Heart palpitations that appear when you lie down are usually caused by a shift in how blood moves through your body, combined with the simple fact that you’re finally still and quiet enough to notice your heartbeat. In most cases, positional palpitations are harmless, but several specific mechanisms explain why they happen, and understanding them can help you figure out what’s going on.
Why Lying Down Changes Your Heartbeat
When you’re standing or sitting upright, gravity pulls blood toward your legs and feet. The moment you lie down, that blood redistributes toward your chest and heart. Your heart suddenly has more blood to pump with each beat, and it responds by adjusting its rhythm and force of contraction.
Your body manages this through pressure sensors embedded in the walls of major arteries. These sensors detect the increased blood volume returning to the heart and send signals to your brain, which then tells your heart to slow down, speed up, or change how forcefully it contracts. Lying down makes reflux worse because gravity is no longer keeping stomach acid where it belongs. The palpitations tend to be more noticeable after eating, after drinking a lot of water, or when you feel bloated. Lying down redistributes that smaller-than-normal blood volume, and your pressure sensors scramble to adjust. The result can feel like a racing or pounding heart. A few changes can significantly reduce how often you feel palpitations at bedtime. Sleep on your back or right side rather than your left. Elevate your head and upper body slightly, which helps with both blood redistribution and acid reflux. Cut off caffeine by early afternoon and avoid alcohol close to bedtime. Stay well hydrated throughout the day, not just in the evening. If you suspect reflux, avoid eating within two to three hours of lying down.
Deep, slow breathing before bed can help calm your nervous system and reduce the adrenaline-driven awareness cycle. Breathing in for four counts, holding for four, and exhaling for six to eight counts activates the same vagus nerve that reflux irritates, but in a way that slows and steadies your heart rate rather than disrupting it. If your palpitations persist despite these adjustments, keeping a simple log of when they occur, how long they last, and what you ate or drank beforehand gives your doctor far more useful information than a verbal description alone.
