Swollen ankles happen when fluid leaks out of your blood vessels and collects in the soft tissue around your feet and lower legs. This is called edema, and it ranges from a harmless side effect of a long day on your feet to a signal that something more serious is going on with your heart, kidneys, or veins. The cause usually depends on whether the swelling affects one leg or both, how quickly it appeared, and what other symptoms came with it.
How Fluid Ends Up in Your Ankles
Your body constantly moves fluid between your bloodstream and surrounding tissues. Pressure inside your blood vessels pushes fluid out, while proteins in your blood pull it back in. Your lymphatic system acts as a cleanup crew, draining whatever excess fluid remains. Swelling happens when this balance tips: either too much fluid is being pushed out, not enough is being pulled back in, or the lymphatic system can’t keep up.
Gravity plays a major role. Fluid naturally pools at the lowest point of your body, which is why ankles and feet swell before anything else. Sitting or standing for hours without moving your legs makes this worse because your calf muscles aren’t contracting and pushing blood back up toward your heart.
Everyday Causes That Are Usually Harmless
Most ankle swelling is temporary and resolves on its own. Hot weather dilates your blood vessels, allowing more fluid to seep into surrounding tissue. A long flight, a road trip, or a desk job that keeps you seated for hours lets gravity do its work unopposed. Eating a salty meal causes your body to retain extra water, and that water tends to settle in your lower legs. A minor ankle sprain or twist can also cause localized swelling from inflammation.
If your swelling goes down overnight, appears equally in both legs, and you feel otherwise fine, one of these everyday triggers is the most likely explanation.
Medications That Cause Swelling
Several common medications cause ankle swelling as a side effect. Blood pressure drugs called calcium channel blockers (amlodipine and nifedipine are among the most prescribed) are frequent culprits. They work by relaxing blood vessels, but this relaxation increases pressure inside the tiny capillaries in your legs, pushing more fluid into surrounding tissue.
Other medications linked to ankle swelling include diabetes drugs in the thiazolidinedione class (like pioglitazone), which cause sodium and fluid retention in the kidneys. Anti-inflammatory painkillers, steroids, and certain hormone therapies can also contribute. If your swelling started shortly after beginning a new medication, that connection is worth flagging with your prescriber.
Chronic Venous Insufficiency
Your leg veins contain one-way valves that keep blood moving upward toward your heart. When those valves weaken or fail, blood flows backward and pools in the lower legs. This pooling raises pressure inside the veins, forces more fluid into surrounding tissue, and over time stretches the veins further, damaging even more valves. It’s a cycle that tends to worsen gradually.
Chronic venous insufficiency is one of the most common causes of persistent ankle swelling, especially in people over 50 or those who spend long hours standing. You might also notice varicose veins, skin discoloration around the ankles (often brownish), or a heavy, aching feeling in your legs that improves when you elevate them. Compression stockings are the first-line approach for managing it, as they apply external pressure that helps the valves do their job.
Heart, Kidney, and Liver Problems
When ankle swelling appears in both legs and doesn’t fully resolve with rest, it can signal a problem with one of the major organs responsible for managing fluid in your body.
In congestive heart failure, the heart can’t pump blood efficiently, so it backs up in the veins of the legs. This raises the pressure inside those veins and pushes fluid into the tissue. The swelling typically worsens throughout the day and may be accompanied by shortness of breath, fatigue, or difficulty lying flat at night.
Kidney disease reduces your body’s ability to filter excess fluid and salt from the blood. The resulting swelling usually shows up in the legs and around the eyes. In a related condition called nephrotic syndrome, the kidneys lose too much protein into the urine. Since blood proteins are what pull fluid back into your vessels, losing them means fluid accumulates in the tissue instead.
Liver damage from cirrhosis can cause fluid buildup in both the abdomen and the legs. This happens partly because a damaged liver produces fewer of the proteins that keep fluid in the bloodstream.
One Swollen Leg: Blood Clot Warning Signs
Swelling that affects only one leg deserves immediate attention because it can indicate a deep vein thrombosis, a blood clot in one of the large veins of the leg. Along with swelling, a DVT often causes pain or cramping (usually starting in the calf), warmth in the affected area, and skin that looks red or purple. However, some blood clots cause no noticeable symptoms at all.
A DVT is dangerous because the clot can break loose and travel to the lungs, creating a life-threatening blockage called a pulmonary embolism. Risk factors include recent surgery, prolonged immobility (like a long flight or bed rest), pregnancy, cancer, and use of hormonal birth control. If you have sudden, unexplained swelling in one leg, especially with pain or skin color changes, seek medical evaluation promptly.
Swelling During Pregnancy
Some ankle swelling is normal during pregnancy, particularly in the third trimester. Your blood volume increases significantly, and the growing uterus puts pressure on the veins that return blood from your legs. Mild, symmetrical swelling in the feet and ankles that comes and goes is expected.
What isn’t normal is sudden swelling, particularly in the face and hands, which can be a sign of preeclampsia. This serious pregnancy complication involves dangerously high blood pressure and can harm both mother and baby. Sudden weight gain, severe headaches, blurred vision, upper belly pain, or severe shortness of breath alongside swelling are signals to get emergency care immediately.
How Swelling Is Evaluated
One simple way to assess ankle swelling is the pitting test: press a finger firmly into the swollen area for several seconds, then release. If an indentation remains, that’s pitting edema, and its severity is graded on a scale from 1 to 4. A grade 1 leaves a shallow dent (about 2mm) that bounces back immediately. A grade 4 leaves a deep impression of about 8mm that takes two to three minutes to fill back in.
Beyond the physical exam, the pattern of swelling tells a lot. Both ankles swelling equally points toward systemic causes like heart failure, kidney disease, medications, or venous insufficiency. One-sided swelling raises concern for a blood clot, injury, or infection. Swelling that resolves overnight suggests a milder cause than swelling that persists around the clock.
Reducing Ankle Swelling at Home
For mild, everyday swelling, a few practical changes can make a noticeable difference. Elevating your legs above the level of your heart for 15 to 20 minutes several times a day helps fluid drain back toward your core. Moving your ankles in circles or flexing your feet while seated keeps your calf muscles active and pumping blood upward. If you sit or stand for long periods, take short walking breaks every hour.
Cutting back on sodium is one of the most effective dietary changes. The American Heart Association recommends staying under 1,500 mg of sodium per day, though even reducing to 2,000 mg daily can help if your current intake is high. Most excess sodium comes from processed and restaurant food rather than the salt shaker, so reading labels matters more than skipping table salt. Compression socks or stockings, available at most pharmacies, apply gentle pressure that supports your veins and prevents fluid from pooling.
If your swelling is new, persistent, worsening, or accompanied by pain, shortness of breath, or skin changes, these home strategies aren’t a substitute for figuring out the underlying cause. The range of possibilities spans from completely benign to quite serious, and the difference often comes down to details that a physical exam and basic lab work can sort out quickly.
