These sudden, painful cramps strike during sleep when your nervous system misfires, sending involuntary signals that lock a muscle into contraction. They’re extremely common, especially after age 50, and while they can feel alarming, they’re almost always harmless.

What’s Actually Happening in Your Muscles

A charley horse is an involuntary, sustained muscle contraction. During sleep, the leading explanation involves a glitch in your spinal reflexes. Your muscles have two built-in feedback systems: one that tells a muscle to contract (driven by sensors called muscle spindles) and one that tells it to relax (driven by sensors in your tendons). Normally, these two signals stay balanced. A nighttime cramp happens when the “contract” signal ramps up while the “relax” signal drops off, creating a runaway contraction with no braking mechanism.

This imbalance originates in the spinal cord, not in the muscle itself. The nerve cells controlling your muscle become overexcited, firing repeatedly and forcing the muscle into a tight, painful knot. Your calf muscle, for example, naturally shortens when your foot points downward under the weight of blankets. A muscle held in a shortened position for a long time is more susceptible to spontaneous contraction because the feedback loop between your tendons and spinal cord is less active. There’s simply less “relax” signaling happening when the muscle isn’t being stretched or loaded.

On top of that, the fatigue your muscles accumulated during the day doesn’t disappear when you go to bed. If you were on your feet a lot, exercised hard, or even sat in one position for an extended period, your muscles carry residual fatigue into the night. That fatigue appears to be one of the triggers that tips the excitatory/inhibitory balance in your spinal cord toward cramping.

The Electrolyte and Dehydration Question

This is where most advice you’ll find online gets it wrong. The idea that nighttime cramps come from low potassium, magnesium, or dehydration is deeply ingrained, but the clinical evidence doesn’t support it. That doesn’t mean electrolytes are irrelevant to muscle function in general. Severe deficiencies can cause muscle problems. Supplementing with potassium, calcium, or magnesium has no proven benefit for preventing these cramps. Drinking extra water before bed is similarly unsupported as a preventive strategy.

Risk Factors That Actually Matter

Age is the strongest predictor. The older you get, the more frequently nocturnal cramps tend to occur. This likely reflects age-related changes in nerve function, loss of muscle mass, and tendons that shorten over time.

Pregnancy is another major risk factor. Between 30% and 50% of pregnant women experience nighttime leg cramps, with the highest rates during the third trimester. The combination of weight gain, postural changes, and loosening of ligaments puts extra strain on leg muscles. Women who have been pregnant multiple times report even higher rates, with one study finding that over 92% of multigravida women experienced nocturnal cramps within a three-month period.

Prolonged standing or sitting during the day increases your risk. So does any sudden increase in physical activity your body isn’t used to. Stretching your calves before bed is one of the most consistently recommended strategies. A simple wall stretch, holding for 30 seconds per leg, can make a real difference if done nightly.
