Why Are My Teeth So Sensitive and How to Stop It

Tooth sensitivity happens when the protective outer layer of your teeth wears down or your gums pull back, exposing the softer inner layer called dentin. This is remarkably common, affecting anywhere from 12% to over 40% of dental patients depending on the population studied. The sharp, sudden pain you feel from cold drinks, hot food, or even a breath of cold air has a specific biological explanation, and in most cases, the cause is treatable.

What’s Happening Inside Your Tooth

Dentin, the layer beneath your enamel, is full of microscopic tubes filled with fluid. When something hot, cold, sweet, or acidic reaches those tubes, the fluid inside expands or contracts. That movement triggers pressure-sensitive nerve endings, producing the jolt of pain you recognize as sensitivity. Think of it like a tiny hydraulic system: the fluid moves, the nerve fires, and you wince.

The key point is that sensitivity almost always means something has removed or bypassed the barrier that normally shields those fluid-filled tubes. Enamel covers the visible part of your tooth, and a thinner material called cementum covers the root. When either one is compromised, the nerve inside becomes far easier to stimulate.

Enamel Erosion From Acid

Enamel starts to dissolve at a pH of about 5.5. For reference, water is neutral at 7.0, so anything meaningfully acidic can do damage over time. Citrus fruits, soda, wine, sports drinks, and vinegar-based foods all fall below that threshold. The damage is cumulative: occasional exposure is manageable, but frequent acid contact without giving your mouth time to recover gradually thins the enamel and exposes dentin.

Acid reflux (GERD) is a particularly aggressive cause because stomach acid reaches your teeth repeatedly, often without you realizing it. This is worst at night, when you swallow less frequently and produce less saliva. Saliva is your mouth’s natural defense, neutralizing acid and washing it away. Many acid reflux medications also cause dry mouth, which further reduces that protection. If you notice sensitivity mainly on the inside surfaces of your teeth or on chewing surfaces, acid reflux may be a factor worth investigating.

Gum Recession

Your tooth roots aren’t covered with enamel. They’re covered with cementum, which is much weaker. When your gums recede, those roots become exposed, and sensitivity follows. You’ll typically notice it near the gumline, especially when eating or drinking something cold.

The most common causes of gum recession include brushing too hard or with a stiff-bristled toothbrush, plaque and tartar buildup, gum disease, tobacco use, misaligned teeth, and even lip or tongue piercings that rub against the gum tissue. Some people also have naturally thinner gum tissue that recedes more easily with age.

Grinding Your Teeth

If you clench or grind your teeth (a condition called bruxism), you may be wearing through your enamel mechanically. The National Institute of Dental and Craniofacial Research lists worn enamel, flattened or chipped teeth, and increased sensitivity as hallmark signs. Many people grind at night without knowing it, so the first clue is often waking up with a sore jaw or noticing that your teeth look flatter than they used to. A dentist can usually spot the wear pattern during a routine exam.

Whitening Products

Teeth whitening is one of the most common triggers for sudden sensitivity in otherwise healthy teeth. The peroxide in whitening products temporarily penetrates enamel and irritates the nerve. A study using professional-strength 35% hydrogen peroxide found that sensitivity peaked right after each whitening session but gradually faded and did not exceed 48 hours. If your sensitivity started shortly after whitening (whether in-office or with at-home strips), it will very likely resolve on its own within a couple of days. Spacing out whitening sessions and using a desensitizing toothpaste during treatment can help.

When Sensitivity Signals Something Deeper

Not all tooth pain is simple sensitivity. The nerve inside your tooth (the pulp) can become inflamed from deep cavities, cracks, or repeated dental procedures. There’s a practical way to tell the difference.

With ordinary sensitivity, the pain starts when a stimulus hits the tooth (cold, sweet, air) and stops within one to two seconds after the stimulus is removed. If your pain lingers for minutes after you remove what triggered it, or if it shows up spontaneously with no trigger at all, that points to deeper nerve inflammation that won’t resolve on its own. Pain triggered mainly by heat rather than cold is another red flag, as is a tooth that feels “high” when you bite down, like it’s sitting taller than the others. These patterns suggest the nerve tissue is seriously compromised and needs professional treatment.

What You Can Do at Home

Desensitizing toothpaste is the simplest first step. These products contain compounds that gradually block the fluid-filled tubes in your dentin, reducing how much stimulus reaches the nerve. They typically take a few weeks of consistent use before you notice a meaningful difference, so patience matters.

Beyond toothpaste, a few habits make a significant difference. Switch to a soft-bristled toothbrush and use gentle pressure. After eating or drinking anything acidic, wait at least 30 minutes before brushing, because acid softens enamel temporarily and brushing right away accelerates the erosion. Drinking water after acidic foods helps rinse your mouth in the meantime. If you grind your teeth at night, a custom mouthguard protects your enamel from further wear.

Professional Treatment Options

If home care isn’t enough, there are in-office treatments that provide faster relief. Two of the most common are fluoride varnish and dental bonding agents, both applied directly to the sensitive areas.

A randomized trial comparing the two found that both reduced sensitivity, but bonding agents worked faster and more completely. Patients who received a bonding agent saw their pain scores drop from roughly 6.9 out of 10 at baseline to 1.8 at four weeks. Those who received fluoride varnish dropped from 7.0 to 3.2 over the same period. About 65% of patients in the bonding group felt relief within 24 hours. These are single-application treatments, though they may need to be reapplied over time as the coating wears.

For sensitivity caused by significant gum recession, a dentist may recommend gum grafting to cover exposed roots. For cracked teeth or large areas of enamel loss, crowns or bonded restorations can rebuild the protective barrier permanently. The right approach depends entirely on what’s causing your sensitivity, which is why identifying the underlying reason matters more than just managing the symptom.
