A daily 5 mg dose of tadalafil keeps a low, steady level of the drug in your system around the clock, improving erections and, in many cases, easing urinary symptoms from an enlarged prostate. It’s the lowest daily dose available and works differently from the higher on-demand doses (10 mg or 20 mg) that you take before sex. Instead of timing a pill, you take it once a day and stay ready whenever the moment comes.
How It Works in Your Body
When you’re sexually aroused, your body releases nitric oxide, a chemical signal that triggers a chain reaction to relax smooth muscle in the blood vessels of the penis. That relaxation depends on a molecule called cGMP. The problem is that an enzyme called PDE5 breaks down cGMP almost as fast as your body makes it. Tadalafil blocks that enzyme, so cGMP accumulates and the blood vessels stay relaxed longer, allowing more blood flow into the penis and producing a firmer erection.
This isn’t an automatic process. You still need arousal to start the nitric oxide signal. The drug simply removes a bottleneck so your body’s natural response works more effectively.
The same relaxation effect happens in smooth muscle throughout the pelvis, including the prostate, bladder neck, and urethra. That’s why a single 5 mg daily dose is also prescribed for men with lower urinary tract symptoms from benign prostatic hyperplasia (BPH), and it’s the only dose approved for that purpose.
How Well It Works for Erections
An integrated analysis of six randomized trials covering nearly 2,000 men found that 5 mg daily tadalafil produced a clinically meaningful improvement in erectile function scores. How much benefit you get depends on severity. Among men with mild erectile dysfunction at baseline, about 75% reached a meaningful improvement and 66% returned to normal function. For moderate ED, those numbers were 63% and 48%. For severe ED, 45% saw meaningful improvement and 29% reached normal scores.
The drug also performed well in men with common health conditions that often make ED harder to treat. Men with high blood pressure, heart conditions, high cholesterol, obesity, and those who smoke all reached clinically meaningful improvements on the 5 mg dose. Men whose ED was primarily psychological rather than physical saw some of the largest gains.
How It Compares to Higher On-Demand Doses
The 5 mg daily dose produces results comparable to the 20 mg on-demand dose, just through a different approach. In a head-to-head trial, 95% of men on daily 5 mg reported the treatment was effective at the end of 12 weeks, compared with 86% on the 20 mg on-demand dose. Successful intercourse rates were also similar: 73% for the daily group versus 70% for the on-demand group.
Compliance was higher with daily dosing (100% vs. 86%), and there was a trend toward fewer side effects. In broader preference studies, about 72% of men preferred daily therapy over on-demand dosing. The main advantage is spontaneity: you don’t need to plan around a pill, and there’s no pressure to time sexual activity within a specific window.
What It Does for Urinary Symptoms
For men with BPH, taking 5 mg daily reduced prostate symptom scores by 5.6 points over 12 weeks, compared with 3.6 points for placebo. That two-point difference may sound modest, but it translates to noticeably easier urination, less urgency, fewer nighttime bathroom trips, and a stronger stream. Improvements start appearing within the first week and reach statistical significance by week four.
The mechanism is the same smooth muscle relaxation that helps with erections, just in different tissue. By relaxing the muscles around the prostate, bladder neck, and urethra, tadalafil reduces the physical squeeze that makes urination difficult. It also increases blood flow to pelvic tissues, which may calm overactive nerve signaling in the bladder.
How Long It Takes to Work
Tadalafil has a half-life of about 17.5 hours, meaning it takes roughly that long for half the drug to clear your system. When you take it daily, each new dose adds to what’s still circulating from the day before. After about five days of consistent dosing, you reach a steady state where the amount entering and leaving your body is balanced. At that point, you have a stable baseline level of the drug in your bloodstream at all times.
You may notice some effect from the very first dose, since even a single 5 mg tablet produces measurable blood levels. But the full benefit builds over several days to weeks. For urinary symptoms, clinical improvement was detectable at one week but continued to strengthen through the 12-week study period.
Common Side Effects
Because tadalafil relaxes blood vessels throughout the body, not just in the pelvis, the most common side effects reflect that widespread vasodilation. Headache is the most frequently reported issue, followed by indigestion, back pain, muscle aches, flushing, and nasal congestion. These tend to be mild and often diminish after the first few weeks of daily use as your body adjusts.
The 5 mg dose generally causes fewer and milder side effects than the 10 mg or 20 mg doses. Back pain and muscle aches, when they occur, typically show up 12 to 24 hours after a dose and resolve within a couple of days.
Interactions That Matter
The most dangerous interaction is with nitrate medications, which include nitroglycerin tablets, patches, sprays, and isosorbide pills prescribed for chest pain. Both tadalafil and nitrates increase the same signaling molecule (cGMP), and combining them can cause a severe, potentially life-threatening drop in blood pressure. In clinical testing, nearly half of men who took tadalafil with nitroglycerin experienced a standing systolic blood pressure below 85 mmHg, a level that can cause fainting and cardiovascular collapse. Nitrates should not be used within 48 hours of taking tadalafil.
Alcohol adds to the blood pressure-lowering effect. Having five or more drinks while on tadalafil has been linked to orthostatic hypotension, the kind of sudden blood pressure drop that hits when you stand up and can cause dizziness or fainting. Keeping alcohol to one or two drinks avoids this risk for most people.
Alpha-blockers, often prescribed for BPH or high blood pressure, can also amplify the blood pressure drop. If you’re already on an alpha-blocker, the 5 mg dose is typically started only after you’ve been stable on the alpha-blocker for a period of time.
Who May Need a Different Dose
Kidney function affects how quickly your body clears tadalafil. If your kidney filtration rate falls between 30 and 50 mL/min, the typical starting point is 2.5 mg daily rather than 5 mg, with an increase only if that lower dose is tolerated well. For more severe kidney impairment (below 30 mL/min) or for those on dialysis, daily tadalafil is not recommended at all.
The 5 mg dose is the ceiling for daily use. If it doesn’t provide enough benefit, the conversation with your prescriber usually shifts toward on-demand dosing at 10 mg or 20 mg rather than increasing the daily amount. Some men start at 2.5 mg daily and move to 5 mg based on response, which is the standard dose-finding approach.
