Why Am I Getting Body Acne? Causes, Triggers & Fixes

Body acne develops through the same core process as facial acne: excess oil production, clogged pores, bacteria, and inflammation. But the skin on your chest, back, and shoulders has larger pores and more oil glands than most of your face, which makes those areas especially prone to breakouts. What’s different about body acne is that clothing, sweat, and friction play a much bigger role in triggering and worsening it.

The Four Factors Behind Any Breakout

Every acne lesion starts with the same chain of events. Your oil glands produce sebum, a waxy substance that normally keeps skin moisturized. When too much sebum is produced, it mixes with dead skin cells and plugs the opening of a hair follicle. Bacteria that naturally live on your skin get trapped inside that clogged pore and multiply, which triggers your immune system to send inflammatory cells to the area. The result is a red, swollen bump.

Androgens, a group of hormones that includes testosterone, are a major driver. They increase both the amount of oil your skin produces and how quickly the cells lining your hair follicles turn over, making clogs more likely. This is why body acne often flares during puberty, around your period, during pregnancy, or when you start or stop hormonal birth control. It’s also why people who take androgen-blocking medications often see their acne improve, confirming the hormonal connection. Genetics matter too. If your parents dealt with body acne, your oil glands are more likely to be overactive.

Friction, Heat, and Sweat

A specific type of body acne called acne mechanica is triggered when skin is pressed or rubbed against clothing, gear, or equipment. It’s most common in athletes and active people because intense activity combines four aggravating factors at once: occlusion (skin covered tightly), heat, friction, and pressure. Backpack straps, sports bras, football pads, tight waistbands, and even prolonged contact with a chair or bed can all cause it.

The mechanism is straightforward. Friction irritates the skin surface and pushes sweat, oil, and dead cells deeper into pores. Heat increases oil production. Occlusion traps moisture against the skin, creating an ideal environment for bacteria. If you’ve noticed breakouts that follow the lines of your clothing, along your bra band, across your shoulders where a backpack sits, or on areas that press against gym equipment, friction is likely a primary trigger.

Your Products May Be Clogging Pores

Body lotions, sunscreens, and even laundry detergents can contribute to breakouts if they contain ingredients that clog pores. Ingredients are rated on a comedogenic scale from 0 (won’t clog pores) to 5 (very likely to). Some of the worst offenders are surprisingly common in body care products: coconut oil and cocoa butter both rate a 4, wheat germ oil rates a 5, and isopropyl myristate, a texture-enhancing ingredient found in many lotions, rates a 5.

If you’re moisturizing your chest and back with a rich body butter or using a hair conditioner that rinses down your back in the shower, those products could be the culprit. Look for labels that say “non-comedogenic” or “oil-free,” and consider rinsing your back and shoulders as the last step of your shower, after all conditioner and product residue has been washed away.

It Might Not Be Acne

Not every bumpy breakout on your body is actually acne. Folliculitis, an infection or inflammation of hair follicles, can look nearly identical but requires different treatment. Two common types are worth knowing about.

Bacterial folliculitis, usually caused by Staphylococcus aureus, produces small red or white pus-filled bumps that can appear anywhere you have hair follicles. Hot tub folliculitis shows up one to two days after sitting in contaminated water and typically fades on its own within a few days.

Fungal folliculitis (sometimes called “fungal acne”) is caused by Malassezia yeast that naturally lives on your skin. It usually appears on the upper chest and back as clusters of uniform, itchy bumps. The itch is a key differentiator: regular acne is rarely itchy, while fungal folliculitis almost always is. Sweat makes it worse. This distinction matters because fungal folliculitis won’t respond to standard acne treatments and may actually worsen with antibiotics, since killing off competing bacteria gives yeast more room to grow. If your body breakouts are persistently itchy and haven’t improved with typical acne products, a dermatologist can distinguish between the two.

What to Do After a Workout

The American Academy of Dermatology recommends showering immediately after exercise. Sitting in sweaty clothes gives bacteria time to multiply in clogged pores, and the longer sweat stays on your skin, the more it mixes with oil and dead cells to form plugs. If you can’t shower right away, changing out of damp clothing is the next best step.

What you wear during exercise matters as well. Synthetic fabrics like polyester trap heat and moisture against the skin, increasing sweat buildup and pore congestion. A clean, absorbent cotton shirt worn under sports equipment can reduce the four contributing factors of acne mechanica: occlusion, heat, friction, and pressure. Bamboo fabric is another option, as it’s naturally moisture-wicking and has antimicrobial properties that help prevent bacterial buildup. For sleeping, silk and bamboo outperform cotton, which absorbs moisture but can trap bacteria against the skin overnight.

Over-the-Counter Treatments That Work

Two active ingredients handle the majority of mild to moderate body acne. Benzoyl peroxide kills acne-causing bacteria on contact and is available in concentrations from 2.5% to 10%. If you’re new to it, start with a 2.5% wash and use it once daily. If you see minimal improvement after six weeks, step up to 5%. Body skin is thicker than facial skin and generally tolerates higher concentrations, but benzoyl peroxide will bleach towels and clothing, so rinse thoroughly.

Salicylic acid works differently. It’s oil-soluble, meaning it can penetrate into clogged pores and dissolve the mix of sebum and dead cells causing the blockage. Over-the-counter products range from 0.5% to 2% for daily washes and up to 7% for targeted treatments. It’s gentle enough to use morning and night. For body acne, a salicylic acid body wash that you leave on for 30 to 60 seconds before rinsing gives the ingredient time to work without being too harsh.

Using both ingredients together can be effective since they target different parts of the problem: salicylic acid clears the clog while benzoyl peroxide kills bacteria. Apply them at different times of day to minimize dryness and irritation.

When Body Acne Needs Stronger Treatment

Severe body acne, particularly on the back and chest, is harder to treat with topical products alone. The skin is difficult to reach, thicker, and covered by clothing most of the day. Current guidelines recommend systemic (oral) treatment for severe truncal acne, with isotretinoin considered the gold standard.

Isotretinoin dramatically reduces oil production and is the most effective option for acne that’s scarring, hasn’t responded to other treatments, or is causing significant psychological distress. Treatment typically continues until the skin is clear or nearly clear, then for an additional two to three months. Recent studies have found that lower doses can be just as effective as higher ones, with fewer side effects and better patient satisfaction. The medication requires regular monitoring and, for those who can become pregnant, strict pregnancy prevention due to the risk of birth defects.

For moderate body acne that isn’t responding to over-the-counter products, oral antibiotics combined with a topical retinoid are another option. A dermatologist can help determine which approach fits your situation based on how widespread the acne is, whether it’s leaving scars, and what you’ve already tried.
