Nail ridges are extremely common, and in most cases they’re a harmless sign of aging. Your nails grow from a cluster of cells called the nail matrix, tucked just beneath your cuticle. That matrix produces roughly 196 layers of cells that stack together to form each nail plate. When anything disrupts that layering process, whether it’s normal wear and tear over decades or a temporary health event, the result shows up as visible ridges on the nail surface.
The direction of your ridges matters. Vertical ridges (running from cuticle to tip) are almost always benign. Horizontal ridges (running side to side) are more likely to signal that something interrupted your nail growth from the inside.
Vertical Ridges and Aging
Vertical ridges, sometimes called longitudinal striations, are the most common type. They run from the base of your nail to the free edge, and they tend to become more noticeable after your 30s and 40s. These ridges are a normal part of aging, similar to how skin loses smoothness over time. The nail matrix gradually becomes less efficient at producing perfectly uniform layers of cells, and the subtle unevenness creates visible lines on the nail surface.
Not everyone develops them at the same rate. Genetics, how well-hydrated you are, and how much wear your hands endure all play a role. Vertical ridges on their own, without color changes or pain, rarely point to a medical problem.
When Ridges Run Sideways
Horizontal ridges are a different story. Known clinically as Beau’s lines, these are indentations that run across the nail from side to side. They form when something temporarily halts or slows new cell production in the nail matrix. Because fingernails grow about 3 to 4 millimeters per month, you can often estimate when the disruption happened by measuring how far the ridge has grown out from the cuticle.
A wide range of events can trigger Beau’s lines:
- Acute illness or high fever: COVID-19, pneumonia, measles, mumps, and strep infections can all cause them.
- Major health events: A heart attack or severe physical trauma can pause nail growth long enough to leave a mark.
- Chronic conditions: Diabetes, hypothyroidism, peripheral artery disease, and Raynaud’s phenomenon interfere with blood flow to the nail matrix over time.
- Nutritional gaps: Severe zinc deficiency or inadequate protein intake.
- Skin conditions: Eczema and psoriasis affecting the fingers can disrupt the matrix directly.
- Severe emotional stress: Significant life events like a death in the family, divorce, or job loss can affect nail growth enough to produce horizontal ridges.
- Chemotherapy: Cancer treatment commonly interrupts the rapidly dividing cells of the nail matrix.
If you notice horizontal ridges on multiple nails at the same position, that typically points to a systemic cause, something that affected your whole body at once. A single nail with a horizontal ridge is more likely the result of local trauma, like catching your finger in a door.
Nutrient Deficiencies That Affect Nails
Iron deficiency is one of the most well-documented nutritional causes of nail changes. Low iron can produce vertical ridges, and in more advanced cases, nails may become thin and scoop upward at the edges (a condition called spoon nails). If your ridges appeared alongside fatigue, pale skin, or shortness of breath, iron levels are worth checking with a simple blood test.
Zinc deficiency can also contribute, particularly to horizontal ridges and brittle texture. If a blood test confirms a deficiency, supplementing with iron or zinc often improves nail quality over several months as new nail grows in. Since fingernails take about six months to fully replace themselves, don’t expect overnight changes.
Skin Conditions and the Nail Matrix
Psoriasis and lichen planus can both affect the nails, sometimes before any rash appears on the skin. Nail psoriasis often causes small pits (tiny dents scattered across the surface), but it can also produce longitudinal ridges, splits along those ridges, and Beau’s lines. The nails may thicken, crumble at the edges, or separate from the nail bed underneath.
If your ridges are accompanied by pitting, discoloration, or nails that seem to be lifting away from the skin, a skin condition affecting the matrix is a likely explanation. These changes can look similar to a fungal nail infection, so getting the right diagnosis matters for choosing the right treatment.
Nail Changes Linked to Organ Disease
Certain nail patterns are associated with chronic kidney or liver disease, though these tend to involve color changes more than simple ridging. In chronic kidney disease, nails may turn white on the half closest to the cuticle and pink or brown on the outer half. Up to one-third of people on dialysis develop this pattern. The visible half-moon at the base of the nail may also disappear entirely.
In liver disease, particularly cirrhosis, nails can turn mostly white with a normal-colored strip at the tip. This pattern also shows up in congestive heart failure and advanced diabetes. These aren’t changes you’d easily confuse with ordinary ridges. They involve dramatic shifts in nail color across distinct zones.
Warning Signs Worth Investigating
Most nail ridges don’t need medical attention. But certain changes warrant a closer look because they can, in rare cases, signal something serious like melanoma under the nail. Be alert for:
- A new dark brown or black streak running vertically through the nail, especially in fair-skinned individuals
- A pigmented band that is darkening, widening, or bleeding over time
- A dark band wider than 6 millimeters, or one that is wider near the cuticle than at the tip
- Dark pigment that spills onto the skin around the nail (known as the Hutchinson sign)
- Any nail ridge or streak accompanied by ulceration, crumbling, or destruction of the nail plate
These features are uncommon, but they deserve prompt evaluation. Melanoma of the nail bed is treatable when caught early.
Improving the Appearance of Ridged Nails
If your ridges are the harmless vertical type, a few simple habits can reduce their appearance and keep nails healthier. Soaking your nails in water for about five minutes, then applying a lotion containing lanolin or alpha-hydroxy acids, helps lock in moisture and smooth the nail surface over time. Keeping cuticles moisturized is especially important because the cuticle protects the matrix underneath.
Nail polishes and hardeners containing nylon or hyaluronic acid can temporarily fill in ridges and add strength to soft, brittle nails. Avoid peeling off gel polish or aggressively buffing ridges down to a smooth surface, as both can thin the nail plate and make ridging worse in the long run. Gentle buffing is fine, but the goal is to smooth lightly rather than grind the nail flat.
For ridges caused by a nutritional deficiency, correcting the underlying shortage is the only real fix. New, smoother nail will gradually grow in from the base, pushing the ridged portion forward until you can trim it away. Expect the full transition to take four to six months for fingernails and closer to a year for toenails.
