First, insulin spikes from high-glycemic foods stimulate hormones that increase oil output on the scalp. Second, sugar and simple carbs promote systemic inflammation, which can make your scalp more reactive. Some dermatologists also point to yeast-containing foods like beer, bread, and wine as potential contributors to fungal overgrowth, though the evidence here is less definitive than for sugar and processed foods.
Infrequent Washing
If you go too long between washes, sebum accumulates on the scalp and creates a richer environment for Malassezia. This doesn’t mean you need to wash daily (more on that below), but finding the right frequency for your hair type matters.
Dandruff vs. Dry Scalp vs. Something Else
People often assume dandruff means their scalp is dry, but it’s usually the opposite. Dandruff flakes tend to be oily, yellowish, and larger, sticking to the scalp or hair. Dry scalp flakes are smaller, white, and fall off easily. Dry scalp also comes with tightness and tends to worsen in winter when humidity drops, while dandruff can flare year-round.
Dandruff and seborrheic dermatitis exist on the same spectrum. Mild flaking without much redness is typically called dandruff. When flaking becomes heavier, with noticeable redness, greasy scales, or patches that extend beyond the scalp to the eyebrows, sides of the nose, or behind the ears, it’s usually classified as seborrheic dermatitis. How Medicated Shampoos Work
The most effective dandruff shampoos target the problem at its source: the Malassezia fungus, the excess skin cell buildup, or both. The American Academy of Dermatology recommends looking for one of these active ingredients:
- Zinc pyrithione attacks Malassezia in multiple ways. It floods the yeast’s cells with toxic levels of zinc, disrupts the energy-producing machinery inside them, and reduces the lipase enzymes the fungus uses to break down scalp oil. That last effect is especially important because fewer lipases mean less oleic acid irritating your scalp.
- Ketoconazole is an antifungal that directly kills Malassezia. It’s available over the counter at 1% strength and by prescription at 2%.
- Selenium sulfide slows the rate at which scalp skin cells multiply, reducing the raw material that forms flakes. It also has antifungal properties.
- Salicylic acid works differently from the others. Rather than targeting the fungus, it softens and dissolves the substance that makes dead skin cells clump together, helping existing flakes shed more easily. It also draws moisture into the outer skin layer, loosening scales.
- Coal tar slows skin cell production and reduces inflammation, though it can make your scalp more sensitive to UV light.
If one ingredient doesn’t work for you after a few weeks of consistent use, try switching to a different one. Some people rotate between two shampoos with different active ingredients for the best results.
How to Use Dandruff Shampoo Effectively
The most common mistake is treating dandruff shampoo like regular shampoo: lathering quickly and rinsing it out. These products need contact time with your scalp to work. Massage the shampoo into your scalp (not just your hair) and leave it on for 5 to 10 minutes before rinsing.
How often you use it depends on your hair type. If you have fine or straight hair, or your scalp tends to be oily, you may benefit from washing daily and using the medicated shampoo twice a week. If you have coarse, curly, or coily hair, once a week is a reasonable starting point. Keep the medicated shampoo on your scalp rather than pulling it through the lengths of your hair, since the active ingredients can be drying to textured hair.

Medicated shampoo handles the immediate symptoms, but addressing the underlying triggers can reduce how often dandruff comes back. Cutting back on sugar and refined carbohydrates may lower both inflammation and excess oil production. Managing stress through whatever works for you, whether that’s exercise, sleep, or simply reducing your workload, can blunt the cortisol-driven oil surges that feed Malassezia.
It’s worth accepting that dandruff is a chronic, recurring condition for most people rather than something you cure once. The fungus that causes it is a permanent resident of your skin. The goal is management: finding the right shampoo, the right wash frequency, and the lifestyle balance that keeps flaking minimal. Most people can get dandruff fully under control with over-the-counter products alone, and flare-ups that do break through usually respond quickly once you restart treatment.
