Sunken eyes usually come down to one of a few things: natural aging, genetics, dehydration, or lifestyle factors like poor sleep. In most cases, the hollowed look isn’t a sign of something dangerous. It happens when the fat and bone that normally support the area around your eyes thin out, shift, or lose volume, making the eye socket appear deeper and darker.
Understanding which factor is driving your particular case helps you figure out whether it’s something you can change, something cosmetic you can address, or something worth bringing up with a doctor.
How the Eye Area Loses Volume
Your eyes sit in bony sockets surrounded by small pads of fat, thin skin, and connective tissue. That fat acts as cushioning, and the skin over it is some of the thinnest on your entire body. When any part of this system loses support, the result is a visible hollow, often called a “tear trough,” that runs from the inner corner of your eye downward across the upper cheek. The shadow cast by this groove is what most people perceive as dark circles or a sunken look.
With age, several things happen at once. The bones of the eye socket gradually resorb, creating a slightly larger opening. The fat pads that once kept the area smooth either shrink or slide downward. Skin loses elasticity. These changes tend to accelerate after age 50, when fat volume in the under-eye area increases but its density drops, causing tissue to sag and bulge in some spots while leaving hollows in others. The overall effect is a tired, aged appearance even when you feel fine.
Genetics and Bone Structure
Some people are simply born with deeper-set eyes. The shape of your skull, the depth of your eye sockets, and how much natural fat padding you carry around your eyes are all inherited traits. If your parents or grandparents had prominent hollows under their eyes, you’re more likely to as well, sometimes as early as your twenties or even your teens.
Hereditary pigmentation plays a role too. Families often share a tendency toward darker skin in the under-eye area, which amplifies the sunken appearance. This pigmentation runs in generations and is especially common in people with deeper skin tones. Combined with a naturally concave contour around the eyes, even mild pigmentation can make the hollows look more dramatic than they are.
Dehydration and Sleep Loss
Dehydration is one of the fastest ways to make your eyes look sunken. When your body is low on fluids, the thin skin under your eyes loses what little plumpness it has, making the socket more visible. You’ll often notice this after a night of drinking alcohol, during illness with vomiting or diarrhea, or simply after not drinking enough water on a hot day. The fix here is straightforward: rehydrate, and the hollows typically improve within hours to a day.
Sleep deprivation works differently but produces a similar look. Poor sleep dilates blood vessels under the eyes and causes fluid to pool unevenly, leaving you with a combination of puffiness and dark shadows that makes the orbital area look hollow and discolored. Chronic sleep loss compounds the problem over time because the skin’s repair processes depend heavily on rest.
Allergies and Sinus Congestion
If your sunken, dark eyes tend to flare up seasonally or alongside a stuffy nose, allergies may be the culprit. Nasal congestion from allergic or non-allergic rhinitis slows blood flow through the tiny veins beneath your lower eyelids. That venous stasis causes fluid to accumulate in the groove under the eye, producing a blue-gray to purple discoloration known as “allergic shiners.”
The darkness isn’t bruising. It’s pooled blood showing through extremely thin skin. When the congestion clears, whether from treating the allergy or waiting out a cold, the discoloration and hollow appearance typically fade. People with chronic, year-round allergies often have persistent shiners that they mistake for a permanent feature of their face.
Weight Loss and Low Body Fat
Rapid or significant weight loss is a common and often overlooked cause. The fat pads around your eyes respond to overall body fat levels. When you lose a substantial amount of weight, especially quickly, the face loses volume before many other areas. People who drop weight through illness, restrictive dieting, or intense training sometimes notice their eyes look dramatically more hollow even though the rest of their body looks healthy.
This is also why sunken eyes can be an early visible sign of malnutrition or eating disorders. If the hollowing appeared alongside unintentional weight loss, that’s worth investigating with a healthcare provider.
Medical Conditions That Cause Sunken Eyes
Most sunken eyes are cosmetic, but a few medical conditions can cause a more pronounced version called enophthalmos, where the eyeball itself sits further back in the socket than normal.
Trauma is the most common medical cause. Car accidents, sports injuries, or any blow to the face that fractures the thin bones of the eye socket can allow the eye to sink backward. This is usually obvious because it follows an injury and often affects only one side.
A less obvious condition called silent sinus syndrome can also cause one eye to gradually sink. In this condition, chronic disease in the maxillary sinus (the large sinus beneath your cheekbone) weakens the floor of the eye socket, causing it to bow downward. People with silent sinus syndrome often don’t have typical sinus symptoms, which is why it’s called “silent.” The change in eye position develops slowly and is almost always on one side only.
Other causes of enophthalmos include age-related bone changes, certain vascular problems, and conditions that affect facial bone development. If only one eye appears sunken, if the change happened suddenly, or if you’re experiencing double vision or difficulty moving your eye, those are signs that something beyond normal aging or genetics is going on.
What Actually Helps
The right approach depends entirely on the cause. For dehydration, sleep loss, and allergies, the solution is treating the underlying problem. Staying well-hydrated, improving sleep habits, and managing nasal congestion with appropriate allergy treatment can noticeably reduce the hollowed look.
For genetic or age-related hollowing, topical products have limited impact. Caffeine-based eye creams are widely marketed, but research tells a less encouraging story. In one controlled trial of 34 volunteers, a caffeine gel performed no better than a plain gel base at reducing under-eye puffiness. The cooling sensation of any gel applied to the area provided the same temporary improvement, suggesting it’s the cold, not the caffeine, doing the work.
Cold compresses or chilled spoons can temporarily reduce puffiness and constrict blood vessels, making hollows less noticeable for a few hours. Retinol-based products may modestly thicken the skin over months of use, but they won’t replace lost fat volume.
Dermal Fillers
For people bothered enough to seek a cosmetic fix, hyaluronic acid fillers injected into the tear trough are the most common professional treatment. The procedure involves small amounts of gel, typically around 0.45 mL per side, placed beneath the skin to restore the volume that fat and bone have lost. Results last longer than many people expect. While the subjective effect averages about 11 months, three-dimensional imaging shows measurable volume improvement lasting over 14 months, with some studies demonstrating significant results up to 18 months after a single treatment.
Fillers in the under-eye area carry specific risks, including visible lumps, bluish discoloration from the product showing through thin skin, and, rarely, vascular complications. This is a technically demanding area to inject, so the skill of the practitioner matters more here than in almost any other filler location.
One Eye vs. Both Eyes
Symmetry is an important clue. If both eyes look equally sunken, the cause is almost certainly aging, genetics, dehydration, or lifestyle. If only one eye appears hollow or has changed position, that points toward something structural: a past fracture you may have forgotten, silent sinus syndrome, or another condition affecting the bone or tissue on that side. Asymmetric changes warrant a medical evaluation, particularly if they’ve developed gradually without an obvious explanation.
