Why Are Social Determinants of Health Important?

Social determinants of health matter because they shape roughly 80 to 90 percent of what keeps people healthy or makes them sick. Medical care accounts for only 10 to 20 percent of the modifiable contributors to health outcomes. The rest comes from the conditions where people live, work, learn, and grow: income, education, housing, food access, and community safety. Understanding these factors explains why two people with the same diagnosis can have vastly different outcomes depending on their zip code.

What Social Determinants Actually Are

The U.S. Department of Health and Human Services defines social determinants of health as “the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.” These conditions fall into five domains: economic stability, education access and quality, health care access and quality, neighborhood and built environment, and social and community context.

None of these are medical conditions. They’re the circumstances surrounding your life that determine whether you can afford healthy food, whether your home has mold in the walls, whether you have reliable transportation to a pharmacy, or whether chronic stress from financial instability is slowly wearing down your body. They set the stage long before a doctor enters the picture.

How Your Neighborhood Shapes Your Lifespan

One of the starkest illustrations of why social determinants matter is the life expectancy gap between neighboring communities. Research from Virginia Commonwealth University found that life expectancy can differ by as much as 20 years between neighborhoods only about five miles apart. That gap isn’t explained by genetics or personal choices alone. It reflects differences in air quality, walkability, access to grocery stores, safety from violence, and proximity to health care.

Housing quality is a major piece of this. People with the lowest incomes are often forced into substandard rentals with mold, vermin, water leaks, and inadequate heating or cooling, all of which directly cause or worsen respiratory conditions, infections, and mental health problems. A study of newly homeless individuals entering the New York City shelter system found that 17 percent had asthma, 17 percent had hypertension, 6 percent had diabetes, 35 percent had major depression, and 53 percent had a substance use disorder. Those rates are significantly higher than the general population.

Children are especially vulnerable. Moving three or more times in a single year has been linked to chronic health conditions and poor physical development. The instability itself becomes a health risk, disrupting routines, school attendance, and the social connections that buffer against stress.

Food Access and Chronic Disease

Adults who experience food insecurity are two to three times more likely to develop type 2 diabetes than those with reliable access to nutritious meals. The connection is straightforward: when healthy food is too expensive or unavailable, people turn to cheaper options that tend to be high in added sugars, saturated fat, and sodium. Those foods provide calories but accelerate the metabolic damage that leads to diabetes, heart disease, and kidney problems.

For people who already have diabetes, food insecurity makes the condition harder to manage. Controlling blood sugar requires consistent access to balanced meals, which is difficult when your grocery budget fluctuates or your nearest store stocks mostly processed food. Poorly managed blood sugar increases the risk of complications like vision loss, kidney disease, and lower-limb amputations. The disease doesn’t just progress faster because of biology. It progresses faster because the environment makes proper management nearly impossible.

How Social Stress Gets Under Your Skin

Social determinants don’t just affect health through behavior and access. They change your biology. When you face chronic stress from poverty, discrimination, unsafe housing, or job instability, your body’s stress response systems stay activated far longer than they should. Over time, this creates what researchers call allostatic load: the cumulative physiological wear and tear from prolonged stress.

Here’s what that looks like inside the body. Chronic stress keeps cortisol and adrenaline elevated, which directly raises blood pressure by constricting blood vessels. Over months and years, this sustained activation damages cardiovascular, metabolic, and immune pathways. It increases inflammation, raises cholesterol, disrupts blood sugar regulation, and promotes clotting. These aren’t abstract risks. They’re the biological mechanisms behind heart attacks, strokes, and diabetes.

The effects are not distributed equally. Research published in PLOS ONE found that Black participants carried higher markers of physiological stress than White participants, even after accounting for behavioral factors like smoking. Black nonsmokers actually had the highest overall allostatic load scores in the study, suggesting that the chronic stress of racial discrimination and structural disadvantage creates measurable biological damage independent of personal health behaviors. This finding is critical: it shows that social conditions literally reshape the body’s stress chemistry.

The Economic Cost of Ignoring These Factors

Health disparities driven by social determinants carry enormous financial consequences. An NIH-funded study found that racial and ethnic health disparities cost the U.S. economy $451 billion in 2018, a 41 percent increase from the $320 billion estimated in 2014. Education-related health disparities were even more expensive. The total burden for people without a college degree reached $978 billion in 2018, roughly double the annual growth rate of the U.S. economy that year.

These costs show up as higher emergency room usage, more hospitalizations, greater disability, and lost productivity. When people can’t manage chronic conditions because they lack stable housing, reliable transportation, or affordable food, the health system absorbs that cost in the most expensive way possible: through crisis care rather than prevention.

How Health Care Is Starting to Respond

The medical system has historically treated patients as if their health begins and ends in the exam room. That’s changing. Health care providers now have a formal coding system to document social determinants in patient records, covering problems related to housing, food insecurity, transportation, employment, education, literacy, and social environment. These codes allow clinics and hospitals to track which patients face non-medical barriers and connect them with resources like food assistance, housing programs, or transportation services.

Screening for social needs is becoming routine in many health systems. During a visit, you might be asked whether you’ve had trouble paying for food, whether you feel safe at home, or whether you’ve had difficulty getting to appointments. These questions aren’t idle curiosity. They help providers understand why a treatment plan might not be working and what support could make a difference. A prescription for a healthier diet means very little if your nearest grocery store is a 45-minute bus ride away.

This shift reflects a broader recognition that investing in housing, education, food access, and economic opportunity is health care, even if it doesn’t happen in a hospital. The 80 to 90 percent of health outcomes shaped outside the clinic won’t improve by building more clinics. They improve when the conditions of daily life change.
