When both eyes twitch at the same time, the most likely explanation is still the common, harmless kind of eyelid twitch, just happening to hit both sides. Stress, poor sleep, caffeine, and too much screen time are the usual triggers, and the twitching almost always stops on its own within days to weeks. In uncommon cases, bilateral twitching can signal a condition called benign essential blepharospasm, which is worth knowing about so you can tell the difference.

The Two Types of Eye Twitching

The most common type is called myokymia. It’s a small, involuntary flutter of the eyelid muscle that most people experience at some point in their lives. Myokymia typically affects just one eye at a time, either the upper or lower lid, but it can alternate between eyes or show up in both during the same stretch of time. This is the type that feels like a tiny pulse under the skin and is more annoying than anything else.

The less common type is benign essential blepharospasm. This condition starts as increased blinking in both eyes and can progress to forceful, involuntary squeezing of the eyelids shut. Unlike myokymia, blepharospasm is bilateral from the start and tends to worsen over time rather than come and go. It can become severe enough to interfere with driving, reading, and daily tasks. If your twitching feels more like strong, repeated blinking that you can’t control rather than a subtle flutter, that distinction matters.

Common Triggers for Both Eyes

When both eyes are twitching, it usually means a trigger is affecting your nervous system broadly rather than irritating one specific nerve. The most well-established triggers are fatigue, stress, caffeine, and alcohol. These don’t cause structural damage. They temporarily make the small muscles around your eyes more excitable, so they fire on their own.

Screen time is one of the strongest and most measurable risk factors. A 2024 study published in Cureus compared 103 people with persistent eyelid twitching to 103 people without it. The twitching group averaged nearly 7 hours of daily screen time, compared to about 5 hours in the control group. There was also a strong correlation between how long someone spent on screens and how long their twitching lasted. The likely mechanism: when you stare at a bright screen, the muscle that controls your eyelid stays partially contracted to reduce the light entering your eye. Hours of sustained contraction without adequate relaxation can leave that muscle twitchy and irritable, and because both eyes are doing the same work, both can be affected.

Dry eyes and eyelid inflammation (blepharitis) are another common cause that naturally affects both sides. When the surface of your eye is irritated or your tear film is unstable, it triggers a reflex contraction of the eyelid muscle. Eye care specialists call this “reflex blepharospasm.” People with this pattern often notice burning or dry, gritty eyes before the twitching starts. Treating the underlying dryness or inflammation typically resolves the twitching.

The Magnesium Question

If you’ve searched for eye twitching before, you’ve probably seen magnesium deficiency listed as a top cause. The evidence for this is surprisingly weak. A study that specifically tested whether low magnesium levels were linked to eyelid twitching measured magnesium, calcium, phosphate, and thyroid hormones in patients with twitching and compared them to people without symptoms. None of the lab results, including magnesium, showed a significant difference between the two groups. That doesn’t mean nutrition plays zero role in muscle function, but it does mean that reaching for a magnesium supplement isn’t the reliable fix it’s often made out to be. Addressing sleep, stress, and screen habits is more likely to help.

What Helps It Stop

For the common type of twitching, the fix is usually straightforward: reduce the triggers. Cut back on caffeine, prioritize sleep, and take regular breaks from screens. The 20-20-20 rule works well here: every 20 minutes, look at something 20 feet away for 20 seconds. This gives the muscles around your eyes a chance to relax. If your eyes feel dry or gritty, preservative-free artificial tears can calm the surface irritation that feeds the twitch reflex.

Cold compresses over closed eyes for a few minutes can also interrupt the spasm cycle. Most episodes of bilateral myokymia resolve within a few days to a couple of weeks once triggers are managed.

For benign essential blepharospasm, treatment looks different. Because the condition involves involuntary muscle contractions that don’t respond to lifestyle changes, targeted injections that temporarily relax the eyelid muscles are the standard approach. Relief typically begins about two and a half days after treatment and lasts around three months on average, though some people go six months or longer between sessions. An eye specialist or neurologist manages this treatment.

Signs That Need Medical Attention

Most bilateral twitching is harmless, but certain patterns warrant a visit to a healthcare provider. Pay attention if:

  • Duration: The twitching hasn’t stopped after a few weeks despite reducing triggers.
  • Intensity: Your eyelid closes completely with each twitch, or you have difficulty opening the eye.
  • Spread: The twitching has moved to other parts of your face or body.
  • Other symptoms: The area around your eye feels weak or stiff, your eye is red or swollen, there’s discharge, or your eyelids are drooping.

These signs can point toward blepharospasm or, rarely, a neurological condition that needs evaluation. A complete eyelid closure with each twitch is especially important to flag, because that’s the hallmark of blepharospasm rather than simple myokymia.
