Bleeding while taking medroxyprogesterone is frustrating, but it’s not uncommon. The medication is prescribed to regulate periods, treat heavy bleeding, or protect the uterine lining, yet some people find they’re still spotting or bleeding through treatment. What Testing Looks Like
If your bleeding hasn’t improved after two or three cycles, your provider will likely want to investigate further. The standard first step is a transvaginal ultrasound, which can measure the thickness of your uterine lining and identify fibroids or polyps. For a more detailed look at the uterine cavity, a saline-infused sonogram (where sterile fluid is used to expand the uterus during imaging) increases the sensitivity for detecting abnormalities like polyps or submucosal fibroids.
For people over 45, or anyone with risk factors for endometrial disease, an endometrial biopsy is a standard next step. This involves taking a small tissue sample from the lining to check for hyperplasia or abnormal cells. The procedure is done in a clinic setting and takes only a few minutes, though it can cause cramping similar to a strong period cramp. If a biopsy is inconclusive or can’t be completed, a more thorough tissue sampling under sedation may be recommended.
Signs That Bleeding Needs Urgent Attention
Some level of irregular bleeding on medroxyprogesterone can be normal, but certain patterns cross into territory that needs prompt evaluation. The CDC defines heavy menstrual bleeding as needing to change your pad or tampon after less than two hours, or passing clots the size of a quarter or larger. Soaking through one or more pads every hour for several hours in a row is a threshold that warrants contacting your provider right away.
Other signs to take seriously: bleeding that lasts longer than seven days per cycle, needing to wake up multiple times at night to change protection, or feeling dizzy, lightheaded, or unusually fatigued (which can signal that blood loss is affecting your iron levels or blood volume). These symptoms don’t necessarily mean something dangerous is happening, but they do mean the current treatment plan isn’t working well enough and needs to be reassessed.
Why It Matters to Follow Up
The temptation when bleeding persists is to assume the medication just isn’t working and either stop it or wait it out. Neither approach is ideal. Stopping abruptly can trigger a heavier withdrawal bleed, and waiting indefinitely means a potentially treatable cause goes unaddressed. If you’ve given the medication a fair trial of two to three cycles at the correct dose and you’re still bleeding, that information is diagnostically useful.
