Heart palpitations are extremely common, and in most cases they’re harmless. When your heart seems to skip a beat, flutter, pound, or race, you’re feeling an abnormal movement of the heart inside your chest. A population-based study of healthy adults aged 25 to 41 found that 69% had at least one premature heartbeat during a single day of monitoring, with most people recording only a couple. In other words, the occasional skipped beat is closer to normal than abnormal.
That said, palpitations have many possible triggers, and understanding yours can help you decide whether it’s something to address or simply something to notice and move on from.
What Actually Happens During a “Skipped Beat”
Most palpitations come from premature beats, which are extra heartbeats that fire slightly early. Ironically, the sensation you notice isn’t usually the extra beat itself. The premature beat interrupts the heart’s normal timing, which allows the chambers to fill with more blood than usual before the next contraction. That next beat is stronger than normal, and that’s the thump or “skip” you feel. Your heart didn’t actually skip anything; it just paused briefly and then hit harder.
Under ordinary conditions, you don’t feel your heart beating at all. Palpitations become noticeable when there’s a change in rate, rhythm, or the force of contraction. That’s why you might notice them when you’re lying quietly in bed but not during a busy afternoon: your attention and the absence of competing sensations make the irregular beat easier to detect.
Stress, Anxiety, and Adrenaline
Stress is one of the most common palpitation triggers. When you’re anxious or under pressure, your body releases adrenaline and cortisol. Adrenaline directly speeds up the heart and makes it contract more forcefully. Cortisol, your longer-acting stress hormone, keeps the system revved up even after the initial surge passes.
Anxiety also shifts the balance of your nervous system. Your heart rate is controlled by two competing inputs: one that speeds it up (sympathetic) and one that slows it down (parasympathetic). In people with chronic anxiety or depression, the beat-to-beat variation in heart rate drops, a sign that the calming branch of the nervous system isn’t doing its job as effectively. This imbalance makes premature beats and racing sensations more likely, and it also makes you more aware of normal heartbeats that you’d otherwise ignore.
Caffeine, Alcohol, and Other Substances
Caffeine is a classic trigger, though sensitivity varies widely. Some people can drink several cups of coffee without issue; others notice fluttering after a single espresso. If you suspect caffeine, cutting back for a week or two is the simplest test.
Alcohol is a more reliable culprit. It can directly irritate heart muscle and disrupt the electrical signals that keep your rhythm steady. It also makes you urinate more, which flushes out electrolytes your heart needs to function properly. Cleveland Clinic describes “holiday heart syndrome,” a pattern of short-term irregular rhythm, sometimes atrial fibrillation, that appears a day or two after heavy drinking. Researchers named the condition after noticing a spike in cases every December and January. While holiday heart is typically linked to five or more drinks in a session, even moderate alcohol intake can provoke palpitations in sensitive people.
Nicotine, decongestants containing pseudoephedrine, some asthma inhalers, and stimulant medications can all do the same thing by activating the same adrenaline-driven pathways.
Hormonal Changes in Women
Fluctuating hormone levels are a major and often overlooked cause of palpitations. As many as 54% of women report heart palpitations during menopause. Estrogen influences how the autonomic nervous system regulates heart rate, and the hormonal surges and drops of perimenopause can make the heart more reactive. Many women notice palpitations alongside hot flashes, which makes sense because both are driven by the same hormonal shifts.
Pregnancy and the days just before a period can produce similar effects. If your palpitations seem to follow a monthly pattern or started around the time of other perimenopausal symptoms, hormones are a likely explanation.
Dehydration, Electrolytes, and Sleep
Your heart’s electrical system depends on a precise balance of potassium, magnesium, calcium, and sodium. Anything that throws off those levels, including heavy sweating, vomiting, diarrhea, skipping meals, or simply not drinking enough water, can make premature beats more frequent. This is one reason palpitations are common during illness or after intense exercise.
Sleep deprivation is another underappreciated trigger. Poor sleep raises baseline stress hormones, increases sympathetic nervous system activity, and lowers your threshold for noticing irregular beats. People who work night shifts or have disrupted sleep patterns often report more frequent palpitations.
When Palpitations Point to an Arrhythmia
The vast majority of palpitations are benign premature beats. But some reflect a sustained abnormal rhythm that needs attention. Atrial fibrillation, the most common clinical arrhythmia, feels like a rapid, irregular fluttering that can last minutes to hours. Supraventricular tachycardia (SVT) typically causes a sudden-onset racing heart, often above 150 beats per minute, that starts and stops abruptly. Both are treatable, but they do require a diagnosis.
Certain clues suggest your palpitations may be more than occasional premature beats:
- Duration: Episodes lasting several minutes or longer, rather than a few seconds
- Rapid rate: A heart rate that stays well above 100 beats per minute at rest
- Regularity: Palpitations that come in sustained runs rather than isolated thumps
- Triggers: Episodes that happen during exercise rather than only at rest
Thyroid disorders, anemia, and heart valve problems can also cause palpitations. If yours are new, frequent, or changing in pattern, a basic workup including blood tests and an electrocardiogram can rule out these causes quickly.
How Palpitations Are Diagnosed
The challenge with palpitations is that they tend to come and go, so a standard EKG in a doctor’s office often looks perfectly normal. To catch what’s actually happening during an episode, doctors use portable monitors.
A Holter monitor is a small recording device connected to electrodes on your chest. It continuously tracks your heart rhythm over 24 hours or longer, capturing every beat whether you feel it or not. An event recorder works differently: you wear it for days or weeks, but it only records when you press a button during symptoms. The choice between them depends on how often your palpitations occur. If you’re having daily episodes, a Holter is usually enough. If episodes are sporadic, an event recorder or a longer-term patch monitor gives a better chance of catching one.
Reducing Palpitations on Your Own
Once serious causes have been ruled out, most palpitations respond well to lifestyle adjustments. Cutting back on caffeine and alcohol is the obvious first step. Staying well hydrated and maintaining consistent sleep are simple but effective. Regular aerobic exercise, somewhat counterintuitively, tends to reduce palpitation frequency over time by strengthening parasympathetic tone, the calming side of your nervous system.
For stress-driven palpitations, slow breathing techniques work surprisingly well. Breathing in for four counts and out for six to eight counts activates the vagus nerve, which directly slows heart rate. Practicing this during an episode can sometimes stop palpitations within a minute or two. Regular mindfulness or meditation practice can lower your baseline sympathetic activation, making episodes less frequent overall.
If palpitations are accompanied by chest pain lasting more than a few minutes, dizziness, lightheadedness, fainting, or shortness of breath, those warrant immediate medical evaluation. These combinations can signal that the heart isn’t pumping effectively during the rhythm disturbance, which is a different clinical situation from the harmless premature beat that simply feels unsettling.
