That strange combination of dizziness and feeling like you’re living inside a dream is more common than you might think. Between 25% and 75% of people experience at least one episode of this detached, unreal sensation during their lifetime. The dreamlike feeling has a name: derealization. When it shows up alongside dizziness, it usually points to one of several identifiable causes, ranging from anxiety and sleep loss to problems with your inner ear’s balance system.
What Derealization Actually Feels Like
Derealization is the sensation that your surroundings aren’t quite real. The world may look flat, foggy, or distant, like you’re watching your own life through a pane of glass or from inside a bubble. Colors can seem muted. Familiar places feel strange. Some people describe it as being on autopilot, going through the motions while feeling mentally “somewhere else.”
The key feature is that you know something is off. Unlike psychosis, where a person loses touch with what’s real, derealization leaves your reality-testing intact. You’re aware the feeling is wrong, which is often what makes it so unsettling. When dizziness layers on top of this, the combination can feel genuinely frightening, like your body and your perception of the world are both malfunctioning at once.
How Your Balance System Creates the Dreamlike Feeling
Your inner ear does more than help you hear. It contains a balance system (the vestibular system) that constantly tells your brain where you are in space, whether you’re moving, tilting, or standing still. When that system sends unreliable signals, your brain struggles to build a stable picture of the world around you. Research published in the Journal of the Neurological Sciences found that spatial disorientation and anxiety together mediate the transformation of vestibular dysfunction into feelings of depersonalization and derealization.
In other words, a balance problem doesn’t just make you dizzy. It can literally make reality feel distorted. Your brain relies on consistent sensory information to construct “normal.” When that information conflicts, what you see and feel stops matching up, and the result is that dreamlike, disconnected quality. Studies have specifically linked this to dysfunction in the saccule, a tiny organ in the inner ear that senses vertical motion and gravity.
Persistent Postural-Perceptual Dizziness
If the dizziness and unreality have been lingering for weeks or months, a condition called persistent postural-perceptual dizziness (PPPD) may be involved. PPPD is diagnosed when dizziness, unsteadiness, or a non-spinning sense of movement occurs on most days for at least three months. Symptoms tend to worsen when you’re upright, when you move or are moved (like riding in a car), or when you’re looking at busy visual scenes like scrolling on your phone or walking through a crowded store.
People with PPPD frequently report feeling “spaced out,” as if their legs are spongy, or as if they’re floating. That mild dissociation is a recognized part of the condition, not a separate problem. PPPD often begins after an initial triggering event, such as an inner ear infection, a concussion, or a period of intense anxiety, and then persists even after the original trigger resolves. The brain essentially gets stuck in a heightened state of motion sensitivity.
Vestibular Migraine
Migraine doesn’t always mean a headache. Vestibular migraine causes episodes of vertigo or dizziness that can last anywhere from a few minutes to 72 hours, with about 30% of patients experiencing attacks that last minutes, 30% lasting hours, and another 30% lasting several days. Some people have brief episodes of only seconds that repeat with head movement or visual stimulation.
During a vestibular migraine episode, you might feel a false sensation of self-motion (like the room is spinning or you’re swaying), dizziness triggered by moving your head, or nausea with a disturbed sense of where you are in space. The dreamlike quality often accompanies these episodes because the same sensory confusion that causes dizziness also disrupts your normal sense of being grounded in reality. You may or may not have a headache at the same time, which is why many people don’t connect the symptoms to migraine at all.
Anxiety and Stress as Triggers
Anxiety is one of the most common causes of both dizziness and derealization occurring together, and it works through multiple pathways at once. Stress hormones affect blood flow, breathing patterns, and muscle tension in ways that produce lightheadedness. At the same time, severe or chronic anxiety can trigger dissociative symptoms as a kind of protective response, where the brain dials down your connection to your surroundings when it perceives overwhelming threat.
Derealization as a disorder is most often rooted in severe stress, particularly emotional abuse or neglect during childhood, witnessing domestic violence, or other major trauma. But everyday stressors can trigger episodes too: relationship conflict, financial pressure, work burnout, or depression. If you’ve noticed the dreamlike dizziness worsens during high-stress periods or panic attacks, this connection is likely playing a role.
Low Blood Sugar and Dehydration
Sometimes the cause is straightforward and physical. When blood sugar drops below about 70 mg/dL, the brain starts running low on its primary fuel. Early symptoms include dizziness and lightheadedness. As levels fall further, confusion sets in, along with difficulty completing routine tasks and unusual behavior. That cognitive fog can feel a lot like derealization, especially if you don’t recognize what’s happening.
Dehydration produces a similar overlap. Even mild dehydration reduces blood volume, which means less blood flow to the brain. The result is lightheadedness paired with a foggy, detached mental state. If your symptoms tend to show up in the afternoon, after skipping meals, during hot weather, or after exercise, these basic metabolic factors are worth addressing first. Eating regularly, staying hydrated, and noticing whether the feelings track with your meals can help you rule this in or out quickly.
Sleep Deprivation and Perception
Poor sleep directly impairs the vestibular system’s ability to function and recover. Animal research has shown that even moderate sleep deprivation, around six hours of disrupted sleep over five consecutive days, significantly worsens vestibular compensation. In practical terms, this means that if your balance system is already slightly off (from stress, a mild inner ear issue, or anything else), sleep loss makes it substantially harder for your brain to correct the problem.
Sleep deprivation also independently produces derealization. After a night or two of poor sleep, many people report that the world feels slightly “off,” flat, or dreamlike. This is because sleep is when the brain consolidates sensory processing and recalibrates its internal models of reality. Cutting that process short leaves you in a state where perception feels unreliable, and if dizziness is already present, the two symptoms amplify each other.
Sorting Out What’s Causing Your Symptoms
Because so many conditions produce this same combination, paying attention to the pattern of your symptoms can help narrow things down. Consider a few key questions:
- Timing: Do episodes last seconds, hours, or days? Brief episodes triggered by head movement point toward vestibular migraine or positional vertigo. Hours-long episodes with visual sensitivity suggest PPPD.
- Triggers: Do symptoms worsen in crowded or visually busy environments? That’s characteristic of PPPD. Worse with stress or panic? Anxiety-driven derealization. Worse when you skip meals or feel thirsty? Metabolic causes.
- Duration of the overall problem: If symptoms have persisted on most days for three months or more, PPPD criteria may be met. If it’s episodic and comes in distinct attacks, vestibular migraine is more likely.
- Sleep and stress context: A period of poor sleep, high stress, or both can explain new onset of these symptoms even without an underlying vestibular disorder.
Many people experience more than one of these factors at the same time. An inner ear problem creates dizziness, which triggers anxiety, which worsens derealization, which disrupts sleep, which impairs vestibular recovery. Breaking any part of that cycle, whether through better sleep, stress management, or treatment for the vestibular issue, often improves the whole picture.
