In a study of 430 breastfed infants, higher concentrations of leptin in breast milk were linked to leaner body composition at both four months and one year of age. Leptin is the hormone that signals fullness, and exposure to it through milk may help calibrate a baby’s appetite regulation from the very start. Insulin in breast milk showed a more nuanced pattern, with moderate concentrations predicting the healthiest weight outcomes. These hormonal signals are one reason researchers believe breastfeeding is associated with lower rates of childhood obesity, though the relationship involves many overlapping factors.
Cognitive Benefits
Breastfed infants tend to score higher on cognitive assessments than formula-fed infants, a pattern that appears consistently across studies. A 2024 systematic review in Frontiers in Public Health confirmed that breastfed preterm infants showed superior long-term cognitive scores compared to those who were never breastfed, with the clearest differences appearing in assessments before 18 months.
The reasons likely involve both nutrition and behavior. Breast milk contains long-chain fatty acids that support brain development, and breastfeeding itself involves close physical contact, skin-to-skin warmth, and responsive interaction that may independently support early neural wiring. Separating the nutritional effect from the bonding effect is difficult, but the overall pattern favoring breastfed infants is well established.
Protection Against Ear Infections
Middle ear infections are one of the most common reasons parents bring babies to the doctor, and how a baby feeds appears to matter. Interestingly, the physical act of nursing at the breast seems to be the key factor, not just the milk itself. Research published in The Journal of Pediatrics found that babies fed expressed breast milk from a bottle actually had higher odds of ear infections than babies who nursed directly. The mechanics of breastfeeding, which involve different swallowing patterns and less air intake than bottle feeding, likely help keep fluid from pooling near the ear canal.
Health Benefits for the Parent
The advantages aren’t one-sided. Breastfeeding changes the nursing parent’s hormonal environment in ways that lower long-term disease risk. Among women carrying BRCA1 or BRCA2 gene mutations, a history of breastfeeding is associated with a 23% reduction in ovarian cancer risk. The protective effect increased with breastfeeding duration up to about seven months, then leveled off. For women with BRCA2 mutations specifically, the reduction reached 38%.
When breastfeeding was combined with a history of oral contraceptive use, the ovarian cancer risk dropped by 53%. Breastfeeding also helps the uterus contract back to its pre-pregnancy size more quickly, reduces postpartum bleeding, and delays the return of menstruation, which conserves iron stores.
Breast Milk Adapts, Formula Cannot

Its composition shifts throughout a single feeding, starting watery to quench thirst and becoming fattier toward the end to satisfy hunger. It changes across the day, with evening milk containing more sleep-promoting compounds. It changes across months, adjusting calorie density and immune factors as the baby grows. And it responds to signals from the baby’s own saliva during nursing, ramping up antibody production when the baby is fighting an illness.
Formula provides consistent, standardized nutrition, which is genuinely its strength for parents who need a reliable alternative. But consistency also means it cannot respond to a baby’s individual biology the way breast milk does. Every can of formula delivers the same product regardless of the baby’s age, health status, or time of day.
What Formula Does Well
None of this means formula is harmful. Modern infant formula is carefully engineered to meet all of a baby’s basic nutritional requirements, and millions of babies thrive on it. Formula is fortified with iron, vitamin D, and other nutrients that breast milk provides in lower amounts. For parents who cannot breastfeed or choose not to, formula is a safe and complete source of infant nutrition.
The differences between breast milk and formula are real and well documented, but they exist on a spectrum. Many of the protective effects of breastfeeding are dose-dependent, meaning even a few weeks of breastfeeding provides some benefit compared to none.
