Sudden ankle swelling usually comes from one of a handful of causes: an injury, prolonged sitting or standing, a medication side effect, excess salt intake, or a more serious issue with your heart, kidneys, or veins. The key to figuring out what’s going on is whether the swelling affects one ankle or both, and whether you have other symptoms alongside it.
One-sided swelling points toward a local problem like a sprain, infection, or blood clot. Swelling in both ankles at once is more likely tied to something systemic, such as fluid retention from medication, diet, or an organ that isn’t working at full capacity.
Injuries and Infections
The most straightforward explanation for sudden swelling in one ankle is a physical injury. A sprained ankle, a stress fracture, or a torn tendon all trigger an inflammatory response that floods the area with fluid. You’ll usually remember the moment it happened, or at least recall an unusual amount of activity. The swelling tends to come with pain that gets worse when you put weight on the joint.
Cellulitis, a bacterial skin infection, can also cause one ankle to swell rapidly. The skin over the area typically turns red, feels warm to the touch, and may be tender. Cellulitis spreads quickly and needs antibiotic treatment, so this isn’t one to wait out.
Blood Clots in the Leg
A deep vein thrombosis (DVT) is a blood clot that forms in a deep vein, usually in the lower leg or thigh. It causes swelling that’s almost always on one side only, along with pain or cramping that often starts in the calf, warmth in the affected leg, and skin that may look red or purple. Some DVTs cause no noticeable symptoms at all, which is part of what makes them dangerous.
DVT is more likely if you’ve recently been immobile for a long stretch, such as a long flight, a road trip, or recovery from surgery. Pregnancy, birth control pills, obesity, and a family history of clotting disorders also raise your risk. If a clot breaks free and travels to the lungs, it becomes a pulmonary embolism, which can be life-threatening. Sudden shortness of breath, chest pain, or coughing up blood alongside leg swelling warrants emergency care.
When doctors suspect DVT, they typically order an ultrasound of the leg. For clots above the knee, this test catches about 96.5% of cases. For smaller clots in the calf, the detection rate drops to around 57%, so repeat imaging or additional testing is sometimes needed.
Medications That Cause Swelling
If your ankles puffed up soon after starting a new prescription, the medication is a likely culprit. Blood pressure drugs in the calcium channel blocker family are notorious for this. Amlodipine, one of the most commonly prescribed, causes ankle swelling in roughly 1 in 6 people who take it. The drug relaxes blood vessels, which allows more fluid to leak into surrounding tissue, especially in the lower legs where gravity pulls it down.
Other medications linked to ankle swelling include certain diabetes drugs, hormone therapies (including estrogen and testosterone), steroids, and even over-the-counter pain relievers like ibuprofen and naproxen. If you suspect a medication is behind your swelling, don’t stop taking it on your own. Your doctor can often switch you to an alternative that doesn’t have the same effect.
Too Much Salt or Too Long on Your Feet
A sudden spike in sodium intake, like a weekend of takeout or a heavily salted restaurant meal, can cause noticeable fluid retention. Your body holds onto extra water to keep sodium concentrations balanced in your blood, and gravity pulls that fluid toward your ankles and feet. That said, research in healthy adults suggests that even fairly high salt intake doesn’t always lead to visible edema on its own. If a salty meal causes dramatic swelling, it may be unmasking an underlying issue with your veins, heart, or kidneys that makes you more susceptible to fluid shifts.
Standing or sitting in one position for hours has a similar effect. Without the muscle contractions in your calves that help pump blood back up toward your heart, fluid pools in your lower legs. Long flights, desk jobs, and car trips are common triggers. This kind of swelling is usually mild and resolves once you move around and elevate your legs.
Heart, Kidney, and Liver Problems
When both ankles swell suddenly and you can’t pin it on an obvious cause, it’s worth considering whether an internal organ is struggling. This is especially true if the swelling doesn’t go away overnight or if it’s getting progressively worse.
In heart failure, the heart can’t pump blood efficiently, so blood backs up in the veins. This raises pressure inside the blood vessels of your legs and forces fluid out into the surrounding tissue. At the same time, reduced blood flow to the kidneys triggers a hormonal chain reaction that tells the body to retain even more salt and water, compounding the problem. The result is persistent swelling in both ankles and lower legs, often accompanied by shortness of breath, fatigue, or difficulty lying flat.
Kidney disease disrupts the body’s ability to filter excess fluid and waste. When the kidneys fall behind, fluid accumulates throughout the body, and the ankles are often where you notice it first. Liver cirrhosis can have a similar downstream effect by altering the proteins in your blood that normally keep fluid inside your vessels.
These conditions don’t usually appear out of nowhere. Most people have risk factors or earlier warning signs, but ankle swelling can sometimes be the symptom that finally gets noticed.
Chronic Venous Insufficiency
Your leg veins contain one-way valves that keep blood moving upward against gravity. When those valves weaken or become damaged, blood pools in the lower legs, a condition called chronic venous insufficiency. The swelling tends to build over the course of the day and improve after a night of sleep. Over time, you may notice varicose veins, skin discoloration around the ankles, or a heavy, achy feeling in your legs.
While this condition develops gradually, many people don’t register the swelling until it crosses a threshold that seems sudden. A day of extra standing, warm weather, or a salty meal can tip mild venous insufficiency into obvious puffiness.
How to Tell If Your Swelling Is Serious
Press a finger firmly into the swollen area for about five seconds and then release. If your finger leaves a visible dent that takes time to fill back in, that’s called pitting edema, and how deep and long-lasting the pit is gives a rough sense of severity. A shallow 2-millimeter indent that bounces back immediately is grade 1 and generally mild. A deep 8-millimeter pit that takes two to three minutes to refill is grade 4 and signals significant fluid overload.
Swelling that affects only one leg, appears with redness or warmth, or comes alongside chest pain and shortness of breath needs prompt medical evaluation. Swelling in both legs that develops rapidly, especially if you also feel unusually tired or short of breath, also warrants a call to your doctor sooner rather than later. Swelling in multiple parts of your body at once, not just the ankles but also the hands, face, or abdomen, is a particularly important signal that something systemic is going on.
Reducing the Swelling at Home
For mild, non-emergency swelling, a few practical steps can help. Elevating your legs above heart level for 20 to 30 minutes encourages fluid to drain back toward your core. Moving around periodically, especially calf raises or short walks, activates the muscle pump in your lower legs that pushes blood upward.
Compression socks are one of the most effective tools for managing recurring ankle swelling. For everyday puffiness from sitting, standing, or travel, over-the-counter socks in the 15 to 20 mmHg range provide meaningful relief. If your swelling is moderate, with visible pitting that lasts several seconds or varicose veins, a doctor may recommend the 20 to 30 mmHg range. Higher compression levels (30 mmHg and above) are reserved for conditions like lymphedema or chronic venous insufficiency and require a prescription.
Cutting back on sodium helps if dietary salt is contributing to your fluid retention. Processed foods, canned soups, deli meats, and restaurant meals are the biggest sources for most people, often delivering far more sodium than you’d guess from taste alone.
