When this pathway works well, your body clears small fungal colonies before they cause symptoms. When it doesn’t, fungi like Candida can grow unchecked. People with genetic variations affecting this Th17 pathway can develop chronic, recurring infections from childhood onward. But more commonly, the immune suppression is acquired: from medications, chronic illness, stress, or poor sleep.

Medical Conditions That Fuel Recurrence

Poorly controlled diabetes is one of the most common drivers of repeat fungal infections. Elevated blood sugar creates a favorable environment for fungi and impairs immune cell function. Research in diabetic patients found that an HbA1c level above 8.9% (a marker of average blood sugar over three months) was associated with fungal growth with about 80% reliability. If your blood sugar has been running high, that alone can explain why infections keep coming back, and getting your glucose under better control is often the single most impactful change you can make.

Other conditions that raise your risk include HIV, cancer, and chronic lung disease. Each of these either directly weakens immune function or requires treatments (like chemotherapy or long-term corticosteroids) that suppress the immune system. Even conditions you might not associate with infection risk, like thyroid disorders or chronic stress, can dampen immune responses enough to tip the balance in favor of fungal growth.

Medications That Shift the Balance

Antibiotics are a frequent and underappreciated trigger. They kill bacteria indiscriminately, including the beneficial bacteria that normally compete with fungi for space and resources on your skin, in your gut, and in the vaginal tract. After a course of antibiotics, fungi like Candida face less competition and can proliferate rapidly. If you notice a pattern of fungal infections following antibiotic use, that connection is worth discussing with your doctor.

Oral corticosteroids and inhaled steroids (commonly used for asthma) suppress local immune defenses. Immunosuppressant drugs taken after organ transplants or for autoimmune conditions have a similar effect. Hormonal contraceptives can also influence vaginal yeast infections by altering the local environment, though this varies significantly from person to person.

Why Infections Survive Treatment

One reason fungal infections return is that they were never fully eliminated in the first place. Fungi, particularly Candida species, can form biofilms: structured communities of cells encased in a protective matrix that shields them from both your immune system and antifungal medications. These biofilms are a major cause of chronic, persistent infections and drug resistance.

The protective matrix physically blocks antifungal drugs from reaching the cells inside. Some cells within the biofilm enter a dormant state, making them nearly impervious to medications that target actively growing fungi. If you meet that threshold, the approach to treatment changes. Rather than treating each episode individually, doctors typically recommend a longer suppressive regimen to keep fungi from re-establishing.

Getting the right diagnosis also matters more than you might expect. The standard microscopy test (KOH preparation) used to diagnose skin and nail fungal infections has a sensitivity of about 80 to 88%, meaning it misses roughly one in five to one in eight cases. Fungal culture, often considered more definitive, actually has a lower sensitivity of around 59%, though its specificity is higher at 82%. This means that if you’ve been told a test was negative but your symptoms persist, a repeat test or a different testing method may be warranted. Identifying the exact fungal species is important because some species are naturally resistant to common antifungal drugs.

Environmental and Lifestyle Factors

Fungi thrive in warm, moist environments. Wearing tight, non-breathable clothing, sitting in damp workout gear, or keeping skin folds consistently moist creates ideal conditions for fungal growth. This applies to athlete’s foot, jock itch, yeast infections, and intertrigo (infection in skin folds) alike.

Practical changes that reduce moisture make a real difference: switching to breathable cotton underwear, changing out of sweaty clothes promptly, drying skin folds thoroughly after bathing, and using moisture-wicking fabrics during exercise. For people prone to infections in skin folds, applying a thin layer of antifungal powder to dry skin can help prevent recurrence. These measures won’t overcome a significant immune or metabolic problem on their own, but they remove one of the conditions fungi need to flourish.

Diet plays a less direct but still relevant role. High sugar intake can raise blood glucose levels, even in people without diabetes, providing fuel for fungal growth. Alcohol consumption, particularly heavy drinking, suppresses immune function and can disrupt the gut microbiome in ways that favor Candida overgrowth.

The Role of Probiotics

Probiotics have gained attention as a way to prevent recurrent fungal infections, particularly vaginal yeast infections. A recent meta-analysis of randomized controlled trials found that probiotics used alongside standard antifungal treatment significantly improved cure rates and reduced recurrence compared to antifungal treatment alone. Two strains in particular, Lactobacillus rhamnosus GR-1 and Lactobacillus reuteri RC-14, have the most research behind them.

However, the picture is nuanced. Probiotics alone showed some benefit over placebo for long-term clearance of recurrent infections, but antifungal medications still achieved higher long-term cure rates when compared head to head. The current evidence suggests probiotics work best as a complement to antifungal treatment, not a replacement. The optimal dose and duration haven’t been clearly established, so results can vary depending on the product and how it’s used.

Breaking the Cycle

If you’re dealing with repeat fungal infections, the most productive first step is identifying whether an underlying condition is involved. Blood sugar testing (fasting glucose or HbA1c) is a reasonable starting point, especially if infections are vaginal or affect skin folds. If you’re on medications that suppress the immune system or have recently taken antibiotics, that context helps explain the pattern.

For infections that keep returning despite treatment, ask about longer suppressive therapy rather than repeated short courses. Make sure the infecting species has been identified through culture, since non-standard species sometimes require different medications. Address environmental factors like moisture and clothing, add a well-studied probiotic strain if you’re dealing with vaginal candidiasis, and pay attention to blood sugar even if you haven’t been diagnosed with diabetes. Recurrent fungal infections are frustrating, but they’re almost always traceable to a specific, correctable cause.
