Nighttime dizziness most often comes from a change in head position that disturbs your inner ear, a drop in blood pressure when you stand up, or a combination of factors like dehydration and medication side effects that hit harder after hours of lying down. The good news is that the most common cause, a condition called BPPV, is harmless and highly treatable.
Inner Ear Crystals Are the Most Common Culprit
Benign paroxysmal positional vertigo, or BPPV, is the single most frequent cause of nighttime dizziness. Inside your inner ear, tiny calcium crystals help you sense gravity and movement. Sometimes these crystals break loose and drift into the fluid-filled canals that detect head rotation. When you roll over in bed, lie down, or sit up, the loose crystals shift with gravity, pushing fluid through those canals and sending false signals to your brain that the room is spinning.
BPPV episodes are brief, usually lasting less than a minute, but they can be intense. The spinning typically starts a second or two after you move your head and fades once you hold still. It tends to affect one side more than the other, so you may notice it only when turning a certain direction. A healthcare provider can confirm the diagnosis with a simple head-positioning test and often resolve it in a single visit using a guided series of head movements that coax the crystals back where they belong.
Blood Pressure Drops When You Stand Up
If your dizziness hits the moment you get out of bed, rather than when you roll over, a blood pressure drop is a likely explanation. Normally, when you stand, pressure sensors near your heart and neck detect the shift and signal your heart to pump faster and your blood vessels to tighten. This keeps blood flowing to your brain. When that reflex is too slow or too weak, blood pools in your legs and abdomen, your brain briefly gets less oxygen, and you feel lightheaded or faint.
This is more common at night for a practical reason: you’ve been lying flat for hours, your heart rate is low, and your body may not snap into compensatory mode as quickly as it would during the day. Dehydration compounds the problem. You lose fluid through breathing and sweating while you sleep, and less fluid in your bloodstream means less blood traveling back to the heart. If you haven’t had water in several hours, even mild dehydration can be enough to trigger dizziness when you stand.
Older adults, people on blood pressure medications or water pills, and anyone taking cardiovascular drugs are especially vulnerable. Many heart and blood pressure medications work by relaxing blood vessels or reducing fluid volume, both of which make it harder for your body to compensate when you stand quickly in the dark.
Medications That Cause Nighttime Dizziness
A surprisingly long list of common medications can cause dizziness or vertigo as a side effect. Blood pressure drugs, pain medications like codeine, certain antibiotics, anti-seizure medications, acid reflux drugs, and sedatives including benzodiazepines all appear on that list. Even melatonin, which many people take specifically at bedtime, has been associated with dizziness.
Nighttime is when these effects tend to peak. Many people take their medications in the evening, so drug levels are highest during the hours you’re in bed. If you started a new medication and noticed dizziness soon after, that timing is worth mentioning to your prescriber. In some cases, adjusting the dose or the time of day you take it can solve the problem.
Low Blood Sugar During Sleep
People with diabetes, particularly those on insulin, can experience blood sugar drops below 70 mg/dL during the night. This is called nocturnal hypoglycemia, and dizziness is one of its hallmark symptoms. Other signs include waking up drenched in sweat, trembling, a racing heartbeat, restless sleep, or vivid nightmares that jolt you awake.
Even people without diabetes can experience mild blood sugar dips overnight, especially if they skipped dinner, drank alcohol in the evening, or exercised heavily late in the day. The dizziness in these cases tends to feel more like lightheadedness or unsteadiness rather than a spinning sensation.
Sleep Apnea and Morning Dizziness
If your dizziness is worst when you first wake up rather than in the middle of the night, sleep apnea deserves consideration. Sleep apnea causes repeated pauses in breathing, sometimes dozens of times per hour, leading to drops in blood oxygen levels throughout the night. This intermittent oxygen deprivation damages blood vessels over time, raises blood pressure, and disrupts blood flow to the brain.
Research published in Otology & Neurotology found that people with sleep apnea have a significantly elevated risk of conditions that cause vertigo, including migraine-associated dizziness and Ménière’s disease. Some patients in the study experienced brief recurring spells of vertigo throughout the day that resolved once their sleep apnea was treated. Morning headaches and daytime fatigue alongside dizziness are strong clues that breathing problems during sleep may be involved.
Anxiety and Hyperventilation
Nighttime anxiety and nocturnal panic attacks can produce dizziness that feels remarkably like a medical emergency. The mechanism is straightforward: anxiety triggers rapid, shallow breathing, which blows off too much carbon dioxide from your blood. Low carbon dioxide causes blood vessels in the brain to constrict, reducing blood flow. At the same time, the chemical shift in your blood makes it harder for oxygen to release from red blood cells into brain tissue. The result is lightheadedness, a floating sensation, or even near-fainting that can mimic vertigo.
Nocturnal panic attacks often wake you from sleep with a jolt, accompanied by a racing heart, sweating, and a sense of dread. The dizziness in these episodes fades as your breathing slows back to normal, which helps distinguish it from inner ear problems.
Nutritional Deficiencies
Vitamin B12 deficiency can cause dizziness that worsens in low-light conditions like a dark bedroom. B12 is essential for nerve function, and a long-standing deficiency damages the nerves responsible for balance and spatial awareness. You might notice unsteady movements, muscle weakness, or changes in thinking alongside the dizziness. The symptoms develop gradually over months, so they’re easy to miss until they become pronounced. A simple blood test can confirm or rule out this cause.
Reducing Nighttime Dizziness
The Vestibular Disorders Association recommends a specific routine for getting out of bed safely: sit on the edge of the bed for a few moments before standing, gently turn your head up, down, and side to side a couple of times to “wake up” your balance system, and make sure you can see clearly before taking your first step. This brief pause gives your blood pressure time to adjust and lets your brain recalibrate.
Lighting matters more than most people realize. In darkness, your brain relies more heavily on your inner ear and the position sensors in your muscles and joints for balance. If either of those systems is even slightly off, removing visual cues by stumbling through a dark room makes everything worse. Nightlights that turn on immediately (not the slow-warming kind) along the path from your bed to the bathroom can make a real difference.
Staying hydrated in the evening helps prevent the blood volume drops that cause lightheadedness. You don’t need to overdo it and interrupt your sleep with bathroom trips, but a small glass of water before bed is worthwhile if you tend to wake up dizzy. Limiting alcohol to two drinks or fewer also helps, since alcohol is both dehydrating and directly disruptive to the inner ear’s fluid balance.
For fall prevention at home, grab bars near the toilet and in the shower, low-pile carpeting or nonslip mats, and clear pathways free of clutter are practical changes. Wearing secure, nonslip footwear rather than going barefoot reduces risk, especially on hard floors.
Signs That Need Urgent Attention
Most nighttime dizziness is benign, but sudden dizziness paired with certain other symptoms can signal a stroke. The warning signs to watch for are sudden numbness or weakness on one side of the body, sudden difficulty speaking or understanding speech, sudden vision loss in one or both eyes, sudden loss of coordination, or a sudden severe headache unlike anything you’ve experienced before. If any of these appear alongside dizziness, call emergency services immediately. The CDC recommends the F.A.S.T. test: check whether the Face droops on one side, whether one Arm drifts downward, whether Speech is slurred, and if so, it’s Time to call 911.
