Black spots on teeth usually come from one of a few common causes: early or advancing tooth decay, tartar buildup, or staining from foods, drinks, and certain supplements. About 21% of adults between 20 and 64 have at least one tooth with untreated decay, so if you’re noticing dark discoloration, you’re far from alone. The key is figuring out which type of spot you’re dealing with, because some are cosmetic and some signal damage that’s getting worse.
Tooth Decay Is the Most Common Cause
The most important possibility to rule out is a cavity. Tooth decay starts when bacteria in plaque convert sugars from your food into acids. Those acids strip minerals from your enamel, the hard outer shell of the tooth. In the earliest stage, this shows up as a chalky white spot. If nothing changes, that white spot darkens to brown and eventually black as the enamel continues to break down.
Once a dark spot from decay appears, the damage has typically moved past the surface. Beneath your enamel sits a softer layer called dentin, and acid can spread more quickly through it. A black spot that feels rough, sticky, or sensitive to cold and sweet foods is a strong sign that decay has progressed. At this point, a filling or other professional treatment is the only way to stop it. No amount of brushing will reverse a cavity that’s already formed a dark spot.
Tartar Buildup Near the Gumline
Plaque that isn’t brushed away hardens into tartar (also called calculus) through a process where calcium and phosphate in your saliva mineralize the bacterial film. The tartar you can see above the gumline is usually yellowish or off-white. But tartar that forms below the gumline, called subgingival calculus, is a different story. It typically ranges from dark brown to greenish black.
If your black spots sit right along or just under the edge of your gums, subgingival tartar is a likely culprit. You can’t remove tartar at home once it’s hardened. It requires a professional cleaning where a hygienist scales it off the tooth surface. Left in place, it irritates the gums and contributes to gum disease, so those dark deposits along your gumline are worth addressing sooner rather than later.
Food, Drink, and Tobacco Staining
External stains sit on the surface of the enamel rather than inside it. Coffee, tea, red wine, and dark berries all contain compounds called chromogens that bind to the tooth surface. Tobacco use, whether smoked or chewed, is one of the most aggressive sources of dark staining. Over time, these surface stains can concentrate in the tiny grooves and pits of your teeth, creating spots that look distinctly black rather than uniformly discolored.
The good news is that extrinsic stains don’t mean your tooth structure is damaged. A professional cleaning can remove most of them. The bad news is that they tend to come back if the source (daily coffee, smoking) stays the same. Whitening toothpastes can help with mild surface staining, but they work through mild abrasion, and overly abrasive products can roughen your enamel and actually make future staining worse.
Iron Supplements and Medications
Liquid iron supplements are a surprisingly common cause of black spots on teeth, especially in children and people taking them for anemia. Iron reacts chemically with the tooth surface and with sulfur-containing compounds produced by bacteria in the mouth. The result is a dark, insoluble deposit that stains the teeth black. Teeth with weaker or thinner enamel, a condition called hypomineralization, are especially vulnerable because structural defects in the surface allow the iron to penetrate more easily.
If you’re taking liquid iron and noticing new dark spots, good oral hygiene can help prevent additional staining. Brushing soon after taking the supplement, using a straw to minimize contact with teeth, and keeping up with fluoride toothpaste all reduce the risk. The existing stains typically need to be polished off by a dental professional, but they don’t indicate decay on their own.
Black Staining From Bacteria
Some people develop a distinctive type of dark line or spotting along the gumline caused by chromogenic bacteria. These are specific bacterial species whose metabolic byproducts react with iron in saliva and gum fluid to form a dark, insoluble deposit, likely ferric sulfide. This type of staining is more common in children but can appear in adults too. It tends to recur even after professional removal, and its exact cause isn’t completely understood.
Chromogenic bacterial staining is mostly a cosmetic issue. Interestingly, some research suggests that people with this type of staining may actually have lower rates of cavities, possibly because the bacterial community responsible outcompetes the acid-producing bacteria that cause decay. Still, the dark appearance bothers most people enough to have it cleaned off periodically.
How to Tell What’s Causing Your Spots
A few clues can help you narrow it down before you get to a dentist:
- Location matters. Spots in the grooves and pits on the chewing surfaces of back teeth are classic for decay. Dark lines right at the gumline suggest tartar or chromogenic bacterial staining. Broad discoloration on the front surfaces of teeth points toward food, drink, or tobacco stains.
- Texture matters. Run your tongue over the spot. A rough, sticky, or soft area is more concerning for decay. Smooth, hard discoloration is more likely staining or tartar.
- Sensitivity matters. If a dark spot comes with pain when eating something cold, sweet, or hot, decay has likely reached the deeper layers of the tooth.
What a Professional Cleaning Can and Can’t Fix
Surface stains from food, bacteria, and iron supplements respond well to professional polishing. Tartar requires scaling. Neither of these situations means your tooth is structurally damaged, and treatment is straightforward.
Decay is different. Once a cavity has formed a visible black spot, the damaged tooth structure needs to be removed and replaced with a filling. If the decay has reached the pulp, the innermost layer containing nerves and blood vessels, the treatment becomes more involved. The earlier you address a dark spot caused by decay, the simpler and less expensive the fix.
Why Charcoal Toothpaste Isn’t the Answer
If your first instinct is to try an abrasive or charcoal-based toothpaste, it’s worth knowing that the clinical evidence behind these products is thin. A systematic review found that there isn’t adequate evidence to confirm that activated charcoal is safe or effective for whitening. Highly abrasive toothpastes can roughen the enamel surface, which makes teeth more prone to picking up new stains and potentially weakens the outer layer that protects against decay. A standard fluoride toothpaste does more to protect your teeth than any charcoal product currently on the market.
