Why Am I Feeling So Tired? Common Causes Explained

Persistent tiredness almost always has a identifiable cause, and it’s rarely just “not getting enough sleep.” Fatigue that lingers for weeks typically traces back to one or more overlapping factors: how you eat, how you move, how you sleep, what’s happening in your blood, or what’s going on in your mind. The good news is that most of these causes are fixable once you know what to look for.

Your Iron Levels May Be Low

Iron deficiency is one of the most common and most overlooked reasons for chronic tiredness, especially in women. Most people assume low iron only matters if you’re anemic, but fatigue can set in well before your hemoglobin drops to anemic levels. A key study published by the American Society of Hematology found that women with ferritin levels at or below 50 ng/mL reported fatigue as a major complaint, even with normal hemoglobin. The effect was strongest in women whose ferritin fell below 15 ng/mL.

Here’s why iron matters so much for energy: your cells produce energy through tiny structures called mitochondria, and iron is essential to the enzymes that power that process. Iron shuttles charged particles across mitochondrial membranes, driving the production of ATP, which is your body’s energy currency. When iron is low, this system slows down. Your muscles, brain, and organs all generate less energy at the cellular level, and you feel it as deep, unshakable tiredness. A simple blood test measuring ferritin (your stored iron) can reveal the problem.

Your Thyroid Could Be Underperforming

Your thyroid gland controls your metabolic rate, which is the speed at which your body converts food into usable energy. When the thyroid underperforms (hypothyroidism), everything slows down: your heart rate, your digestion, your body temperature, and your energy. You might also notice weight gain, dry skin, or feeling cold all the time.

A TSH (thyroid-stimulating hormone) blood test is the standard screening tool. For most healthy, non-pregnant adults, normal TSH falls roughly between 0.5 and 5.0 uIU/mL, though the ideal range is narrower depending on your age and health. When TSH is elevated, it means your pituitary gland is working overtime trying to stimulate a sluggish thyroid. Thyroid problems are treatable with medication, and energy levels often improve within weeks of starting treatment.

You Might Not Be Sleeping as Well as You Think

Spending eight hours in bed doesn’t guarantee eight hours of restorative sleep. Obstructive sleep apnea, a condition where your airway repeatedly collapses during sleep, fragments your rest without you knowing it. You might wake up feeling exhausted, have morning headaches, or feel foggy all day, with no memory of waking up dozens of times overnight.

Sleep apnea severity is measured by how many breathing interruptions you have per hour. Fewer than five is normal. Between 5 and 15 is mild, 15 to 30 is moderate, and above 30 is severe. Even mild sleep apnea can leave you dragging through the day because it prevents your brain from cycling through the deep sleep stages that restore energy. Risk factors include excess weight, a thick neck circumference, and nasal congestion, but thin, young people get it too. A sleep study, which you can sometimes do at home, is the standard way to find out.

Depression Disguised as Exhaustion

Fatigue is one of the core diagnostic criteria for major depressive disorder, listed right alongside low mood and loss of interest in things you used to enjoy. What makes depression-related fatigue tricky is that it can show up before you feel emotionally “depressed.” You might just feel drained, heavy, unmotivated, and assume it’s a physical problem.

Depression fatigue has a particular quality: it doesn’t improve much with rest. You can sleep ten hours and still feel like you’re moving through wet concrete. If your tiredness comes with difficulty concentrating, changes in appetite, withdrawal from social life, or a sense that nothing sounds enjoyable anymore, the fatigue and the mood symptoms are likely connected. Treatment, whether therapy, medication, or both, often improves energy levels alongside mood.

What You Eat and Drink Matters More Than You’d Expect

Vitamin B12 is essential for making red blood cells and keeping nerve cells healthy. When B12 is low, your body can’t produce enough healthy red blood cells to carry oxygen efficiently, and fatigue is one of the earliest symptoms. People who eat little or no animal products, adults over 50 (who absorb B12 less efficiently), and anyone taking certain acid-reducing medications are at higher risk. A blood test can check your levels.

