In institutionalized settings, rates of sundowning exceed 80% of dementia patients, making it one of the most widespread behavioral symptoms of the disease. Understanding why it happens can help you respond to it more effectively.
The Brain’s Internal Clock Breaks Down
Your brain has a tiny cluster of cells that acts as a master clock, keeping your sleep-wake cycle, body temperature, hormone levels, and alertness synchronized with the 24-hour day. In Alzheimer’s and other dementias, this clock region degenerates as the disease progresses. The damage doesn’t just disrupt sleep. It throws off the entire daily rhythm of motor activity, body temperature, and hormone secretion.
One key hormone affected is melatonin, which normally rises at night to promote sleep and drops during the day. In people with Alzheimer’s, melatonin levels are abnormally high during daytime hours, and the normal nighttime spike becomes blunted or absent. Bright light, which would normally suppress daytime melatonin in a healthy person, has little effect on Alzheimer’s patients. The result is a brain that can no longer distinguish day from night in a biochemical sense, leaving the person in a disorienting twilight state as evening approaches.
There’s also a neural pathway connecting the brain’s clock region to an area that regulates aggression. When the clock functions normally, it acts as a gate, keeping aggressive impulses in check at appropriate times. When the clock deteriorates, that gate stops working properly, which helps explain why agitation and even combative behavior tend to surge in the late afternoon and evening.
Fading Light Distorts What They See
Dementia doesn’t just affect memory. It damages the brain’s ability to interpret visual information, a condition called visual agnosia. The eyes still work, but the brain can’t make sense of what they’re seeing. Some patients lose the ability to recognize where they are, even in a familiar room. Others see only scattered objects without understanding how they relate to each other, as if looking at puzzle pieces instead of a complete picture.
Now add low light to that equation. As evening arrives and rooms grow dim, shadows stretch across walls and furniture. A healthy brain easily identifies a coat on a chair as exactly that. A brain with visual processing damage may interpret the same shadow as an intruder. Tasks that were once automatic, like following a conversation, navigating a hallway, or eating a meal, now require intense concentration. By late afternoon, the brain is running on empty. The cognitive resources that helped them stay oriented, manage anxiety, and interpret their surroundings during the morning are simply used up.
This fatigue compounds every other factor. A person who could recognize their living room at 10 a.m. may not be able to at 6 p.m., not because the disease suddenly worsened but because they’ve exhausted their remaining capacity to process information. The Boundary Between Sleep and Waking Blurs
Healthy brains draw a sharp line between being asleep and being awake. In dementia, that line erodes. Patients may have trouble separating reality from dreams, a factor the Mayo Clinic identifies as a direct contributor to nighttime fear. How to Reduce Nighttime Fear
Keep the Environment Well Lit
Since so much nighttime fear stems from visual misinterpretation, lighting is one of the most effective tools available. Keep rooms well lit in the evening, and use nightlights in hallways and bathrooms to prevent the deep shadows that trigger confusion. For a more structured approach, bright light therapy using a 10,000-lux light box for 30 minutes in the morning has been shown to help stabilize the circadian rhythm in dementia patients when used consistently at least five days a week. If morning sessions alone don’t help after one to two weeks, adding an afternoon or evening session can improve results. The light box should be placed at or above eye level, about one to three feet away, and the person doesn’t need to stare directly at it.
Wind Down the Evening
Reduce stimulation as the day progresses. Television, loud music, and household chores in the evening can add to confusion. Instead, shift toward calm, familiar activities. A predictable evening routine helps the brain anticipate what comes next, reducing the anxiety that comes from constant uncertainty. Make sure the person isn’t hungry, thirsty, or in pain, since any of these can amplify agitation without the person being able to articulate what’s wrong.
Respond With Calm Reassurance
When fear does set in, what you say matters less than how you say it. Speak calmly and slowly. Don’t argue with what the person believes they’re seeing or experiencing, because to their brain, the threat is real. Instead, reassure them that they are safe and that you are there to help. Gentle touch, like holding a hand or a light touch on the shoulder, can be more effective than words. If verbal reassurance isn’t working, try redirecting their attention to a simple activity: folding a towel, having a warm drink, or listening to familiar music.
Watch your own body language carefully. If you’re visibly stressed or frustrated, the person will pick up on that tension even when they can’t understand your words, and it will escalate their fear. Taking a few deep breaths before engaging can make a real difference in the outcome.
When Agitation Becomes Severe
In cases where nighttime agitation is severe, dangerous, or causes significant distress, doctors may consider antipsychotic medications. These are prescribed at much lower doses for older adults with dementia, typically one-third to one-half the standard adult dose, and are intended as a last resort rather than a first-line approach. The goal is always to use the lowest amount that provides relief, because these medications carry meaningful risks for older adults with dementia. Non-drug strategies should always be tried thoroughly first.
Underlying Medical Causes to Rule Out
A sudden worsening of nighttime fear or confusion often signals something beyond the dementia itself. Urinary tract infections, constipation, undiagnosed pain, and medication side effects can all dramatically increase agitation, and the person may not be able to tell you something is wrong. If nighttime behavior changes noticeably over a short period, a medical evaluation can catch treatable problems that are making everything worse.
