Your blood type is more than a label for transfusions. The A, B, AB, and O groups sitting on the surface of your red blood cells influence your risk for heart disease, certain cancers, blood clots, and even how your gut bacteria thrive. The connections are real but modest, and they sit alongside one very popular claim that doesn’t hold up: the idea that blood type determines your personality or ideal diet.
Heart Disease and Blood Clots
People with non-O blood types (A, B, or AB) have a roughly 12% higher risk of cardiovascular events compared to those with type O. That number comes from a meta-analysis combining data from six large studies, including the long-running Framingham Heart Study. A 12% bump is meaningful at a population level, but it’s a fraction of the risk added by smoking, high blood pressure, or inactivity. Think of it as one small factor among many, not a destiny.
The mechanism behind this involves clotting proteins. Your blood type directly affects levels of von Willebrand factor, a protein that helps blood clot. Type O individuals have the lowest levels, while type B individuals have the highest, with an overall difference of about 32%. Lower clotting factor levels mean type O blood is slightly less “sticky,” which may explain the reduced cardiovascular risk. It also means type O individuals bleed a bit more after surgery or injury, a tradeoff that occasionally matters in emergency medicine.
Cancer Risk Varies by Type
The strongest cancer connection involves the pancreas. Compared to type O, people with type B blood have a 53% higher relative risk of pancreatic cancer. Type AB carries a 47% increase, and type A a 38% increase. These numbers sound alarming, but context matters: pancreatic cancer is rare overall, affecting roughly 1.7% of people in a lifetime. A 53% increase on a small base number still leaves the absolute risk low.
Gastric cancer has also been linked to type A blood, a connection first observed decades ago. The biology likely involves how blood group antigens on the stomach lining interact with the bacterium H. pylori, which drives most stomach cancers. Type O appears to offer some protection here as well, though the effect size is smaller than for pancreatic cancer.
Type O and Malaria Protection
If you’ve ever wondered why type O is the most common blood group worldwide, malaria is part of the answer. The parasite that causes severe malaria hijacks red blood cells and makes them clump together with healthy cells into sticky clusters called rosettes. These rosettes block tiny blood vessels, causing the life-threatening complications of the disease.
The A and B antigens on non-O red blood cells act as docking points for this clumping. Infected cells “prefer” binding to A or B cells and form larger, more stable rosettes. Type O cells lack these surface antigens, so rosettes are smaller and less dangerous. People who carry two copies of the O gene (rather than one O and one A or B) get the strongest protection. Over thousands of generations in malaria-endemic regions, this survival advantage pushed type O to high frequency in populations across sub-Saharan Africa, South America, and Southeast Asia.
Diabetes and Metabolic Health
Type B blood has been associated with a higher risk of developing type 2 diabetes. One cross-sectional study found that individuals with type B had nearly twice the odds of type 2 diabetes compared to non-B individuals, after adjusting for other factors. Type A showed up more frequently among diabetes patients as well, though that association did not reach statistical significance.
These findings are preliminary and come from smaller studies, so they carry less weight than the cardiovascular data. Lifestyle factors like diet, exercise, and body weight still dwarf any blood-type influence on diabetes risk.
Cognitive Health and Type AB
Type AB, the rarest blood group (present in about 4% of the population), has drawn attention in dementia research. A study using UK Biobank data found that men with the AB genotype had a 34% higher risk of Alzheimer’s disease compared to other blood types. The picture gets more complicated when genetics are layered in: AB men who also carried one copy of the APOE e4 gene (the strongest known genetic risk factor for Alzheimer’s) had a 75% increased risk. Curiously, AB men with two copies of APOE e4 actually showed a lower risk of dementia, suggesting the interaction between blood type and other genes is not straightforward.
These findings applied primarily to men. The researchers did not find the same pattern in women, which highlights how blood type effects can differ by sex.
Your Gut Bacteria Care About Your Blood Type
Blood type antigens aren’t just on red blood cells. About 80% of people (called “secretors”) also express them in saliva, mucus, and the lining of the gut. This matters because gut bacteria feed on these sugar molecules. Secretors with type A blood have the highest diversity of Bifidobacteria, a group of beneficial microbes, because bacteria in this genus particularly like to consume the A antigen as a nutrient source.
Secretors in general support more Bifidobacteria than non-secretors, partly because they produce a sugar called 2′-fucosyllactose in the gut’s mucus layer. This is the same compound found in breast milk that helps establish a healthy microbiome in newborns. Non-secretors, who make up roughly 20% of the population, lack this gut-derived supply and tend to carry lower levels of these beneficial bacteria. Secretor status is genetically determined and linked to (but separate from) your ABO type.
The Personality Theory Doesn’t Hold Up
In Japan and South Korea, the belief that blood type predicts personality is deeply embedded in popular culture. Type A is supposedly detail-oriented, type B creative and selfish, type O confident, and type AB eccentric. It’s treated much like astrology is in Western countries.
Large-scale studies have tried to validate this. One Japanese survey of 3,750 people did find that respondents scored slightly higher on personality traits associated with their own blood type. But when the researchers looked at participants who had never heard of blood type personality theory, the differences shrank dramatically, suggesting a self-fulfilling prophecy rather than biology. More importantly, a 2017 meta-analysis of over 260,000 people examined the genetic influence of blood-type-related genes on the Big Five personality traits and found the maximum effect was 0.04%, a number so small it’s considered statistical noise. No study using standard personality tests has produced consistently replicable results linking ABO type to temperament.
The Blood Type Diet Has No Clinical Evidence
The “Eat Right 4 Your Type” diet, popularized in the late 1990s, claims that type O individuals should eat high-protein diets, type A should be vegetarian, type B should eat dairy, and type AB should combine elements of A and B. It’s sold millions of books.
A systematic review screened 1,415 references and found zero studies that actually tested whether following a blood-type-specific diet improved health outcomes compared to a standard diet. Not one. Some of the diets themselves are reasonably healthy (eating more vegetables is good advice regardless of blood type), but the idea that your ABO group should dictate what you eat has no supporting evidence. If a blood-type diet helped you feel better, it was likely because of what you added or removed from your plate, not because of the type on your blood donor card.
What Actually Matters
Blood type is a fixed genetic trait you can’t change, and the health risks associated with it are generally small compared to modifiable factors. A type A person who exercises, doesn’t smoke, and manages their blood pressure will almost certainly have better cardiovascular outcomes than a type O person who does none of those things. Where blood type genuinely matters most is still its original use case: transfusions, organ transplants, and pregnancy compatibility. The disease associations are real, statistically validated, and worth knowing, but they belong in the “interesting context” category rather than the “reason to worry” category.