Blood sugar swings also play a role. Reactive hypoglycemia, a drop in blood sugar that typically hits within four hours of eating, can cause sudden waves of fatigue, shakiness, and brain fog. This is more likely after meals heavy in refined carbohydrates (white bread, sugary drinks, pastries), which trigger a sharp insulin spike followed by a crash. Pairing carbohydrates with protein, fat, or fiber slows the insulin response and keeps your energy more stable.

Dehydration is another surprisingly potent cause of tiredness. Losing just 1 to 2% of your body weight in fluid, an amount that can happen by mid-afternoon if you’re not drinking enough, is enough to reduce endurance and increase perceived effort during normal activities. For a 150-pound person, that’s only about 1.5 to 3 pounds of water loss. Most people don’t register this as thirst; they register it as fatigue.

The Caffeine Trap

Caffeine works by blocking receptors for adenosine, a molecule that builds up in your brain throughout the day and makes you feel sleepy. Block those receptors and you feel alert. The problem is that caffeine has a half-life of about 5 to 6 hours, meaning half of the caffeine from your 2 p.m. coffee is still active at 8 p.m. This can silently disrupt your sleep quality even if you fall asleep on time.

There’s a second layer to the problem. When you regularly block adenosine receptors with caffeine, your brain adapts by producing more receptors. This means you need more caffeine to get the same effect, and when the caffeine wears off, the extra receptors leave you feeling more tired than you would have without caffeine at all. If you drink caffeine daily and feel like you can’t function without it, you may be caught in this cycle. Gradually reducing your intake over a week or two resets the system, though you’ll feel worse before you feel better.

Sitting Too Much Drains Your Energy

This one feels counterintuitive: if you’re tired, how could exercise help? But inactivity itself is a powerful driver of fatigue. Research comparing sedentary and active individuals found dramatic differences at the cellular level. Sedentary people showed 36% lower activity in one key mitochondrial energy pathway and 34% less total capacity to produce energy in their muscle cells. Their ability to burn fat for fuel was cut roughly in half, and their capacity to clear lactate (the byproduct that makes muscles feel heavy and tired) was significantly reduced.

In practical terms, this means that the less you move, the less energy your cells are equipped to produce, which makes you feel more tired, which makes you move less. It’s a self-reinforcing cycle. Even modest increases in physical activity, like a daily 20-to-30-minute walk, begin reversing these cellular changes within weeks.

Fatigue That Follows an Infection

If your tiredness started after COVID-19, the flu, mono, or another viral illness, post-viral fatigue is a real and recognized phenomenon. Long COVID is the most studied version, and its hallmark symptom is post-exertional malaise: a worsening of fatigue that hits 12 to 48 hours after physical or mental effort and can last days or weeks. This isn’t normal “overdoing it” soreness. It’s a disproportionate crash from activity that would have been easy before.

Long COVID symptoms can emerge, resolve, and reappear unpredictably. They can also develop after a mild or even asymptomatic infection. There’s no lab test that definitively confirms it, so diagnosis is based on your history and symptom pattern. The fatigue seen in long COVID overlaps with what’s experienced in chronic fatigue syndrome and fibromyalgia, and management strategies from those conditions, particularly careful pacing of activity, are often helpful.

When Fatigue Signals Something More Serious

Most fatigue has a mundane, fixable explanation. But certain patterns warrant prompt medical attention. If your tiredness comes with a low-grade fever, unexplained weight loss, shortness of breath, or loss of appetite, it could point to an underlying infection or illness that needs investigation. The same applies if you consistently wake up exhausted despite what seems like adequate sleep, if you’ve lost the motivation to start your day in a way that feels different from your normal personality, or if activities that used to be easy now feel like a struggle.

As a general benchmark, fatigue lasting more than two weeks without an obvious explanation (like a stretch of bad sleep or a known illness) is worth bringing to a doctor. A basic workup including blood counts, iron and ferritin levels, thyroid function, B12, and blood sugar can rule out or identify the most common medical causes, often with a single blood draw.
