The active ingredient in Monistat cream is miconazole nitrate, an antifungal that kills the yeast responsible for vaginal yeast infections. The concentration varies by product: Monistat 7 contains 2% miconazole nitrate, Monistat 3 contains 4%, and Monistat 1 delivers a higher single dose. All three versions use the same antifungal compound, just at different strengths for different treatment lengths.
The Active Ingredient: Miconazole Nitrate
Miconazole nitrate belongs to a class of antifungals called azoles. It works by blocking a specific step in how yeast cells build their outer membranes. Yeast cells need a fatty substance called ergosterol to keep their membranes intact, much like how your skin needs certain oils to stay flexible and protective. Miconazole interrupts the production of ergosterol, which makes the yeast cell membrane full of holes. The cell essentially leaks its contents and dies.
This is why the cream treats the infection rather than just relieving symptoms. It’s directly lethal to the Candida yeast that causes most vaginal yeast infections.
How Strength Differs Across Products
The number in each Monistat product refers to how many days you use it, not the strength of the cream. Counterintuitively, the shorter treatments are more concentrated:
- Monistat 7 contains 2% miconazole nitrate, delivering 100 mg per dose nightly for seven days.
- Monistat 3 contains 4% miconazole nitrate, delivering 200 mg per dose nightly for three days.
- Monistat 1 delivers the full course in a single high-dose application.
The total amount of antifungal you receive across the full treatment is roughly comparable between products. The trade-off is that higher-concentration, shorter treatments can cause more intense local irritation in some people.
Inactive Ingredients in the Cream
Beyond miconazole nitrate, Monistat cream contains several inactive ingredients that give the cream its texture, keep it stable, and help it spread evenly. The Monistat 7 vaginal cream formulation lists these inactive ingredients: benzoic acid, cetyl alcohol, isopropyl myristate, polysorbate 60, potassium hydroxide, propylene glycol, purified water, and stearyl alcohol.
Each serves a specific purpose. Cetyl alcohol and stearyl alcohol are fatty alcohols (not the drying kind) that thicken the cream and make it smooth. They’re common in moisturizers and lotions. Isopropyl myristate helps the cream absorb into tissue rather than sitting on the surface. Polysorbate 60 is an emulsifier that keeps the water-based and oil-based components from separating. Propylene glycol acts as a humectant, helping the cream retain moisture. Potassium hydroxide adjusts the pH of the formulation, and purified water serves as the base.
Benzoic acid functions as a preservative, preventing bacterial growth in the product. It’s worth noting because it’s one of the ingredients most commonly linked to skin sensitivity. Some people experience mild burning or irritation from benzoic acid, which can be hard to distinguish from the yeast infection itself.
The External Vulvar Cream
Many Monistat products come as “combination packs” that include both an internal vaginal cream (or suppository) and a separate tube of external cream for itch relief around the vulva. The external cream also contains 2% miconazole nitrate and uses the same inactive ingredient base: purified water, propylene glycol, stearyl alcohol, cetyl alcohol, polysorbate 60, isopropyl myristate, benzoic acid, and potassium hydroxide.
This external cream is designed to treat yeast on the outer skin while the internal cream or suppository works inside. It’s not a numbing cream or anti-itch medication in the traditional sense. It relieves itching by killing the yeast causing it.
Possible Irritation From Ingredients
Burning or irritation after applying Monistat is relatively common and doesn’t always mean you’re allergic to the product. Inflamed tissue from a yeast infection is already sensitive, and introducing any cream can temporarily intensify that feeling. That said, isolated reports of allergic contact dermatitis have been linked to miconazole cream formulations. The ingredients most likely to trigger sensitivity are benzoic acid and the fatty alcohols (cetyl and stearyl alcohol), which are known contact allergens for a small number of people.
If you’ve had reactions to lotions or cosmetics containing cetyl alcohol or stearyl alcohol before, you may be more likely to experience irritation with Monistat. Mild burning that fades within the first day or two is typical. Worsening redness, swelling, or a rash that spreads suggests an ingredient sensitivity rather than a normal response.
One Important Drug Interaction
Miconazole, even when applied topically, can interact with the blood thinner warfarin. The antifungal interferes with how the body breaks down warfarin, causing the drug to build up and thin the blood more than intended. A safety review by UK regulators found that among 146 reported cases of this interaction, roughly half involved dangerously elevated bleeding risk, and three cases resulted in fatal bleeding events. If you take warfarin, this is a conversation to have with your pharmacist before using Monistat.
Storage
Monistat cream should be stored at room temperature, between 68°F and 77°F. Avoid leaving it in a hot car or bathroom cabinet that gets steamy, as heat can break down the formulation and reduce its effectiveness.
