Why Am I Always Tired and Have No Energy?

Persistent tiredness that doesn’t improve with rest usually has an identifiable cause, and often more than one. The most common culprits fall into a few categories: nutritional deficiencies that starve your cells of oxygen and energy, hormonal imbalances that slow your metabolism, sleep disorders that fragment your rest without you realizing it, and mood conditions that drain motivation at a neurochemical level. The good news is that most of these are treatable once identified.

Iron Deficiency: The Most Overlooked Cause

Iron deficiency is one of the most common nutritional deficiencies worldwide, and fatigue is its hallmark symptom. Your body needs iron to build hemoglobin, the protein in red blood cells that carries oxygen to every tissue. When iron drops too low, your muscles, brain, and organs simply don’t get enough oxygen to produce energy efficiently. The result is a bone-deep tiredness that no amount of sleep fixes, often accompanied by shortness of breath during ordinary activities, pale skin, and brittle nails.

Women with heavy periods, vegetarians, frequent blood donors, and people with digestive conditions that impair absorption are at the highest risk. A simple blood test measuring your ferritin (stored iron) and hemoglobin levels can confirm or rule this out quickly. If your levels are low, iron-rich foods like red meat, lentils, and spinach help, though supplementation is often necessary to replenish depleted stores.

Thyroid Problems and Metabolic Slowdown

Your thyroid gland controls your metabolic rate, which is the speed at which your body converts food into usable energy. When the thyroid underperforms (hypothyroidism), everything slows down. You feel sluggish, cold, foggy, and exhausted even after sleeping ten hours. Weight gain, dry skin, and constipation often tag along.

Thyroid function is assessed through a TSH blood test. For adults, a normal TSH range is roughly 0.27 to 4.2 micro-international units per milliliter. Values above that range suggest the thyroid isn’t producing enough hormone, forcing the pituitary gland to release more TSH to compensate. Hypothyroidism is especially common in women over 40 but can occur at any age. Treatment with thyroid hormone replacement typically restores energy levels within weeks to a few months.

Sleep Quality Matters More Than Hours

You can spend eight or nine hours in bed and still wake up drained if your sleep is being interrupted in ways you don’t notice. Obstructive sleep apnea is a prime example. Your airway partially or fully collapses during sleep, your oxygen drops, and your brain jolts you awake just enough to resume breathing. These arousals are so brief that most people have no memory of them, yet they can happen dozens of times per hour, preventing you from reaching the deep, restorative stages of sleep your body needs.

Sleep apnea severity is measured by how many breathing disruptions occur per hour. Fewer than five is considered normal. Five to fifteen is mild, fifteen to thirty is moderate, and thirty or more is severe. Snoring, gasping during sleep, morning headaches, and daytime sleepiness are classic signs. A partner who witnesses your breathing pauses is often the first clue. Risk factors include excess weight, a thick neck, and a naturally narrow airway, though lean people can have it too. A sleep study, sometimes done at home with a portable monitor, confirms the diagnosis.

Vitamin D and B12 Deficiencies

Vitamin D deficiency is remarkably common, particularly in people who spend most of their time indoors or live in northern latitudes. The NIH considers blood levels below 12 ng/mL deficient and levels between 12 and 20 ng/mL inadequate for overall health. Levels at or above 20 ng/mL are generally considered sufficient. Fatigue, muscle weakness, and low mood are frequent symptoms of insufficiency, and because they overlap with so many other conditions, vitamin D often goes unchecked.

Vitamin B12 plays a different but equally important role. Your body uses it to produce healthy red blood cells. When B12 drops below roughly 200 pg/mL, you can develop a type of anemia where red blood cells become abnormally large and inefficient at carrying oxygen. This leads to fatigue, weakness, numbness or tingling in the hands and feet, and difficulty concentrating. Vegans and vegetarians are at particular risk since B12 occurs naturally only in animal products. Older adults also absorb it less efficiently from food. Both deficiencies are detectable through standard blood work.

Depression and the Neuroscience of Exhaustion

Depression doesn’t always look like sadness. For many people, the dominant symptom is a crushing physical fatigue: feeling leaden, unmotivated, and unable to generate the energy for tasks that used to be effortless. This isn’t laziness or a character flaw. It has a neurochemical basis.

Serotonin, a brain chemical involved in mood regulation, also plays a direct role in fatigue. Elevated serotonin activity in the brain has been linked to both physical and mental exhaustion, essentially reducing the brain’s ability to generate and sustain the signals that activate your muscles during normal daily activities. Dopamine pathways involved in motivation and reward are also disrupted in depression, which is why even things you enjoy can feel like they require enormous effort. If your fatigue comes with persistent low mood, loss of interest, changes in appetite, or difficulty sleeping, a mental health evaluation is a reasonable next step.

Dehydration: A Simple Fix That’s Easy to Miss

Losing just 2% of your body weight in fluid, roughly 3 pounds for a 150-pound person, measurably impairs both physical performance and cognitive function. Most people associate dehydration with exercise or hot weather, but chronic mild dehydration from simply not drinking enough water throughout the day is surprisingly common. Coffee, while not as dehydrating as once believed, doesn’t fully replace water. If your urine is consistently dark yellow and you feel sluggish by mid-afternoon, increasing your water intake for a week is a free experiment worth trying before pursuing more complex explanations.

Chronic Fatigue Syndrome (ME/CFS)

When fatigue is severe, lasts longer than six months, and doesn’t improve with rest, it may meet the criteria for myalgic encephalomyelitis/chronic fatigue syndrome. ME/CFS is a distinct medical condition, not a catch-all label for being tired. Diagnosis requires three core symptoms: a substantial reduction in your ability to do activities you could handle before you got sick, unrefreshing sleep, and post-exertional malaise.

Post-exertional malaise is the defining feature. It means that physical, mental, or emotional effort that would have been fine before your illness now triggers a crash, a worsening of all symptoms that typically hits 12 to 48 hours after the activity and can last days or weeks. Standard blood tests for liver, kidney, and heart function, along with blood cell counts, all come back normal in ME/CFS patients, which is part of what makes diagnosis so frustrating. There is no single test for it. If this pattern sounds familiar, bring it up explicitly with your doctor, as many clinicians are still unfamiliar with the condition.

What Blood Work Can Reveal

If you’ve been tired for more than a few weeks and lifestyle adjustments like better sleep, more water, and regular movement haven’t helped, a basic set of blood tests can screen for the most common medical causes. A useful starting panel typically includes a complete blood count (which catches anemia), iron and ferritin levels, thyroid function (TSH), vitamin D, vitamin B12, blood sugar and hemoglobin A1C (to screen for diabetes), and markers of liver and kidney function. These tests are routine, inexpensive, and available through any primary care provider.

If everything comes back normal, that’s actually useful information. It narrows the search toward sleep disorders, mental health conditions, or less common causes. Keeping a two-week log of your sleep times, energy levels, meals, and physical activity before your appointment gives your doctor far more to work with than a vague report of being tired all the time.
