Why Are My Hands and Feet Cold: Causes and Fixes

Cold hands and feet are usually your body’s normal response to cool temperatures or stress. Your nervous system narrows blood vessels in your extremities to keep warm blood closer to your vital organs, and your fingers and toes cool down as a result. But when your hands and feet stay cold even in warm environments, or the coldness comes with color changes, numbness, or fatigue, something else may be going on.

How Your Body Prioritizes Heat

Your sympathetic nervous system constantly monitors your core temperature. When it senses cold, it triggers vasoconstriction, a tightening of the muscles around blood vessels near the skin’s surface. This shrinks the space inside those vessels, reducing blood flow to your skin, fingers, and toes so that warm blood stays near your heart, lungs, and brain. It’s an efficient survival mechanism, but it means your extremities are always the first to get cold and the last to warm up.

Some people have a more aggressive version of this response than others. Women tend to experience colder extremities than men, partly due to differences in body composition and hormone levels that influence how blood vessels respond to temperature changes. If you’ve always run cold but feel fine otherwise, your nervous system is simply more reactive, not malfunctioning.

Stress and the Fight-or-Flight Response

You don’t need to be in a cold room to get cold hands. Emotional stress and anxiety trigger the same narrowing of blood vessels. During a stress response, your heart beats faster and pushes blood toward your muscles, heart, and other vital organs. Blood flow to your skin and extremities drops. This is why your hands can feel icy during a tense meeting or before a presentation, even if the room is perfectly warm. Chronic stress keeps this system partially activated, which can make persistently cold hands a daily issue rather than an occasional one.

Iron Deficiency and Anemia

Iron deficiency is one of the most common medical reasons for chronically cold hands and feet. Your body needs iron to make hemoglobin, the molecule inside red blood cells that carries oxygen through your bloodstream. Without enough hemoglobin, less oxygen reaches your tissues, and your body compensates by redirecting blood flow away from your extremities toward your core. The result: cold fingers and toes, pale skin, and persistent fatigue.

Other signs of iron deficiency include brittle nails, headaches, dizziness, and unusual cravings for non-food items like ice. It’s especially common in women with heavy periods, people on restrictive diets, and anyone with chronic blood loss from conditions like ulcers. A simple blood count can confirm whether your iron and hemoglobin levels are low.

Underactive Thyroid

Your thyroid gland sets the pace of your metabolism. When it produces too little thyroid hormone, a condition called hypothyroidism, your basal metabolic rate drops and your body generates less internal heat. Research comparing people with hypothyroidism to healthy controls found significantly lower metabolic rates (roughly 1,196 calories per day versus 1,363) along with measurably lower core body temperatures. That metabolic slowdown makes you feel cold all over, but hands and feet are hit hardest because they’re farthest from your core.

Other clues that your thyroid might be underperforming include unexplained weight gain, dry skin, thinning hair, constipation, and feeling sluggish or mentally foggy. Hypothyroidism is diagnosed with a blood test measuring thyroid hormone levels and is very treatable once identified.

Raynaud’s Phenomenon

If your fingers or toes turn distinctly white, then blue, then red in response to cold or stress, you likely have Raynaud’s phenomenon. It affects the small blood vessels supplying your skin, causing them to narrow far more dramatically than normal. During an episode, the affected areas first go pale as blood flow cuts off, then turn bluish as oxygen levels drop. When circulation returns, they may flush red and throb, tingle, or swell.

Common triggers include reaching into a freezer, holding a cold drink, or even just walking outside on a chilly day. Emotional stress can also set off an episode. Most people have primary Raynaud’s, meaning it happens on its own without an underlying disease. It’s uncomfortable but not dangerous. Secondary Raynaud’s, which is linked to autoimmune conditions like lupus or scleroderma, tends to be more severe and can cause skin sores or tissue damage.

To distinguish between the two types, doctors sometimes use a test called nailfold capillaroscopy, examining the tiny blood vessels at the base of your fingernails under magnification. Blood tests for markers of autoimmune activity, such as antinuclear antibodies and inflammatory markers, can help rule out an underlying condition.

Peripheral Artery Disease

When cold feet come with leg pain during walking, slow-healing sores, or noticeable changes to the skin on your legs, the cause may be peripheral artery disease (PAD). This condition develops when fatty deposits build up inside the arteries that supply your legs and feet, physically restricting blood flow. One telling sign is coldness in one foot but not the other, or a noticeably weaker pulse in one leg.

PAD tends to develop gradually. Early on, you might notice cramping in your calves or thighs during walks that goes away with rest. Over time, other signs appear: shiny skin on the legs, slower toenail growth, hair loss on the legs, or color changes in the feet. The risk is highest if you smoke, have diabetes, high blood pressure, or high cholesterol. PAD is a sign of broader cardiovascular disease and worth investigating, because the same plaque buildup affecting your legs is often present in blood vessels elsewhere in the body.

Vitamin B12 Deficiency

Low B12 doesn’t make your hands physically colder in the way iron deficiency does, but it can make them feel cold. B12 deficiency damages the nerves that carry sensory signals from your extremities, producing numbness, tingling, and pins-and-needles sensations that your brain can interpret as coldness. These neurological symptoms can also include problems with balance, muscle weakness, and mood changes ranging from mild anxiety to confusion. B12 deficiency is common in people over 60, vegetarians, vegans, and anyone with digestive conditions that impair nutrient absorption.

What You Can Do

If your cold hands and feet are simply a strong response to temperature, layering is your best tool. Insulated gloves, wool socks, and keeping your core warm (not just your extremities) help the most, because when your core is warm, your nervous system relaxes its grip on blood vessels in your hands and feet. Regular physical activity improves circulation over time by keeping blood vessels flexible and responsive.

Cutting back on caffeine and nicotine can also help, since both cause blood vessels to constrict. If stress is a factor, techniques that calm the sympathetic nervous system, like slow breathing, meditation, or even just holding a warm drink, can ease the vasoconstriction that chills your fingers.

When cold extremities persist in warm settings, come with color changes or numbness, or arrive alongside fatigue and other new symptoms, blood work can quickly check for the most common culprits: iron levels, thyroid function, B12 status, and markers of inflammation or autoimmune activity. Most of these conditions are straightforward to treat once identified, and the cold hands tend to resolve along with them.
