Why Do Crowns Fall Out? Causes and Prevention

Dental crowns fall out because the bond between the crown and the underlying tooth weakens over time. This can happen gradually through cement breakdown, tooth decay, or grinding, or it can happen suddenly when you bite into something sticky or hard. About 1 in 5 crowns fail within 10 years, and roughly 30% don’t make it to the 15-year mark, so a lost crown is a common dental problem with well-understood causes.

Decay Under the Crown

The most common biological reason a crown comes loose is decay forming underneath it. Bacteria can work their way past the edges of the crown, especially if the cement seal has any gaps or has started to wear down. Once decay takes hold in the remaining tooth structure, the shape of the tooth changes just enough that the crown no longer fits snugly. At that point, the crown may rock slightly when you chew, which accelerates the loosening until it falls off entirely.

This type of hidden decay is particularly frustrating because you often can’t see it or feel it until the crown is already loose. Regular dental X-rays are the main way to catch it early, since the decay sits beneath the crown’s edges where your toothbrush can’t reach.

Cement Breakdown Over Time

The cement holding your crown in place doesn’t last forever. It slowly degrades from constant exposure to moisture, temperature changes, and acids in your mouth. Lactic acid, which makes up about 70% of the acids produced by plaque bacteria, is particularly destructive. When you eat sugary or starchy foods, bacteria produce a surge of this acid, and the pH in your mouth can drop as low as 4, well into the range that breaks down dental cement through a chemical process called hydrolysis.

Once the cement starts to soften, it absorbs more fluid, which weakens it further in a self-reinforcing cycle. Areas near the gumline are hit hardest because the concentration of bacterial acids is higher there. This is why crowns sometimes feel slightly loose for weeks before they actually come off. The cement is gradually dissolving from the inside out.

Not Enough Tooth to Hold On To

A crown needs a certain amount of healthy tooth structure underneath it to stay put. For back teeth, the prepared tooth ideally needs to be at least 4 millimeters tall, and for front teeth, at least 3 millimeters. The walls of the tooth also need to have the right angle of taper, ideally between 10 and 20 degrees. When the taper is too wide (too much of a cone shape), the crown has less grip and is more likely to slide off.

Sometimes there simply isn’t enough tooth left after removing decay or after a fracture. A dentist may do their best with what’s available, but a short, heavily tapered tooth is fighting physics from day one. This is one reason some crowns fail much sooner than expected, particularly on teeth that had extensive damage before the crown was placed.

Grinding and Clenching

Bruxism, the habit of grinding or clenching your teeth, puts enormous repetitive stress on crowns. The forces involved are far greater than normal chewing and can crack the cement seal, chip the crown material, or gradually rock the crown loose. Even zirconia, one of the strongest crown materials available, is vulnerable to fatigue fractures when subjected to prolonged grinding forces.

If you grind your teeth at night (and many people do without realizing it), an occlusal guard is one of the most effective ways to protect your crowns. These guards reduce the frequency of crown loosening and marginal breakdown by absorbing the forces that would otherwise go directly into the restoration.

Sticky and Hard Foods

Caramel, taffy, and other sticky foods can physically pull a crown off the tooth, especially if the cement has already weakened. This is often the final straw rather than the root cause. A crown with a perfect cement seal and solid tooth structure underneath is unlikely to come off from a piece of candy alone, but one that’s been slowly loosening for months might pop off the next time you chew something chewy or bite down on a hard nut.

How Long Crowns Typically Last

A large retrospective study tracking single crowns over two decades found cumulative survival rates of about 90% at 5 years, 81% at 10 years, 71% at 15 years, and 62% at 20 years. Crown material matters somewhat: metal-ceramic crowns tend to perform slightly better on back teeth than all-ceramic options (about 96% versus 93% at five years), though for front teeth the difference largely disappears.

These numbers reflect all types of failure, not just crowns falling off. Fractures, root canal problems, and decay all count. But they give you a realistic picture of how long you can expect a crown to serve before it needs attention.

What to Do When a Crown Falls Off

If your crown comes off, find it and rinse it gently with water. Rinse your mouth with warm salt water to clear away bacteria and debris. Take a look at both the crown and the exposed tooth. If the crown is intact and the tooth underneath doesn’t have sharp or jagged edges, you can temporarily reseat the crown using a small dab of toothpaste, dental wax, or sugar-free gum to hold it in place until you can get to a dentist.

Don’t use superglue or any household adhesive. These can damage the tooth or make it harder for your dentist to properly recement the crown. Store the crown in a clean container if you can’t reseat it, and avoid chewing on that side. The exposed tooth may be sensitive to temperature and pressure, which is normal since the protective layer is gone.

Keeping a Crown in Place Longer

Since cement breakdown and decay are the two biggest threats, your daily habits make a real difference. Brush twice a day with a soft-bristled toothbrush and non-abrasive toothpaste to avoid wearing down the crown’s edges. Floss daily, but use a specific technique around crowned teeth: slide the floss out gently to the side rather than snapping it upward, which can tug at the crown margins and weaken the seal over time.

Limiting sugary and acidic foods helps slow cement degradation by keeping oral pH higher. If you know you grind your teeth, wearing a night guard protects both the crown and the cement holding it. And regular dental visits with X-rays let your dentist spot early decay or cement gaps before the crown actually comes loose, when a simple recement can solve the problem rather than a full replacement.
