Dark patches around the mouth usually result from excess melanin production triggered by one or more overlapping factors: hormonal changes, sun exposure, skin irritation, or nutritional gaps. The good news is that once you identify the cause, perioral hyperpigmentation responds well to targeted treatment, with most people seeing meaningful improvement within three to six months.
Melasma: The Most Common Culprit
Melasma is the leading cause of dark patches on the face, and the upper lip and skin surrounding the mouth are among its favorite locations. It appears as irregular, brownish discoloration and occurs most often in women in their 20s and 30s. The driving forces are hormonal: estrogen and progesterone increase the activity of pigment-producing cells in the skin, making them more reactive to UV light. This is why melasma frequently shows up during pregnancy (sometimes called “the mask of pregnancy”), while taking oral contraceptives, or during hormone replacement therapy.

Your Toothpaste Might Be Part of the Problem
This one surprises most people. Sodium lauryl sulfate (SLS), the detergent that makes toothpaste foam, is a known irritant to the delicate skin around the mouth. That low-grade irritation can trigger inflammation, and inflammation leads to post-inflammatory hyperpigmentation, the dark marks your skin leaves behind after it heals.
Flavorings are the other major offender. Mint derivatives like spearmint, peppermint, and menthol are the most frequent causes of allergic contact reactions from toothpaste. Cinnamon-derived flavoring (cinnamal) has also been linked to perioral skin changes. Fluoride salts can occasionally cause reactions too. If the darkening around your mouth appeared gradually without an obvious hormonal trigger, switching to an SLS-free, unflavored toothpaste for a few weeks is a simple way to test whether your oral care products are contributing.
Lip Licking and Repeated Irritation
Habitual lip licking creates a cycle that’s hard on the skin. Saliva contains digestive enzymes designed to break down food, and when those enzymes sit on the skin around your lips repeatedly, they strip away the protective barrier. The skin dries out, cracks, and becomes more vulnerable to irritants. Your instinct is to lick again for relief, which restarts the cycle.
Over time, this chronic irritation can progress to irritant contact dermatitis, angular cheilitis (cracking at the corners of the mouth), and secondary infections. Each round of inflammation deposits more melanin into the skin. The resulting ring of darkened skin often traces the exact path your tongue reaches, which is a telltale sign.
Vitamin B12 Deficiency
Low vitamin B12 levels can cause hyperpigmentation, and the skin around the mouth is one of the areas where it tends to show up. The mechanism involves an enzyme called tyrosinase, which controls melanin production. When B12 is deficient, tyrosinase activity increases, causing the skin to produce more pigment than normal. This is a less common cause than melasma or irritation, but it’s worth considering if you follow a plant-based diet, have digestive conditions that affect nutrient absorption, or notice other symptoms like fatigue, tingling in your hands or feet, or a sore tongue.
Addison’s Disease and Adrenal Insufficiency
Darkening around the mouth, especially in the creases and on the lips themselves, can be an early sign of adrenal insufficiency. In Addison’s disease, the adrenal glands don’t produce enough cortisol. The body compensates by ramping up a hormone that, as a side effect, stimulates melanocytes throughout the skin. The pigmentation tends to appear in skin folds, pressure points, and mucous membranes, making the perioral area a common location. This is a rarer cause, but if your darkening is accompanied by unexplained fatigue, weight loss, low blood pressure, or salt cravings, it warrants blood work.
Why Sun Protection Alone Isn’t Enough
Standard sunscreens block ultraviolet light but do very little against visible light, the kind that comes from the sun, screens, and overhead lighting. Visible light makes up about 45% of solar radiation and actively worsens skin darkening, particularly in medium to deep skin tones (Fitzpatrick skin types III and higher). This is why some people are diligent about sunscreen and still see their hyperpigmentation persist or worsen.
The fix is a tinted sunscreen containing iron oxides. Iron oxides absorb, scatter, and reflect visible light in a way that clear sunscreens simply cannot. If you’re treating perioral hyperpigmentation with any method, a tinted broad-spectrum sunscreen is the single most important supporting step. Without it, UV and visible light will keep stimulating melanin production and undermine whatever else you’re doing.
Topical Treatments That Work
Several ingredients have solid evidence for fading hyperpigmentation around the mouth. Hydroquinone at concentrations of 2 to 4% remains the most well-studied option and is available both over the counter (2%) and by prescription (4%). It works by suppressing the enzyme that produces melanin. Kojic acid, derived from fungi, is used at 2 to 4% and is a gentler alternative, sometimes combined with hydroquinone for a stronger effect.
Retinoids (vitamin A derivatives) can reduce dark spots by roughly 64% over three to six months by speeding up skin cell turnover. Alpha-hydroxy acids and beta-hydroxy acids work on a similar principle, exfoliating the pigmented surface layers to reveal fresher skin beneath. With regular use, these show significant improvement in two to six months.
The skin around the mouth is thinner and more reactive than the rest of the face, so starting with lower concentrations and applying every other night helps avoid the irritation that could make pigmentation worse.
Professional Procedures
When topical products aren’t enough, dermatologists can offer procedures that speed up results. Superficial chemical peels use mild acids to remove only the outermost layer of skin and are the gentlest option for the perioral area. Medium-depth peels penetrate deeper and target more stubborn discoloration. Professional chemical peels can produce significant results in about 68 days on average.
Laser therapy is another option, with an average clearance timeline of around 140 days. Microneedling typically shows results in two to four months. One important caution: chemical peels and laser treatments carry a risk of worsening pigmentation, particularly in darker skin tones or in people with active hormonal triggers. Sun exposure and smoking after a peel significantly raise the risk of complications including scarring and further color changes.
How Long Fading Takes
Patience is the hardest part. Post-inflammatory hyperpigmentation can last up to 21 months if left completely untreated. With the right approach, the timeline shrinks dramatically. Prescription-strength treatments typically show improvement in 6 to 12 weeks. Over-the-counter products take longer, generally 12 to 24 weeks. Dark spot correctors with active ingredients like vitamin C or niacinamide can make a visible difference in about 12 weeks.
The single biggest factor in how quickly your skin clears is consistency with sun protection. Even one unprotected afternoon can re-trigger melanin production and set back weeks of progress. A tinted SPF 30 or higher, applied every morning and reapplied during prolonged sun exposure, is non-negotiable for anyone actively treating hyperpigmentation around the mouth.
