Why Do Babies Get the Hepatitis B Vaccine?

Babies receive the hepatitis B vaccine at birth because infants are uniquely vulnerable to the virus. When an adult catches hepatitis B, fewer than 5% develop a chronic, lifelong infection. When an infant is infected, that number jumps to about 95%. This enormous difference in risk is the core reason the first dose is given within 24 hours of birth, before a baby has any chance of exposure.

Why Infants Face Higher Risk Than Adults

Hepatitis B is a virus that attacks the liver. In adults, the immune system is mature enough to fight off the infection in most cases. An infant’s immune system can’t do this effectively. Instead of clearing the virus, a baby’s body essentially tolerates it, allowing it to settle into the liver permanently. Chronic hepatitis B can lead to liver scarring, liver failure, and liver cancer later in life. Vaccinating at birth gives a newborn’s immune system the instructions it needs to recognize and fight the virus before any exposure occurs.

How Babies Can Be Exposed

The most direct route is during birth. A mother with hepatitis B (sometimes without knowing she has it) can pass the virus to her baby during delivery. When the first vaccine dose is given within 24 hours of birth and the full series is completed, it is 70% to 95% effective at preventing this mother-to-infant transmission.

But birth isn’t the only risk. Babies can also contract hepatitis B through contact with blood or bodily fluids from any infected person in the household. A grandparent, sibling, or caregiver with an undiagnosed infection could transmit the virus through everyday contact with cuts, shared personal items, or open sores. Because hepatitis B often has no symptoms, many carriers don’t know they’re infected. This makes it impossible to fully predict which babies are at risk and which aren’t.

Why Every Baby Gets the Vaccine

Pregnant women are screened for hepatitis B, and you might wonder why that screening alone isn’t enough to protect babies. The answer comes down to gaps in the system. Lab results can be wrong. Some mothers aren’t tested. Others acquire the infection late in pregnancy, after their screening. And household contacts may carry the virus without ever being tested at all.

For these reasons, the CDC has recommended since 1991 that all medically stable infants receive the vaccine at birth, regardless of the mother’s infection status. This universal approach works as a safety net: it protects babies whose mothers test positive, babies whose mothers were missed or misidentified, and babies who will encounter the virus from someone else in their home or community. The strategy has been remarkably effective. Since universal infant vaccination began, pediatric hepatitis B infections in the United States have dropped by 99%.

What the Vaccine Contains

The hepatitis B vaccine does not contain any live virus. It’s made using a single protein found on the surface of the hepatitis B virus. That protein is produced by yeast cells in a lab through genetic engineering, then purified and included in the vaccine. When injected, this protein teaches the immune system to recognize hepatitis B without any possibility of causing the infection itself. It is one of the simplest vaccines given in childhood.

The Vaccination Schedule

The standard series involves three doses. The first is given within 24 hours of birth. The second comes at one to two months of age, and the third between six and eighteen months. Babies born weighing less than about 4.4 pounds (2,000 grams) to an uninfected mother follow a slightly different timeline: their first dose is given at one month of age or at hospital discharge, whichever comes first. Very small babies born to mothers who are infected or whose status is unknown receive the vaccine plus an additional protective antibody treatment within 12 hours of birth, followed by a total of four doses to ensure full protection.

Side Effects in Newborns

The vaccine has been given to millions of newborns over more than three decades, and side effects are mild and uncommon. In clinical data, fewer than 10% of newborns experienced pain or soreness at the injection site within four days. Redness at the site occurred in 8% to 20% of newborns. Swelling was reported in up to 4%, and fever in up to about 6%. Serious reactions are extremely rare. The side effect profile in newborns is consistent with what’s seen in older children and adults receiving the same vaccine.

How Long Protection Lasts

The three-dose series completed in infancy provides long-lasting immunity. Studies have shown that even when measurable antibody levels decline over time, the immune system retains a memory of the virus and can mount a rapid response if exposed years or decades later. Booster doses are not routinely recommended for people who completed the full series as children and have healthy immune systems. For most people, the protection from childhood vaccination lasts well into adulthood.
