Why Do Hemorrhoids Bleed? Causes and Common Triggers

Hemorrhoids bleed because the swollen tissue lining the anal canal contains dilated blood vessels sitting just beneath a thin, fragile layer of tissue that gets easily damaged during bowel movements. The bleeding is bright red because it comes from small arteries, not veins, and it’s usually painless because the tissue where internal hemorrhoids form has no pain-sensing nerves.

What Happens Inside the Tissue

Your anal canal naturally contains cushions of tissue rich with blood vessels. These cushions help with continence and normally stay tucked in place, supported by connective tissue. Hemorrhoidal disease starts when that supportive tissue breaks down and the blood vessels inside the cushions become abnormally dilated and distorted.

In healthy tissue, the vessels have a sphincter-like structure with thick muscular walls that regulate blood flow. In hemorrhoids, those walls become thin and flat, leaving wide-open, engorged vessels packed just below the surface. The tissue covering internal hemorrhoids is only about one centimeter thick, and it becomes inflamed, fragile, and easily eroded. When hard stool, loose stool, or simple straining scrapes across that surface during a bowel movement, tiny arteries in the tissue get nicked open.

This is why the blood is bright red rather than dark. Hemorrhoidal tissue has direct connections between arteries and veins, so the blood is oxygenated and freshly arterial. Depending on the size of the vessel that gets eroded, the bleeding can range from a small ooze (when very tiny vessels are involved) to a noticeable spurt (when a slightly larger artery is damaged). Most people notice it as drips into the toilet bowl or streaks on toilet paper.

Why It Doesn’t Usually Hurt

Internal hemorrhoids form above a boundary called the dentate line, inside the upper part of the anal canal. This area is covered by a type of tissue that lacks the nerve fibers responsible for sensing sharp pain. That’s why many people are alarmed by the bleeding but feel nothing when it happens. The combination of painless, bright red blood on the toilet paper or in the bowl is the classic signature of an internal hemorrhoid.

External hemorrhoids, which form lower in the anal canal under skin that is heavily innervated, are a different story. They don’t typically bleed on their own, but if a blood clot forms inside one (a thrombosed hemorrhoid) and the swollen tissue ruptures, the bleeding can be sudden and significant. That type of bleeding usually comes with noticeable pain and a firm, tender lump near the anus.

Common Triggers for Bleeding

Anything that increases pressure on hemorrhoidal tissue or scrapes against it can set off bleeding. The most common culprits:

  • Hard, dry stool. Constipated stool physically abrades the fragile surface of swollen hemorrhoids as it passes through the canal.
  • Straining during bowel movements. Bearing down increases blood pressure inside the already dilated vessels, engorging them further and making the tissue more vulnerable.
  • Diarrhea. Frequent loose stools can irritate and erode hemorrhoidal tissue just as effectively as hard stool.
  • Prolonged sitting on the toilet. Spending extra time reading or scrolling keeps pressure on the anal cushions longer than necessary.

Pregnancy, heavy lifting, and obesity all contribute for the same fundamental reason: sustained pressure on the pelvic floor pushes more blood into the hemorrhoidal vessels and weakens the connective tissue holding everything in place.

What the Blood Tells You

Bright red blood that appears on toilet paper, drips into the bowl, or coats the outside of stool is consistent with hemorrhoids. The blood is fresh and hasn’t traveled through the digestive tract, so it stays red.

Dark, maroon, or tarry black stool signals something different. When blood originates higher in the digestive system (the stomach or small intestine), digestive enzymes chemically alter it during transit, turning it dark brown or black. That type of bleeding requires prompt medical evaluation because it points to problems far from the anal canal.

Even bright red rectal bleeding deserves attention if it persists beyond two weeks, gets progressively worse, or keeps returning in short intervals. A lump near the anus that feels hard, irregular, or rough rather than soft and smooth could indicate something other than a hemorrhoid. Unexplained weight loss, changes in bowel habits, or swollen lymph nodes in the groin alongside rectal bleeding are all reasons to get checked.

How to Reduce Hemorrhoid Bleeding

Since bleeding happens because fragile tissue gets damaged during bowel movements, the goal is to make those bowel movements as gentle as possible. Fiber is the single most effective tool. The recommended daily intake is about 14 grams per 1,000 calories you eat, which works out to roughly 28 grams per day on a standard 2,000-calorie diet. Most people fall well short of that. Gradually increasing fiber through vegetables, fruits, whole grains, or a supplement softens stool, adds bulk, and reduces the straining that engorges hemorrhoidal vessels. Drinking plenty of water alongside the fiber keeps things moving smoothly.

Warm sitz baths help calm inflamed tissue. Sitting in about three inches of warm water (around 104°F or 40°C) for 15 to 20 minutes, three to four times a day, increases blood flow to the area and can reduce swelling. Over-the-counter creams and suppositories containing ingredients that shrink blood vessels or numb the area can provide short-term relief, though they don’t fix the underlying problem.

Avoiding long stretches on the toilet, going when you feel the urge rather than waiting, and never straining to force a bowel movement all reduce the mechanical forces that tear open those superficial vessels.

When Bleeding Keeps Coming Back

For internal hemorrhoids that bleed repeatedly despite dietary changes, rubber band ligation is one of the most common and effective office procedures. A small band is placed around the base of the hemorrhoid, cutting off its blood supply. The tissue shrinks, clots off, and falls away within five to seven days. The procedure also creates scar tissue underneath the surface that helps prevent new hemorrhoids from forming in the same spot.

In studies tracking long-term results, band ligation controlled rectal bleeding in 98% of patients. Recurrence rates were low: about 3.7% at one year and 13% at five years. For hemorrhoids that are too large for banding or have failed other treatments, surgical removal is an option with higher upfront recovery time but very low recurrence rates.
