternating, keep acetaminophen doses at least 4 hours apart and ibuprofen doses at least 6 hours apart, and don’t exceed the 24-hour maximum for either one.
The biggest risk with alternating is losing track of what you took and when. Write it down, use a notes app on your phone, or set alarms. Confusion over timing is how accidental overdoses happen, especially when you’re sick and groggy.
What to Avoid
Do not give aspirin to children or teenagers with a fever. Aspirin during a viral illness like the flu or chickenpox is linked to Reye’s syndrome, a rare but serious condition that causes swelling in the liver and brain. This applies to plain aspirin and any combination product that contains it.
For adults, aspirin does reduce fever, but acetaminophen and ibuprofen are preferred because they carry fewer risks for most people. If you’re already taking aspirin daily for heart health, talk to your pharmacist before adding ibuprofen, since the two can interact.
Fever Medication for Children
Children’s doses are based on weight, not age. Liquid acetaminophen for kids comes in a standard concentration of 160 mg per 5 mL, and you’ll find a dosing chart on the bottle matched to your child’s weight. Give it every 4 to 6 hours as needed, but no more than 5 doses in 24 hours for children under 12.
Acetaminophen should not be given to children under 2 without a doctor’s guidance. Ibuprofen is generally not recommended for infants under 6 months. For older children, either medication works, but stick with one first. Only add the second if a single medication isn’t keeping your child comfortable.
When giving both, keep a written log of each dose, the time, and the amount. It’s easy to mix up which medicine you gave last, especially overnight.
Fluids and Rest Matter More Than You Think
A fever increases water loss through your skin. For every degree above 100.4°F, your body loses roughly 10% more water through evaporation than it normally would. That’s why fevers make you feel dried out, headachy, and exhausted even before you factor in sweating.
Drink water, broth, diluted juice, or an electrolyte drink steadily throughout the day. Don’t wait until you feel thirsty. If your urine is dark yellow, you’re behind on fluids. For children, small frequent sips work better than trying to get them to drink a large amount at once.
Dress in light, breathable clothing and keep blankets to a single layer. Bundling up traps heat and can push your temperature higher.
Skip the Sponge Baths and Supplements
Tepid sponge baths are a common home remedy, but research doesn’t support them. In a study of febrile children, those who received a sponge bath on top of acetaminophen cooled slightly faster in the first hour, but showed no meaningful temperature difference after two hours compared to children who took acetaminophen alone. The sponge-bathed children did, however, score significantly higher on discomfort. Physical cooling methods are generally discouraged unless you’re dealing with heatstroke rather than infection.
Vitamin C and zinc supplements are another popular instinct, but a clinical trial found that even high doses (8,000 mg of vitamin C per day and 50 mg of zinc per day) had no significant effect on fever duration, symptom severity, or recovery time. These doses are well above what most people take, meaning typical over-the-counter supplement doses are even less likely to help. Staying hydrated and resting will do more for you than any supplement regimen.
When a Fever Needs Medical Attention
Most fevers resolve on their own within a few days. Adults with temperatures of 103°F (39.4°C) or higher will typically look and feel noticeably sick, and that’s a reasonable threshold for calling a doctor. A fever that persists beyond three days also warrants a check-in, even if it stays below 103°F.
Seek immediate medical help if a fever comes with any of these symptoms:
- Seizure or loss of consciousness
- Confusion or difficulty staying alert
- Stiff neck
- Trouble breathing
- Severe pain anywhere in the body
- Swelling or inflammation of any body part
- Painful urination or foul-smelling urine
For infants under 3 months, any fever of 100.4°F or higher is a reason to call your pediatrician right away, regardless of how the baby seems otherwise. Young infants can deteriorate quickly, and fever at that age always needs evaluation.
