Why Am I So Cold All the Time: Medical Reasons

Feeling cold all the time usually signals that your body is either producing less heat than it should, losing heat faster than normal, or not circulating warm blood efficiently to your extremities. While some people simply run cooler than others, persistent cold sensitivity often points to an underlying cause worth investigating, from thyroid problems and iron deficiency to low body fat or circulation issues.

How Your Body Controls Temperature

A small structure deep in your brain called the hypothalamus acts as your internal thermostat. It constantly monitors your core temperature and makes adjustments to keep you stable, triggering shivering to generate heat or narrowing blood vessels near the skin to conserve it. Normal body temperature can range from 97°F to 99°F depending on the person, time of day, and activity level. It tends to be lowest in the early morning. So some variation is completely normal.

When something disrupts this system, whether it’s a hormone imbalance, a nutrient shortage, or a blood flow problem, the result is often the same: you feel cold when everyone around you seems comfortable.

Thyroid Problems Are the Most Common Culprit

Thyroid hormones are major regulators of metabolism and heat production. When your thyroid gland is underactive (hypothyroidism), your metabolic rate drops, meaning your body burns less energy and generates less warmth. Cold intolerance is one of the hallmark symptoms, often alongside fatigue, weight gain, dry skin, and brain fog.

Hypothyroidism is especially common in women and becomes more likely with age. A simple blood test measuring thyroid-stimulating hormone (TSH) can detect it, and treatment with thyroid hormone replacement typically resolves the cold sensitivity over weeks to months.

Iron Deficiency and B12 Deficiency

Your body uses iron to make hemoglobin, the protein in red blood cells that carries oxygen. When iron is low, hemoglobin drops, oxygen delivery slows, and circulation suffers. The result is poor blood flow that makes you feel cold, particularly in your hands and feet. Fatigue, pale skin, and brittle nails are other common signs of iron deficiency anemia.

Vitamin B12 works similarly. Without enough B12, your body can’t produce adequate healthy red blood cells, which can leave you anemic and cold. B12 deficiency can also damage the protective coating around your nerves, potentially altering how you perceive temperature. Both deficiencies are diagnosed through routine blood work and are straightforward to treat with supplements or dietary changes, though it can take several weeks for levels to normalize and symptoms to improve.

Low Body Fat and Muscle Mass

Body composition plays a surprisingly large role in how warm you feel. Subcutaneous fat, the layer just beneath your skin, acts as insulation. People with more body fat lose less heat through the skin and shiver less in cold environments. People with very low body fat lack that buffer and feel the cold more intensely.

Muscle mass matters too. Your muscles generate heat even at rest, and people with more lean muscle can tolerate cooler temperatures more comfortably. This is one reason why someone who is very thin, has recently lost a significant amount of weight, or is undereating may suddenly notice they feel cold all the time. It’s not just about fat as insulation. Eating too few calories also reduces your metabolic rate, which means less internal heat production overall.

Circulation Problems

Raynaud’s Phenomenon

If your fingers or toes turn white or blue in response to cold or stress, you may have Raynaud’s. This condition causes the small blood vessels supplying your skin to narrow dramatically, cutting off blood flow temporarily. During an episode, affected areas feel cold and numb, then may throb or tingle as blood flow returns. Raynaud’s most commonly affects fingers and toes but can also hit the nose, lips, and ears.

The most common form, called primary Raynaud’s, isn’t linked to another medical condition and is more of a nuisance than a danger. Triggers include cold air, reaching into a freezer, putting hands in cold water, or even emotional stress. Secondary Raynaud’s is associated with autoimmune conditions and is less common but more serious. Diagnosis is usually based on your symptoms and a physical exam, sometimes with a cold stimulation test where a doctor monitors how your fingers respond to cold exposure.

Peripheral Artery Disease

When fatty deposits (plaque) build up inside the arteries that supply your legs and feet, blood flow to those areas drops. One telltale sign of peripheral artery disease is coldness in one lower leg or foot compared to the other side. This tends to develop gradually and is more common in people over 50, smokers, and those with diabetes or high cholesterol. Other symptoms include leg pain when walking, slow-healing sores on the feet, and weak pulses in the legs.

Medications That Make You Feel Cold

Certain medications can cause cold hands and feet as a side effect. Beta-blockers, commonly prescribed for high blood pressure, heart conditions, and anxiety, are a well-known example. They work by slowing the heart rate and reducing the force of each heartbeat, which can decrease blood flow to the extremities. Non-selective beta-blockers like propranolol are more likely to cause this because they affect blood vessels throughout the body, not just the heart. Older adults tend to be more susceptible to this side effect. If you started feeling colder after beginning a new medication, that connection is worth exploring with whoever prescribed it.

Other Contributing Factors

Dehydration reduces blood volume, which can impair circulation and make you feel chilly. Lack of sleep disrupts your body’s temperature regulation. Diabetes can damage nerves and blood vessels over time, leading to cold extremities. Chronic stress keeps your body in a state where blood is redirected away from the skin and toward vital organs, which can leave your hands and feet feeling icy.

Women tend to feel colder than men at the same ambient temperature, partly because they generally have less muscle mass, more body fat distributed centrally rather than peripherally, and hormonal fluctuations that affect blood vessel behavior. Age also plays a role. As you get older, your metabolic rate naturally decreases and circulation becomes less efficient.

Getting to the Bottom of It

If feeling cold is new, worsening, or accompanied by other symptoms like fatigue, unexplained weight changes, numbness, or skin color changes, blood work can help identify or rule out the most common causes. A complete blood count checks for anemia. A TSH test evaluates thyroid function. Iron and B12 levels can be measured directly. If Raynaud’s or a vascular issue is suspected, your doctor may order additional tests including markers of inflammation or autoimmune activity.

In the meantime, practical strategies help. Layering clothing, staying physically active to boost circulation, eating enough calories to support your metabolic rate, and keeping your core warm (which helps blood flow to your extremities) all make a real difference. But if you’re consistently colder than the people around you and it’s affecting your comfort or daily life, that’s a signal worth investigating rather than just bundling up and pushing through.
