Feeling cold and tired at the same time usually points to your body not producing enough heat or not delivering enough oxygen to your tissues. These two symptoms share a surprisingly short list of root causes, and most of them are detectable with basic blood work. The most common culprits are an underactive thyroid, iron deficiency, low vitamin B12, not eating enough calories, and poor sleep.
Your Thyroid Sets the Thermostat
The thyroid gland controls your basal metabolic rate, which is essentially how much energy your cells burn at rest. When thyroid hormone levels drop, your cells consume less oxygen, produce less heat, and generate less of the energy molecule ATP. The result is a predictable pair of symptoms: cold intolerance and persistent fatigue. Hair loss, weakness, weight gain, and brain fog often show up alongside them.
Hypothyroidism is one of the first things doctors check when someone reports feeling cold and tired. A simple blood test measures TSH (thyroid-stimulating hormone). Normal TSH falls below about 4.5 mIU/L. Levels between 5 and 10 with normal thyroid hormone levels indicate subclinical hypothyroidism, a milder form that can still cause noticeable symptoms. Levels above 10 suggest a more significant problem. If your TSH comes back elevated, treatment typically brings both symptoms under control within weeks.
Iron Deficiency Starves Your Cells of Oxygen
Iron plays a double role in keeping you warm and alert. First, it’s a core component of hemoglobin, the protein in red blood cells that carries oxygen from your lungs to every tissue in your body. When iron drops, less oxygen reaches your muscles and organs, which means less fuel for energy production. Second, iron is built into the enzymes inside mitochondria that actually convert that oxygen into usable energy. Low iron hits both steps at once.
The connection to feeling cold is direct. Iron deficiency also disrupts thyroid hormone production, impairing your body’s ability to maintain normal temperature when exposed to even mild cold. Your hands and feet may feel icy, and you might reach for a sweater when nobody else in the room is uncomfortable.
Ferritin, the storage form of iron, is the best screening test. A level below 15 micrograms per liter confirms iron deficiency. Levels between 15 and 30 suggest probable deficiency. What’s often overlooked: symptoms like fatigue and cold intolerance can appear even when ferritin is technically in the “normal” range but below 30. Women with heavy periods, vegetarians, and frequent blood donors are especially prone to iron stores quietly dropping into this zone.
Vitamin B12 and Other Nutritional Gaps
Your body needs vitamin B12 to manufacture red blood cells. Without enough of it, you develop a type of anemia where red blood cells are abnormally large and inefficient at carrying oxygen. The symptoms overlap heavily with iron deficiency: fatigue, lightheadedness, and low energy, particularly with exertion. Long-term B12 deficiency can also damage nerves, causing numbness and tingling in the hands and feet that some people mistake for poor circulation.
Vitamin D deficiency is another common finding in people who feel persistently drained. Low levels are linked to fatigue, mood changes, and muscle aches. Magnesium deficiency can contribute too, affecting muscle function and energy regulation. All of these can be checked with a single round of blood work.
Not Eating Enough Slows Your Metabolism
If you’ve been dieting, skipping meals, or simply not eating much, your body responds by turning down the furnace. Research published in PLOS ONE found that caloric restriction reduced total daily energy expenditure by roughly 430 calories per day after just three months, even after accounting for the smaller body size. This adaptation includes a measurable drop in the energy your cells spend simply staying alive, plus a decrease in spontaneous physical activity. You move less without realizing it, and your body generates less heat.
Interestingly, the same study found that when people combined calorie cutting with exercise, this metabolic slowdown didn’t occur. So if you’re eating less and also sedentary, the combination is especially likely to leave you cold and dragging. The fix isn’t necessarily eating more overall, but eating enough to support your activity level.
Poor Sleep Disrupts Temperature Regulation
Sleep and body temperature are tightly linked through the same brain circuits. Your core temperature naturally dips during deep sleep, and that dip is part of what initiates and maintains restful sleep cycles. When you cut sleep short or stay up past your normal bedtime, this temperature rhythm gets disrupted.
Sleep deprivation appears to increase heat loss from the body, possibly by activating the same blood vessel dilation that normally happens during deep sleep but at the wrong times. Animal research shows that chronic sleep loss eventually leads to a progressive drop in core body temperature despite the body burning more calories to compensate. In practical terms, running on too little sleep leaves you both exhausted and chilled, and no amount of coffee fixes the temperature piece.
Blood Sugar Swings Can Make You Cold
When blood sugar drops too low, your body triggers a sweating response and redirects blood flow to your limbs, both of which pull heat away from your core. Research in the American Journal of Physiology confirmed that during sustained low blood sugar episodes, core temperature falls because heat is lost through the skin faster than the body can generate it. You don’t need to be diabetic for this to matter. Skipping meals, eating mostly refined carbohydrates, or going long stretches without food can produce mild dips in blood sugar that leave you shaky, tired, and cold.
Circulation Problems and Cold Extremities
If the cold sensation is mainly in your hands and feet rather than all over, poor circulation may be involved. Peripheral artery disease narrows the blood vessels in your limbs, reducing blood flow. Symptoms include skin that feels cool to the touch, color changes in the legs or feet, and aching or burning pain, especially at rest. This is more common in people over 50, smokers, and those with diabetes or high blood pressure.
Raynaud’s phenomenon is another possibility, particularly in younger women. It causes the small blood vessels in your fingers and toes to overreact to cold or stress, temporarily cutting off blood flow and turning them white or blue. It’s uncomfortable but usually not dangerous on its own.
Signs That Warrant Blood Work
A few days of feeling run down during a busy week is normal. The pattern worth investigating looks different: you wake up tired after a full night of sleep, you feel winded doing basic things like climbing stairs, your energy has been low for more than two weeks, or people around you are commenting on how drained you look. Dizziness, frequent headaches, or unexplained aches add to the case for getting tested.
The standard workup is straightforward. A complete blood count checks for anemia. A thyroid panel measures TSH and thyroid hormones. Ferritin, vitamin B12, vitamin D, and magnesium levels round out the picture. If anxiety, depression, or sleep disruption might be factors, those are worth discussing at the same visit. Many of these causes overlap, and it’s common to find more than one contributing issue at a time.
