You actually can use Voltaren gel on your shoulder, but the label says not to because the product was never tested there. The FDA-approved labeling for Voltaren Gel (diclofenac sodium 1%) explicitly states it “has not been evaluated for use on the spine, hip, or shoulder.” That’s not a safety ban. It’s a gap in the clinical evidence, and understanding the difference matters if you’re dealing with shoulder pain.
What the Label Actually Says
The OTC version of Voltaren Gel is approved for osteoarthritis pain in joints “amenable to topical treatment, such as the knees and those of the hands.” The approved body areas are specific: a hand, a wrist, an elbow, a foot, an ankle, or a knee. You can apply it to up to two of those areas at once. The shoulder, hip, and spine are explicitly excluded from this list.
The consumer label on the packaging puts it plainly: “Can I use Diclofenac Sodium Topical Gel on my spine, hips, or shoulders? No.” But the reason given isn’t a known danger. It’s that “the use of Diclofenac Sodium Topical Gel on the spine, hips, and shoulders has not been studied.” The manufacturer submitted clinical trial data for knees and hands when seeking FDA approval, and those are the only joints the product can legally claim to treat.
Why the Shoulder Was Left Out
Getting a drug approved for a specific body part requires running dedicated clinical trials on patients with pain in that exact location. These trials cost millions and take years. When the manufacturer sought OTC approval, it submitted data on the most common osteoarthritis sites: knees and hands. The shoulder simply wasn’t included in those trials, so the FDA couldn’t authorize it for shoulder use.
There’s also a legitimate anatomical question. The shoulder joint sits deeper beneath muscle, tendon, and soft tissue than the knee or finger joints. For a topical gel to work, the active ingredient needs to travel through the skin and reach the joint structures underneath. Research on knees shows that diclofenac does penetrate into joint tissue and fluid after repeated application, remaining detectable more than 12 hours after the last dose. Interestingly, body mass didn’t affect how well the drug reached the knee joint, suggesting that tissue thickness alone isn’t a simple barrier. But no equivalent study has confirmed the same penetration for the shoulder, where the joint capsule is surrounded by the rotator cuff and a thicker layer of muscle.
Does It Work on the Shoulder Anyway?
Some clinical evidence suggests it can help. A randomized, placebo-controlled trial of 155 patients with shoulder periarthritis (inflammation around the shoulder joint) tested a diclofenac gel applied three times daily for ten days. Patients using the diclofenac gel reported significantly less pain than the placebo group by day three, and that difference held through day six. By day ten, pain scores were still lower in the diclofenac group, though the gap had narrowed enough that it was no longer statistically significant. Functional improvements measured by a disability questionnaire also favored the diclofenac group, and no skin irritation or adverse events were reported.
So the gel appears to provide some short-term pain relief for shoulder conditions, even if the effect may be more modest than what you’d see at a superficial joint like the knee. The lack of FDA labeling for the shoulder doesn’t mean the product is useless there. It means the evidence base is thinner, and the manufacturer can’t put it on the box.
Systemic Absorption Isn’t the Concern
Some people assume the shoulder restriction is about safety, specifically the worry that applying gel to a larger area could flood your bloodstream with the drug. The data doesn’t support that fear. In a crossover study of 40 healthy volunteers, applying the gel to both knees and both hands (covering about 1,200 square centimeters of skin) still produced blood levels five to 17 times lower than taking oral diclofenac tablets. Even at the highest topical dose tested, systemic effects were far less pronounced than with the pill form.
The shoulder wouldn’t dramatically change this picture. The application area for one shoulder is comparable to one knee. The restriction isn’t driven by absorption risk.
What Your Doctor Might Say
Many physicians prescribe or recommend topical diclofenac for shoulder pain off-label, meaning for a use not specifically listed on the FDA packaging. Off-label use is common in medicine and is perfectly legal. The prescription version of diclofenac gel contains the same 1% concentration as the OTC product. Your doctor can make a judgment call based on your specific condition, whether that’s rotator cuff tendinopathy, shoulder bursitis, or osteoarthritis of the shoulder joint.
For deeper shoulder problems, though, a topical gel may simply not deliver enough medication to the right spot. Conditions involving the rotator cuff or the joint capsule itself may respond better to oral anti-inflammatory medications, physical therapy, corticosteroid injections guided by ultrasound, or a combination of these. Ice in the first 48 hours after a flare-up, followed by heat, also remains a standard first-line approach for shoulder bursitis and similar conditions.
The Bottom Line on the Label
The Voltaren gel label doesn’t say “don’t use on the shoulder because it’s dangerous.” It says “we didn’t test it there.” That’s a regulatory distinction, not a medical one. The gel likely provides some pain relief for superficial shoulder inflammation, though it may not penetrate deeply enough to treat problems buried within the joint.
