Persistent eye puffiness is almost always caused by fluid collecting in the thin, loose skin around your eyes. This area has the thinnest skin on your body and very little underlying muscle or fat to keep things taut, which means even small shifts in fluid balance show up here first. The cause can be as simple as a salty dinner or sleeping flat, but chronic puffiness that never seems to go away can point to allergies, aging changes, or occasionally a systemic health issue worth investigating.
Why Your Eyes Swell Overnight
Most people notice puffiness is worst in the morning, and there’s a straightforward reason. When you lie flat for hours, gravity no longer pulls fluid down toward your legs and feet. Instead, it redistributes evenly, and some of it pools in the loose tissue around your eye sockets. At the same time, you stop blinking. During the day, each blink acts like a tiny pump that pushes fluid and debris away from your eyes. Once your eyes close for sleep, that pump shuts off. Fluid accumulates, inflammatory compounds in your tear film build up, and by morning your lids look swollen.
This type of puffiness typically fades within 30 to 60 minutes of being upright as gravity and blinking resume their draining work. If your puffiness clears up by mid-morning, the overnight fluid shift is likely the main culprit, and it becomes more pronounced with age as the tissues around your eyes loosen.
Salt, Alcohol, and Other Lifestyle Triggers
A high-salt meal is one of the fastest ways to trigger visible puffiness. Sodium causes your body to hold onto water, and that extra fluid gravitates toward the loosest tissue available, which is the skin around your eyes. If your diet is consistently high in sodium (processed foods, restaurant meals, salty snacks), the puffiness can become a daily baseline rather than an occasional annoyance. Reducing salt intake is one of the most commonly recommended first steps for managing chronic periorbital swelling.
Alcohol works through a different path but produces a similar result. It dehydrates you overall, which triggers your body to compensate by retaining fluid. It also disrupts sleep quality, meaning you spend more time in lighter sleep stages with less efficient fluid regulation. Crying before bed, screen-related eye strain, and sleep deprivation all contribute too. If you can link your worst mornings to specific habits the night before, that pattern is worth paying attention to.
Allergies: The Most Overlooked Cause
If your puffiness comes with itching, redness, or watery eyes, allergies are a strong candidate. When your immune system encounters an allergen like pollen, dust mites, or pet dander, specialized cells in your eye tissue release histamine and other inflammatory chemicals. These chemicals make tiny blood vessels leak fluid into the surrounding tissue, producing that swollen, sometimes slightly purplish look around the eyes.
Seasonal allergies cause puffiness that comes and goes with pollen counts, typically peaking in spring and fall. But perennial allergies, triggered by indoor allergens like dust or mold, can keep your eyes puffy year-round. This is one of the most common reasons someone’s eyes are “always” puffy without an obvious explanation. The swelling may be mild enough that you don’t connect it to allergies, especially if itching isn’t your dominant symptom. More chronic forms of allergic eye inflammation can even cause lasting thickening of the eyelid skin over time.
Over-the-counter antihistamine eye drops or oral antihistamines often produce noticeable improvement within days if allergies are the driver. If the puffiness responds to antihistamines, that essentially confirms the diagnosis.
Age-Related Changes in Orbital Fat
There’s an important distinction between fluid-based puffiness and the kind that comes from structural changes as you age. The fat that normally cushions your eyeball is held in place by a thin membrane called the orbital septum. Over time, this membrane weakens, allowing fat to push forward and create permanent-looking bags under the eyes. Obesity and previous eye trauma can accelerate this process.
The key difference: fluid-based puffiness fluctuates throughout the day and responds to cold compresses, sleep position changes, or dietary adjustments. Fat pad herniation looks roughly the same morning and night and doesn’t respond to lifestyle changes. If your under-eye bags appeared gradually after your 30s or 40s and never really change, you’re likely dealing with structural fat displacement rather than fluid retention. This is a cosmetic issue, not a medical one, though it can coexist with fluid-based swelling to make things look worse in the morning.
Thyroid Disease and Eye Swelling
Thyroid eye disease, most commonly associated with an overactive thyroid (Graves’ disease), causes a distinctive pattern of eye changes that goes well beyond simple puffiness. The immune system attacks the tissue behind and around the eyes, causing inflammation and swelling that can make the eyes appear to bulge forward. Symptoms include swollen, inflamed eyelids, eye irritation, light sensitivity, pain, difficulty moving the eyes, and sometimes double vision.
This condition doesn’t look or feel like ordinary morning puffiness. The swelling tends to be persistent, progressive, and accompanied by other eye symptoms. If your puffiness is paired with a feeling of pressure behind your eyes, visible bulging, or any changes in your vision, blood tests for thyroid hormone levels and thyroid antibodies can help identify or rule out this cause. Lasting changes from thyroid eye disease can include permanently retracted eyelids and protruding eyes, so early evaluation matters.
Kidney Problems and Protein Loss
Chronic, symmetrical puffiness around both eyes, especially if it’s present throughout the day and accompanied by swelling in your ankles or feet, can be a sign of kidney dysfunction. In a condition called nephrotic syndrome, the kidneys’ filtering units become damaged and allow a protein called albumin to leak into the urine. Albumin normally acts like a sponge in your bloodstream, keeping fluid inside your blood vessels. When albumin levels drop, fluid seeps out into surrounding tissues, and the loose skin around your eyes is one of the first places it shows up.
This type of puffiness is typically severe, doesn’t fluctuate much with time of day, and is accompanied by foamy urine (from the protein loss) and significant ankle swelling. It’s uncommon compared to allergies or lifestyle factors, but it’s worth knowing about because it requires medical treatment.
What Actually Helps Reduce Puffiness
Cold compresses are the most universally recommended home treatment. Interestingly, clinical evidence that cold actually reduces the swelling itself is limited. Patient satisfaction is high, and many dermatologists still recommend it, but the effect may be more about comfort and temporary tightening of the skin than true fluid reduction. A chilled spoon, cold tea bags, or a gel mask held over closed eyes for 10 to 15 minutes can still make a visible difference in how you look, even if the mechanism isn’t fully proven.
Sleeping with your head slightly elevated (an extra pillow or a wedge) reduces overnight fluid pooling by working with gravity instead of against it. This is one of the simplest and most effective changes for people whose puffiness is worst in the morning.
Skincare products containing caffeine are widely marketed for under-eye bags. Caffeine is a vasoconstrictor, meaning it tightens blood vessels and temporarily limits fluid leakage, producing a visible “deflating” effect. The catch is that this effect is short-lived, often lasting only a few hours. There’s also a concern that regular use of vasoconstrictors can weaken blood vessel walls over time, potentially making the problem worse in the long run. It’s a reasonable occasional fix but not an ideal daily strategy.
Signs That Puffiness Needs Medical Attention
Most puffy eyes are a nuisance, not a danger. But certain accompanying symptoms shift the picture. Sudden vision loss in one eye, pain when moving your eyes, significant changes in color perception, or intense eye pain with deep redness all warrant prompt evaluation. These can indicate inflammation of the optic nerve, blood vessel blockages in the retina, or deep inflammation of the eye wall, all of which are considered urgent.
Puffiness that is rapidly worsening, only affects one eye, or is paired with fever, facial swelling, or severe headache also falls outside the range of normal fluid retention. And if your puffiness is constant, doesn’t respond to any lifestyle changes, and comes with swelling elsewhere in your body, testing for kidney function, thyroid levels, or other systemic conditions is a reasonable next step.
