Feeling tired all the time usually comes down to one of three things: your body isn’t getting the fuel it needs, your sleep isn’t as restorative as you think, or a medical condition is quietly draining your energy. Sometimes it’s a combination. The good news is that most causes of persistent fatigue are identifiable and fixable, starting with some straightforward lifestyle factors and, if needed, a simple blood draw.
Your Cells May Not Be Making Enough Energy
Every movement you make, every thought you have, runs on a molecule called ATP, which your cells produce around the clock. Several common deficiencies can disrupt that production line. Iron is one of the most important. It sits at the core of key enzymes inside your mitochondria, the tiny power plants in each cell, where it rapidly flips between two chemical states to shuttle electrons and generate ATP. When iron levels drop, those enzymes slow down. Your muscles produce less energy, your brain gets less oxygen, and you feel it as a heavy, persistent tiredness that rest alone won’t fix.
Iron deficiency is one of the most common nutritional deficiencies worldwide, and it doesn’t always show up as full-blown anemia. Your iron stores (measured by a blood marker called ferritin) can be low enough to cause fatigue long before your red blood cell count actually drops. Women with heavy periods, vegetarians, and frequent blood donors are especially vulnerable.
Vitamin B12 plays a similar behind-the-scenes role in energy metabolism and red blood cell production. Deficiency tends to develop slowly, with tiredness and weakness among the earliest symptoms. Adults over 75 are at higher risk because the body’s ability to absorb B12 from food declines with age, but younger adults can be affected too, particularly those on plant-based diets or taking certain acid-reducing medications.
Sleep That Doesn’t Recharge You
The CDC recommends at least seven hours of sleep per night for adults. But quantity is only half the equation. If your sleep is fragmented, you can spend eight or nine hours in bed and still wake up exhausted.
Sleep apnea is a prime example. Your airway partially or fully collapses during sleep, your oxygen level drops, and your brain jolts you just awake enough to resume breathing. This cycle can repeat five to 30 times per hour, all night long, pulling you out of the deep, restorative stages of sleep without you ever being fully aware of it. The result is severe daytime drowsiness, difficulty concentrating, and the kind of fatigue where you could fall asleep at your desk or behind the wheel. If you snore loudly, wake up with headaches, or your partner has noticed you gasping at night, this is worth investigating.
Even without apnea, poor sleep hygiene chips away at energy. Irregular bedtimes, blue light from screens close to bedtime, alcohol (which fragments sleep architecture even though it makes you drowsy), and a warm bedroom all reduce the proportion of deep and REM sleep you get.
How Stress Drains Your Battery
When you perceive a threat, a region at the base of your brain triggers your adrenal glands to flood your system with adrenaline and cortisol. That’s useful in short bursts. But when stressors are constant, whether from work pressure, financial worry, caregiving, or relationship conflict, that fight-or-flight system stays switched on. Prolonged exposure to elevated cortisol disrupts nearly every system in the body: digestion, immune function, sleep regulation, and mood.
The exhaustion that comes from chronic stress feels different from physical tiredness. It’s often described as feeling “wired but tired,” where your body is buzzing with tension yet you have no energy to do anything. Depression and anxiety, which frequently accompany chronic stress, cause their own brand of fatigue. Depression in particular can make even small tasks feel like they require enormous effort, and it disrupts sleep quality in both directions: some people can’t sleep enough, others can’t stop sleeping.
Blood Sugar Swings and What You Eat
That post-lunch energy crash isn’t just in your head. Postprandial somnolence, the technical term for a “food coma,” happens when a meal high in refined carbohydrates or sugar causes a rapid spike in blood glucose followed by a sharp drop. Your body overproduces insulin to manage the spike, blood sugar plummets, and you’re left foggy and dragging.
Western-style eating patterns heavy in refined grains and added sugars are linked with more daytime sleepiness overall. Switching to smaller, balanced meals that prioritize protein, fiber, and lower-glycemic carbohydrates (think whole grains, legumes, vegetables) smooths out those energy swings. You don’t need to overhaul your diet overnight. Even replacing a sugary breakfast cereal with eggs and whole-grain toast can make a noticeable difference within a few days.
Dehydration Is Easy to Miss
Mild dehydration reduces your blood volume, which means your heart has to pump harder to deliver oxygen and nutrients to your tissues. The result is weakness, fatigue, and dizziness, sometimes before you even feel thirsty. Most people underestimate how much fluid they lose through breathing, sweating, and daily activity, especially in warm environments or during exercise. If your urine is darker than pale yellow, you’re likely not drinking enough.
Thyroid and Other Medical Conditions
Your thyroid gland acts as a thermostat for your metabolism. When it underproduces hormones, a condition called hypothyroidism, everything slows down. Your metabolism drops, you gain weight without changing your eating habits, and you feel exhausted all the time. Hypothyroidism is common, particularly in women over 40, and it’s easily detected with a blood test measuring thyroid-stimulating hormone (TSH).
Other medical conditions that commonly cause fatigue include diabetes (both from blood sugar instability and from the body’s impaired ability to use glucose for energy), heart disease (where the heart can’t pump efficiently enough to meet your body’s demands), kidney disease, celiac disease, and chronic infections. Many of these develop gradually, which makes it easy to normalize the tiredness and chalk it up to aging or a busy schedule.
Chronic Fatigue Syndrome
If your fatigue is profound, started at a specific point in time, has lasted more than six months, and isn’t relieved by rest, it may meet the criteria for myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS). The hallmark symptom is post-exertional malaise: a worsening of symptoms after physical, mental, or emotional effort that wouldn’t have been a problem before the illness. This “crash” typically hits 12 to 48 hours after the activity and can last days or weeks. People with ME/CFS also experience unrefreshing sleep, where a full night’s rest doesn’t reduce their exhaustion, and often have cognitive difficulties like trouble concentrating or processing information. ME/CFS is a clinical diagnosis, meaning there’s no single test for it, but it’s a real, physiological condition, not a sign of laziness or depression.
What Testing Looks Like
If lifestyle changes don’t resolve your fatigue within a few weeks, a doctor will typically start with blood work to rule out the most common culprits. A standard workup includes a complete blood count (to check for anemia), iron studies including ferritin (to catch low iron stores even without anemia), thyroid function tests, fasting glucose (to screen for diabetes), kidney and liver function panels, and markers of inflammation. Many providers also test vitamin B12, folate, vitamin D, and screen for celiac disease. These tests are routine, inexpensive, and can often pinpoint a treatable cause in a single visit.
If blood work comes back normal and sleep quality is a concern, a sleep study may be the next step, particularly if you snore, wake up gasping, or feel unrefreshed despite adequate hours in bed. For fatigue that’s accompanied by mood changes, persistent sadness, or loss of interest in things you used to enjoy, a mental health evaluation can uncover depression or anxiety that’s driving the exhaustion.
