Swollen ankles and feet happen when fluid leaks out of tiny blood vessels and collects in the surrounding tissue, a condition called edema. It’s one of the most common reasons people search for health information online, and the causes range from standing too long on a hot day to serious conditions involving the heart, kidneys, or liver. The key question is whether your swelling is temporary and harmless or a signal that something deeper needs attention.

How Fluid Ends Up in Your Ankles

Your smallest blood vessels, the capillaries, constantly balance two opposing forces: pressure pushing fluid out into surrounding tissue and protein-driven pressure pulling it back in. When that balance tips, fluid escapes faster than your body can reabsorb it. Gravity pulls that extra fluid downward, which is why ankles and feet bear the brunt.

Five things can tip the balance: higher pressure inside the capillaries (from standing all day or a heart that isn’t pumping efficiently), lower protein levels in your blood (which reduces the pull back into vessels), damage to the capillary walls that lets fluid leak more freely, blockages in the lymphatic system that normally drains excess fluid, or increased salt concentration in the tissue that draws water out of the bloodstream.

Everyday Causes That Aren’t Dangerous

Most ankle swelling is temporary and linked to lifestyle. Sitting or standing for hours, especially during travel, lets gravity pool fluid in your lower legs. Hot weather dilates blood vessels, making them leakier. Eating a salty meal can cause your body to retain extra water for a day or two. Tight shoes or socks with elastic bands can restrict drainage and worsen the problem.

Certain medications are also common culprits. Blood pressure drugs called calcium channel blockers are notorious for causing ankle swelling. Hormonal medications, some antidepressants, and anti-inflammatory painkillers can all promote fluid retention. If your swelling started shortly after beginning a new medication, that connection is worth discussing with your prescriber.

When Swelling Points to a Bigger Problem

Persistent or worsening swelling in both ankles often signals a systemic issue. The most common serious causes include:

  • Heart failure: When the heart can’t pump blood forward efficiently, it backs up in the veins of the legs. You might also notice shortness of breath when lying flat, fatigue, or abdominal bloating.
  • Kidney disease: Damaged kidneys lose their ability to filter excess fluid and salt. Swelling typically appears in the legs and around the eyes.
  • Liver disease: Cirrhosis reduces the liver’s production of albumin, a protein that keeps fluid inside blood vessels. Fluid builds up in the abdomen and legs.
  • Sleep apnea: Repeated drops in oxygen during sleep can raise pressure in the blood vessels of the lungs, which eventually backs up into the legs. Loud snoring, daytime sleepiness, and witnessed pauses in breathing are telltale signs.

Premenstrual hormone shifts can also cause cyclical swelling in the days before a period, and malnutrition severe enough to drop blood protein levels produces edema as well.

Chronic Venous Insufficiency

One of the most overlooked causes of long-term ankle swelling is chronic venous insufficiency, where the one-way valves in your leg veins stop working properly. Blood pools in the lower legs instead of returning to the heart, raising pressure inside the capillaries and forcing fluid into the tissue. This is the same process behind varicose veins, and the two conditions often appear together.

The swelling from venous insufficiency starts as soft pitting edema that goes down overnight but worsens through the day. Over months or years, persistently high venous pressure damages the skin. You may notice brownish discoloration around the ankles from iron deposits left by leaking red blood cells. Eventually the tissue can harden and thicken, and in advanced cases, slow-healing ulcers develop near the ankle bone. Catching it early, when swelling is the only symptom, makes management far simpler.

One-Sided Swelling Is Different

If only one ankle or foot is swollen, the cause is usually local rather than systemic. A deep vein thrombosis (blood clot in the leg) is the most urgent possibility. Clots typically cause sudden swelling in one leg along with warmth, redness, and a deep aching pain, especially in the calf. This requires same-day medical evaluation because a clot can break loose and travel to the lungs.

Other causes of one-sided swelling include a sprained or fractured ankle, an infection of the skin or soft tissue (cellulitis), or a Baker’s cyst behind the knee that has ruptured. An injury or insect bite you barely noticed can trigger enough inflammation to make one foot puff up noticeably.

Swelling During Pregnancy

Mild ankle swelling is normal in pregnancy, especially in the third trimester, because blood volume increases and the growing uterus compresses veins returning blood from the legs. But sudden or severe swelling, particularly in the hands and face, can be an early sign of preeclampsia, a condition marked by high blood pressure and protein in the urine. Headaches, blurred vision, light sensitivity, and rapid weight gain from fluid retention are additional warning signs. Propping your feet on a stack of pillows while lying on the couch is usually enough.

Reducing sodium intake directly limits how much water your body holds onto. For people with heart failure, guidelines recommend staying under 2,000 mg of sodium per day. Even without heart problems, cutting back on processed foods, canned soups, and restaurant meals can noticeably reduce swelling within a few days. Moving your ankles and calves regularly throughout the day, even just pumping your feet up and down while seated, activates the muscle pump that pushes venous blood upward.

Compression Stockings

Graduated compression stockings apply the most pressure at the ankle and gradually less as they go up the leg, helping veins push blood back toward the heart. For everyday swelling and early venous insufficiency, low compression stockings (under 20 mmHg) are available without a prescription and feel similar to snug athletic socks. Moderate compression (20 to 30 mmHg) is the standard recommendation for chronic venous edema and typically requires a fitting. Higher compression (above 30 mmHg) is reserved for more severe cases. The stockings work best when put on first thing in the morning before fluid has a chance to accumulate.

Signs That Need Prompt Attention

Most ankle swelling resolves with elevation, movement, and time.
