Why Do Breastfed Babies Need Vitamin D?

Breast milk contains very little vitamin D, typically only 0.5 to 1.8 micrograms per liter. That translates to roughly 20 to 72 IU per day for a baby drinking a normal volume of milk, while infants need 400 IU daily. Even the most well-nourished breastfeeding mother produces milk that covers only a fraction of her baby’s requirement, which is why supplementation is recommended starting in the first few days of life.

Why Breast Milk Falls Short

Vitamin D is unusual among nutrients because the body is designed to get most of it from sunlight, not food. When ultraviolet rays hit the skin, they trigger the production of vitamin D in the upper skin layers. Historically, this worked fine for infants who spent time outdoors. But modern life has changed the equation: babies are kept indoors, dressed in clothing, and protected with sunscreen or shade, all of which block the UV exposure needed to make vitamin D naturally.

Breast milk evolved in a context where sun exposure filled the gap. The mammary gland simply doesn’t transfer large amounts of vitamin D into milk, regardless of the mother’s diet. This isn’t a flaw in breast milk. It reflects a biological system that assumed the baby’s skin would do the heavy lifting. Since pediatric guidelines now recommend keeping infants out of direct sunlight (because of skin cancer risk), the original source is effectively off the table, and the gap has to be closed with drops.

What Happens Without Enough Vitamin D

Vitamin D’s main job in infancy is helping the body absorb calcium and phosphorus to build bone. Without it, phosphorus levels drop, and the growth plates at the ends of a baby’s bones can’t mineralize properly. Cells that should die off and be replaced by hard bone instead pile up, producing soft, poorly formed skeletal tissue. Over time, this leads to rickets, a condition marked by bowed legs, bent bones, joint deformities, and bone pain.

Rickets is the extreme end of deficiency. Before bones visibly bend, a baby with low vitamin D may simply have weak or sore muscles, be unusually irritable, or grow more slowly than expected. These early signs are easy to miss, which is one reason supplementation is recommended as a universal precaution rather than something reserved for babies who show symptoms.

Some Babies Are at Higher Risk

Not all breastfed babies face the same level of risk. Several factors make vitamin D deficiency more likely:

  • Darker skin pigmentation. Melanin acts as a natural shield against UV radiation. Babies with darker skin need significantly more sun exposure to produce the same amount of vitamin D as lighter-skinned babies. Since most infants get minimal sun regardless, darker-skinned babies start at a steeper disadvantage.
  • Northern latitudes. Families living farther from the equator get less intense UV light, especially in winter months. In many northern cities, the sun’s angle is too low from roughly October through March to trigger meaningful vitamin D production in the skin.
  • Limited outdoor time. Babies born in winter, those living in urban apartments with little outdoor access, or those consistently covered in clothing are all at greater risk.
  • Maternal deficiency. A baby’s vitamin D stores at birth reflect the mother’s status during pregnancy. Mothers who were deficient themselves give birth to babies who already start low.

Immigrant families who have moved from sunnier climates to northern countries face a compounded risk. Research on first-generation immigrants shows that the combination of darker skin pigmentation, lower UV exposure at the new latitude, and dietary changes after relocation leads to persistently low vitamin D levels in both parents and children.

Why Formula-Fed Babies Don’t Need Drops

Infant formula is fortified with vitamin D during manufacturing. A baby drinking 32 ounces or more of formula per day gets the full 400 IU without any additional supplementation. This is the key difference: the vitamin D is added artificially to formula because it’s known to be lacking in milk-based nutrition.

Babies who get a mix of breast milk and formula fall into a gray area. Unless the baby is consistently drinking at least 32 ounces of formula daily, the CDC recommends supplementing with 400 IU of vitamin D drops just as you would for a fully breastfed infant.

How to Supplement

The standard recommendation from the American Academy of Pediatrics is 400 IU of vitamin D per day, beginning in the first few days after birth. Liquid vitamin D drops designed for infants are widely available over the counter. Most products deliver the full 400 IU in a single drop, which can be placed directly on the nipple before a feeding, dropped into the baby’s mouth, or mixed into a small amount of expressed milk.

Supplementation should continue for as long as the baby is breastfed and not getting 32 ounces of fortified formula daily. Once a child transitions to solid foods and fortified whole milk (after age one), they can typically get enough vitamin D from their diet, though the daily requirement stays at 400 IU through the first year and rises to 600 IU for children over twelve months.

Can Mothers Take Vitamin D Instead?

Some parents prefer supplementing the mother rather than giving drops directly to the baby. Clinical trials have tested this approach, and it works, but only at doses much higher than most people expect. Randomized trials found that a breastfeeding mother needs roughly 6,400 IU per day to raise her milk’s vitamin D content enough to meet her infant’s needs. Doses as high as 2,400 IU per day were not enough to adequately supply the baby.

The Academy of Breastfeeding Medicine recognizes maternal supplementation at 6,400 IU daily as a valid alternative when there’s an objection or barrier to giving drops directly to the infant. At that dose, both the mother and baby achieved healthy vitamin D levels in studies lasting up to seven months. If you’re considering this route, it’s worth noting that the margin for error is smaller: missing a day of your own supplement leaves the baby with virtually no vitamin D intake, whereas infant drops provide a direct, reliable dose independent of the mother’s habits.
