Feeling your heart beating is almost always normal. Your heart sits just behind your breastbone with its muscular lower tip angled slightly to the left, close enough to your chest wall that each contraction can produce a noticeable thump. Most people become aware of this sensation during quiet moments, certain body positions, or after consuming stimulants like caffeine. While the feeling can be unsettling, it rarely signals a heart problem.
How Your Heartbeat Reaches Your Chest Wall
Each heartbeat is a complex sequence of muscle contractions. The left ventricle, which is the heart’s main pumping chamber, generates the most force because it pushes oxygenated blood out to your entire body. When this chamber contracts, it creates a rapid burst of motion that accelerates the surrounding chest wall. That motion is what you feel as a “thump” or pulse in your chest.
The chest wall isn’t rigid. Your rib cage has some natural flexibility, and the soft tissue between your ribs transmits vibrations from the heart outward. During the strongest phase of each beat, when the heart’s valves open and blood rushes into the aorta, the rate of chest wall movement spikes dramatically. Even between beats, as the ventricles relax and refill with blood, there are smaller shifts in chest wall motion. Why Lying on Your Left Side Makes It Worse
If you notice your heartbeat most at night, you’re not imagining it. The tip of the heart, called the apex, is made up largely of the left ventricle and naturally sits slightly left of center in your chest. When you lie on your left side, gravity pulls the apex even closer to the chest wall. At the same time, your body weight pressing against the mattress slightly compresses the rib cage, reducing the distance between your heart and the surface of your chest even further.
The result is a stronger, more noticeable pulse. Add the stillness and quiet of lying in bed, where there’s nothing else competing for your attention, and a perfectly normal heartbeat can feel surprisingly loud. Rolling onto your right side or your back usually reduces the sensation because it shifts the apex away from the chest wall.

A number of everyday factors can make your heartbeat feel stronger or faster without anything being wrong. The most common include:

You notice the faster or harder beating, which makes you more anxious, which keeps the cycle going.
Some people feel palpitations even when their heart rhythm is completely normal. Your brain essentially turns up the volume on sensations it would normally filter out. The heartbeat was always there; your nervous system is just paying closer attention to it. The sensation is distinctive: a brief pause followed by one beat that feels noticeably stronger than usual. That forceful beat isn’t actually abnormal in strength. Most people who have these occasional extra beats experience no symptoms at all, and many never know they’re happening. Other symptoms like unexplained weight loss, hand tremors, and heat intolerance usually accompany the palpitations.
Anemia, where your blood carries less oxygen than normal, forces the heart to beat faster to compensate. You might also feel fatigued, short of breath, or lightheaded. Low or high levels of electrolytes like potassium and magnesium can affect the electrical signals that control heart rhythm. And certain sustained abnormal rhythms, like atrial fibrillation, produce a chaotic fluttering sensation that feels distinctly different from a normal strong heartbeat.
What Testing Looks Like
If your symptoms are frequent or bothersome enough to bring up with a doctor, the initial workup is straightforward: a physical exam, a 12-lead electrocardiogram (a quick, painless recording of your heart’s electrical activity), and basic blood tests to check thyroid function and electrolyte levels. These three steps identify or rule out the most common causes.
If the palpitations are infrequent and don’t show up during the office visit, you may wear a portable heart monitor for one to several days to catch the rhythm during an episode. Smartwatches with ECG features have become increasingly useful as a first step. They show high sensitivity for detecting atrial fibrillation, and their readings perform comparably to medical-grade devices for that specific rhythm. However, there’s still a significant gap between consumer wearables and clinical monitors when it comes to detecting more complex arrhythmias, so a normal smartwatch reading doesn’t replace formal monitoring if your doctor suspects something specific.
Signs That Need Immediate Attention
The vast majority of people who feel their heartbeat have nothing dangerous going on. But certain combinations of symptoms warrant urgent evaluation. If you experience palpitations along with severe shortness of breath, chest pain, or fainting, that combination needs emergency attention. Palpitations that come with lightheadedness so intense you feel like you might pass out also fall into this category. A heart rate that stays above 150 beats per minute at rest, or a new irregular rhythm that doesn’t resolve within a few minutes, is worth getting checked promptly rather than waiting for a routine appointment.
